This Task Assignment (TA) DC015D is a contract document executed between the State of Florida Department of Environmental Protection (FDEP) and GHD Services, Inc., effective October 27, 2025, for annual groundwater monitoring services at the International Professional Cleaners site located at 10420 NW 7th Ave, Miami-Dade County, Miami 33150. The project is designated as ERIC_4365 and is authorized under the underlying Contract Number HW684. The Department Project Manager is Mackenzie Childs (850-245-8929), and the contractor's Project Manager is Brian Moore (813-971-3882). The contract encompasses three subtasks valued at a total of $5,888.28 on a fixed unit rate basis. Subtask 1 involves project management calculated at 13% of direct labor costs. Subtask 2 covers groundwater monitoring including sampling of monitoring wells MW009, MW010, and MW018 using volatile organic halocarbon (VOH) analysis via EPA Method 8260, depth-to-water measurements from all 11 wells, and preparation of a comprehensive summary report with field methodology and analytical results. Subtask 3 addresses PFAS sampling at select wells (MW009, MW018, and MW020) with samples submitted to FDEP's laboratory in Tallahassee, with results incorporated into the summary report. Invoicing is scheduled upon FDEP review and approval of deliverables: the Groundwater Monitoring Report ($5,080.82) due January 8, 2026, and PFAS field notes and laboratory data ($807.46) due January 8, 2025. The cost estimate includes detailed labor allocations across multiple staff categories, vehicle costs of $120.00, laboratory analytical costs of $277.50, subcontractor costs of $137.50 (including $125.00 for IDW disposal through Erwin Remediation), and field equipment expenses totaling $464.20.

COST PROPOSAL FOR TASK DC015D
<International Professional Cleaners, ERIC_4365 >
Groundwater Monitoring C
1. INTRODUCTION:
Active remediation was conducted at this site between July 2004 and early 2016. Given the uncertainty associated with the location of any remaining subsurface contaminant mass and the variation in mass recovery rates through SVE, the value associated with continued system operations is unclear. As such, and based on recent discussions with the Department, system shutdown and periodic groundwater monitoring was recommended for the subject site. Given that the subject facility is currently actively drycleaning with PCE, periodic monitoring was recommended until drycleaning operations and/or the use of PCE is discontinued. Task DC015C includes annual groundwater monitoring in accordance with GHD recommendations (Report dated August 16, 2024) and FDEP approval.
Subtask 1: Project Management Subtask 1 includes all activities associated with the completing annual groundwater sampling, analysis, and reporting for the International Professional Cleaners Site.
Subtask 2: Groundwater Monitoring
Groundwater sampling will be conducted in October 2025. Monitoring wells MW009, MW010, and MW018 will be sampled with volatile organic halocarbon (VOH) analysis using EPA Method 8260. During mobilization for sampling, depth to water measurements will be gathered from all wells (11 total). Groundwater sampling will be conducted in general accordance with FDEP SOPs.
Following groundwater sampling and receipt of analytical data, a summary report will be prepared. The summary report will include a description of field sampling methodology and analytical results. The report will also include recommendations regarding the next step toward site rehabilitation.
Subtask 3: PFAS Sampling As part of subtask 3, GHD will conduct PFAS sampling at select monitoring wells including MW009, MW018, and MW020. PFAS sampling will be conducted in accordance with FDEP and GHD SOPs. The PFAS samples will be submitted to FDEP’s laboratory in Tallahassee for analysis.
Upon receipt of analytical data, the PFAS results will be incorporated into the summary report described under subtask 2.
