SUBJECT: Task Assignment Summary

HW684 DC012B

Construction

This Task Assignment authorizes GHD Services, Inc. (GHD) to oversee the installation of remedial injection wells, conduct remedial injections, and conduct baseline and performance groundwater sampling. Specific subtasks are detailed below:

Subtask 1 - Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2 - Injection Well Installation GHD will oversee the installation of remedial injection wells within the subject facility and to the west. In total, three (3) interior vertical injection wells, ten (10) exterior vertical injection wells, and four (4) exterior angled injection wells will be installed. The wells will be installed using direct-push technology to depths of 24, 43, or 44 feet bls as specified in the RAP. The wells will be completed in flush-mounted stainless steel manhole covers. Following installation, the remedial injection wells will be developed and the development water and cuttings will be containerized for off-site disposal. Upon completion of well installation activities, field notes and well construction permits/logs will be submitted to the FDEP.

Subtask 3 - Baseline Groundwater Sampling As proposed in the approved RAP, GHD personnel will conduct baseline groundwater sampling. Prior to groundwater sampling, a full round of depth to water measurements will be gathered from all available monitoring wells (49 total). Following depth-to-water measurements, groundwater sampling will be conducted at a total of forty-two wells (42) including 32 monitor wells and 10 injection wells as specified in the RAP. Groundwater sampling will be conducted in accordance with FDEP standard operating procedures.

Groundwater samples will be analyzed for the presence of volatile organic halocarbons, total organic carbon, total dissolved solids, total recoverable petroleum hydrocarbons, sodium, ammonia, nitrate, molybdenum, chloride, sulfate, iron, and/or dissolved gases per the RAP. Upon receipt of analytical data, the field notes and laboratory data will be provided to the FDEP along with recommendations for RAP implementation.

Subtask 4 - Remedial Injections Following baseline sampling, substrate injections will commence. The injections will include the selected substrate (EOSPRO) along with selected nutrients. In accordance with the design, 3,080 to 3,115 gallons of solution will be injected into each well. A total of 880 gallons of EOSPRO substrate is specified. In addition to substrate, nutrients, and dechlorinating microbes will be injected into the subsurface. A total of 17 liters of dechlorinating culture is specified.

Subtask 5 - Quarter 1 Performance Groundwater Sampling Approximately 3 months following the injections, groundwater performance monitoring will be conducted. Groundwater sampling will be conducted at ten (10) monitor wells including: MW001, MW006, MW029, MW031, MW032, MW033, MW007, MW024, MW025, and MW028. A full round of water levels will not be gathered during this sampling event. Samples will be analyzed for the presence of volatile organic halocarbons, total organic carbon, total dissolved solids, total recoverable petroleum hydrocarbons, sodium, ammonia, nitrate, molybdenum, chloride, sulfate, iron, and/or dissolved gases per the RAP. Following receipt of analytical data, an injection summary and performance monitoring report (Construction Completion Report) will be prepared.

The report will include a summary of injection activities along with a summary and evaluation of analytical data. The report will also include recommendations for additional testing and/or remediation.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Subcontractors were chosen on a lowest-cost and availability basis.

The Florida Department of Environmental Protection (FDEP) has a current and valid site access agreement for this site, signed December 31, 2013.

Documentation of the costs is included in the attached CEF, Level-of-Effort sheets, and the GHD proposal. The final deliverable due date is March 21, 2023, and the completion date for the Task Assignment is June 23, 2023.

The final cost total for Task Assignment DC012B is $165,548.46.

COST PROPOSAL FOR TASK DC012B

Nu-Way Cleaners Inc., ERIC_4701

Construction (Injection well installation, Baseline Sampling, Injections, Groundwater Monitoring)

1. INTRODUCTION:

A Remedial Action Plan (RAP) was submitted for the Nu-Way Cleaners facility located in Plant City, Hillsborough County, Florida. The RAP proposed well installation and remedial injections for groundwater treatment. The RAP was approved in January 2022. Task DC012B includes costs associated with RAP implementation.

Subtask 1-Project Management Sub Task 1 includes all tasks associated with implementation of well installation, baseline sampling, remedial injections, and one quarter of groundwater monitoring and reporting.

Subtask 2-Injection Well Installation As specified, GHD will oversee the installation of remedial injection wells within the subject facility and to the west. In total, three (3) vertical interior injection wells, ten (10) exterior vertical injection wells, and four (4) exterior angled injection wells will be installed. The wells will be installed using direct-push technology to depths of 24, 43, or 44 feet bls as specified in the RAP. The wells will be completed in flush-mounted stainless steel manhole covers.

Following installation, the remedial inject wells will be developed and the development water and cuttings will be containerized for off-site disposal.

Upon completion of well installation activities, field notes and well construction permits/logs will be submitted to the FDEP.

Subtask 3-Baseline Groundwater Sampling As proposed in the approved RAP, GHD personnel will conduct baseline groundwater sampling.

Subtask 4 - Remedial Injections

Following baseline sampling, substrate injections will commence.

Subtask 5-Quarter 1 Performance Groundwater Sampling Approximately 3 months following the injections, groundwater performance monitoring will be conducted. Groundwater sampling will be conducted at ten (10) monitor wells including:

MW001, MW006, MW029, MW031, MW032, MW033, MW007, MW024, MW025, and MW028.
