FACID NO. 06-9701245; ERIC_4234

Coral Springs, Broward County FROM: Theresa Pepe, Project Manager SUBJECT: Task Assignment DC037A (HW680) DATE: June 21, 2023

Arcadis U.S. Inc. will install and sample a deep monitor well to delineate the groundwater plume. All activities will be conducted in accordance with the June 16, 2023, Work Plan submitted by Doug McGlone of Arcadis.

The final task assignment total is $19,430.67. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: Well Installation & Sampling Arcadis will coordinate access for the off-site property before work commences. A site access form for the source property was obtained in June 2021. No changes in property ownership have occurred since that date.

Arcadis will install a deep off-site monitor well via sonic drilling to delineate the downgradient vinyl chloride plume. The location/depth of the well was approved by the Waste Site Cleanup Section geologist.

PDS was chosen as the drilling subcontractor based on their low bid of three bids solicited. IDW will be drummed and disposed by a licensed waste hauler.

Subtask 4: Reporting Arcadis will submit a well installation and sampling report as the Final Deliverable.

Proposal for Additional Assessment Lori-Al D140 1316 North University Drive, Coral Springs, FL FDEP Facility ID No. ERIC_4234

Dear Theresa:

As requested, Arcadis U.S., Inc. (Arcadis) is pleased to provide the Florida Department of Environmental Protection (FDEP) this proposal for monitor well installation and sampling and preparation of report of findings with recommendations as to whether the monitor well network is adequate.

PROPOSED SCOPE OF WORK

Task 1: Project Management

This task includes project management activities associated with completion of Tasks 2 through 3.

Task 2: Offsite Access, Well Installations and Sampling/Analysis

Arcadis will verify ownership of the University Maplewood Townhomes bordering the site to the west for the installation of one additional monitor well (MW025) for vertical delineation of vinyl chloride. The new well will be constructed with 2-inch diameter pvc and screened 100-110 ft bg. The top of casing elevation of the newly installed well will be surveyed and referenced to the existing well network.

The newly installed well will be sampled for the presence of VOHs by EPA 8260.
