Task Assignment Summary

HW684 TA DC003B

Operations & Maintenance (O&M)

GHD Services, Inc. (GHD) will conduct 12 months of O&M. All work will be conducted in accordance with the scope of work and cost proposal submitted on March 10, 2023.

Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Six Months O&M (April 2023 through September 2023) GHD will mobilize to the site and attempt to locate interior monitor well MW014. If located, the well will be sampled in accordance with FDEP SOPs and the sample analyzed for volatile organic halocarbons by EPA Method 8260. GHD will conduct semi-annual site visits to verify system operations. During semi-annual site visits, GHD personnel will measure vacuums and flows and also collect one vapor discharge sample to verify compliance. Upon receipt of the analytical data, GHD will provide an email with field sampling logs and laboratory data to the Department. Recommendations will be included in the email based on the results.

Subtask 2: Six Months O&M (October 2023 through March 2024) GHD will conduct semi-annual site visits to verify system operations. During semi-annual site visits, GHD personnel will measure vacuums and flows and also collect one vapor discharge sample to verify compliance. GHD will also install a shallow monitoring well to the east of the site and Buckhorn Creek. The monitor well will be installed by-hand to a maximum depth of 10 feet and will be constructed with 5 feet of 1-inch diameter prepack well screen and blank casing. The well will be finished inside a boltdown steel manhole.

Following installation, the well will be surveyed relative to the existing well network. GHD will then conduct annual groundwater monitoring at six site monitoring wells in accordance with recent recommendations. GHD will conduct annual groundwater sampling in May/June 2023. The groundwater monitoring will consist of sampling deep monitor wells (MW018 and MW004) and shallow monitor wells (MW011, MW015, MW016, and MW019). Prior to sampling, a complete set of depth to water measurements will be gathered. Sampling will be conducted in accordance with FDEP SOPs. The groundwater samples will be analyzed for the presence of volatile organic compounds (BTEX and VOHs) by EPA Method 8260. The laboratory analytical results for the EPA Method 8260 parameters and the groundwater flow data will be analyzed and assimilated into a Remedial Action Status Report (RASR) describing contaminant trends, natural attenuation characteristics, and groundwater flow patterns, with recommendations. A RASR will be prepared that summarizes the six months of system operation, analytical data, and evaluation of data trends. The RASR will also include recommendations for additional system operations and/or monitoring.

Subtask 4: Utilities (12 months) Subtask 4 includes twelve months of utility charges (March 2023 through mid-February 2024). The utility charges are estimated at $55 per month.

Subcontractors for well installation and investigation derived waste (IDW) disposal were chosen on a lowest-cost basis.

The subject property sold in October of 2022. GHD has been given labor hours to negotiate site access and obtain a signed site access agreement from the current property owner prior to conducting any other work under this Task Assignment.

Documentation of the costs is included in the attached CEF, Level-of-Effort sheets, and the GHD proposal. The final deliverable due date is April 26, 2024, and the completion date for the task assignment is July 26, 2024.

The final cost total for Task Assignment DC003B is $12,902.95.

COST PROPOSAL FOR TASK DC003B

<Valet Cleaners, 299501431>

< SVE Operations & Maintenance > April 2023 through March 2024

1. INTRODUCTION: <DISCUSS SCOPE OF WORK, DATES >

GHD Services Inc. (GHD) installed and is overseeing operations of a low flow vapor recovery system in accordance with the approved Pilot Test Plan at the Valet Cleaners site. Task DC003B includes twelve (12) additional months of operations and maintenance of the existing system. Task DC003B also includes well installation and annual groundwater monitoring at six wells.

Subtask 1: Project Management Subtask 1 includes all activities associated with management of the operations and maintenance of the existing system for twelve (12) months.

Subtask 2: Six Months O&M (April 2023 through September 2023) Initially, GHD will mobilize to the site and attempt to locate interior monitor well MW014.

Subtask 3: Six Months O&M (October 2023 through March 2024) GHD will conduct semi-annual site visits to verify system operations.

GHD proposes to conduct annual groundwater sampling in May/June 2023. The groundwater monitoring will consist of sampling deep monitor wells (MW018 and MW004) and shallow monitor wells (MW011, MW015, MW016, and the newly installed shallow well [MW019]).

Subtask 4: Utilities (12 months) Subtask 4 includes twelve months of utility charges (April 2023 through March 2024). The utility charges are estimated at $55 per month.
