INITIAL SITE ASSESSMENT WORK PLAN

www.arcadis.com

Former Bartley's Laundry ISA Work Plan 09092022_ 4 time of this submittal. Arcadis will forward information provided by the current owner should it become available in the future.

5 INITIAL SITE ASSESSMENT WORK PLAN

A flexible assessment approach will be used to create a site-specific unsaturated soil and groundwater database capable of determining the absence/presence and extent of chlorinated drycleaning solvents above soil and groundwater CTLs. In addition, potential risks and exposure pathways will be identified and a preliminary recommendations and design provided for remedial action, if justified. The proposed assessment activities will be completed in accordance with FDEP’s Standard Operating Procedures (SOP) (001/01), the site-specific HASP, Chapter 62-780 Florida Administrative Code (F.A.C.), and recent guidance provided in the July 13, 2022 Waste

Cleanup Program Pollution Response Action Contract (WCP PRAC) meeting in Tallahassee, Florida.

Based on the reported historical PCE contamination in the shallow soil (southeast corner of the former Bartley’s facility unit), the anticipated depth-to-water (~7 ft bg), multiple assessment methodologies are proposed.

Unsaturated soil assessment will be completed by the installation of soil borings to two feet into the water table, soil field screening using a PID capable of detecting to parts per billion (ppb), commonly referred to as a ppbRAE®, soil sampling for fixed-based laboratory analysis of volatile organic compounds (VOCs), and completing a Modified Active Soil Gas Survey (MAGS).

The first phase of groundwater assessment will be completed as follows:

installing a source area monitor well (MW001) inside the facility for fixed-based laboratory analysis of VOCs, completing vertical profiling using direct-push technology (DPT) for fixed-based laboratory analysis of VOCs;

and, installing piezometers to assess groundwater gradient conditions.

Following receipt and evaluation of the first phase of assessment, the scope of the next phase of investigation activities, such as additional monitor wells, will be developed in consultation with FDEP. The site assessment methods and techniques are described in detail below.

5.1 Site Preparation and Utility Locate

Arcadis will contact the current site operator and current property owner (Publix representative, Ms. Maya Moss) to share proposed sampling locations and advise schedule/notification requirements prior to completing field work. Arcadis will oversee a subsurface private utility locate using ground penetrating radar (GPS) on the site and surrounding areas. The purpose of the utility locate will be to establish/confirm the suspected locations of underground utilities (Sunshine State One Call will initially locate utilities on and surrounding the site). The initial site visit indicated the presence of underground water, sewer and electrical lines as shown on Figure 3.

Former Bartley's Laundry ISA Work Plan 09092022_ 5

5.2 Soil Assessment and Sub-Slab Soil-Gas Sampling

5.2.1 Unsaturated Soil Assessment

One continuous soil boring (SB010) will be installed in the rear of the facility to a depth of approximately 50 ft bg or into the basal confining clay at the bottom of the surficial aquifer to characterize site lithology and for field screening of the unsaturated zone for synthetic organic vapors. A lithologic log of the general geology at the site will be developed from the soil samples collected from this soil boring. Soil samples from each boring will be described by an onsite Arcadis geologist.

To confirm if past drycleaning operations at the site had an adverse impact to unsaturated soil quality conditions, up to 12 soil borings (SB001-SB012) for collection of soil samples are proposed for installation at the locations shown in Figure 3. The initial sample location will be placed in vicinity of the confirmed PCE soil impacts inside the unit; additional boring/sample locations will be placed within and surrounding the building. Soil boring locations may be adjusted based on the GPR results and location of the sewer lateral.

The soil borings will be installed using a hand auger and DPT. A concrete coring machine will be used to bore through the concrete floor inside the building to access the underlying soils. Soil samples at all soil borings will be collected at 0.5 ft bg, 2 ft bg, and then at two-foot depth intervals to a depth of two feet into the water table

(estimated to be at 7 ft bg). All soil samples (up to 6 samples/boring x 12 initial boring locations or a total of up to

72 soil samples) will be submitted to a fixed-based laboratory for confirmatory analysis of VOCs by EPA Method

8260. Each soil sample will also be collected in encore samplers in anticipation of SPLP analysis. Five SPLP analysis are included in the initial costs. Following the sampling, the boreholes will be sealed with cement grout.

Soil samples will also be transferred to mason jars, covered with aluminum foil and screened with a ppbRAE equipped with a photo-ionization detector to determine if the soils contain the presence of synthetic organic vapors.

5.2.2 Modified Active Gas Sampling

MAGS is proposed for completion inside and outside the building. MAGS will be completed in general agreement with the Modified Active Gas Sampling Manual, dated October 12, 2004. Six MAGS wells (MAG01 – MAG06).

One MAGS point will be installed near the front door because the previous soil impacts were detected in the southeastern corner of the building unit. The MAGS points will be constructed of 2-inch diameter PVC with 0.010-inch slotted screen set approximately one foot below ground surface to approximately one foot above water, estimated to be 7 ft bg, resulting in an assumed screen interval of 1 to 6 ft bg. Final MAGS well construction will be field determined. One vapor sample will be collected from each MAGS well at the end of the test for analysis of

VOCs by EPA Test Method TO-15. Proposed MAGS well layout locations are depicted on Figure 3.

5.3 Groundwater Assessment

5.3.1 Groundwater Vertical Profiling

Groundwater assessment will be conducted utilizing vertical profiling by a direct-push drill rig to collect water samples for VOCs analysis. Five vertical profile (VP) locations (VP001 – VP005) (at or near reported and suspected source areas and downgradient) are proposed as shown on Figure 3. Assuming the water table will be

Former Bartley's Laundry ISA Work Plan 09092022_ 6 encountered at approximately 7 ft bg and the base of the surficial aquifer is approximately 40 ft bg, samples starting at the water table and at 10-ft depth intervals to a total depth of 41 ft-bg (7-11, 17-21, 27-31, and 37-41 ft

bg) will be collected. Lithologic information from Soil Boring SB010 will be used to determine exact sample intervals. Each VP water sample will be submitted to a fixed-based laboratory for confirmatory analysis of VOCs by EPA Method 8260. Following sampling, each vertical profile borehole will be sealed with cement grout.

5.3.2 Piezometer Installation

Arcadis will install two piezometers and utilize the interior monitor well MW001 (described below) and the existing well (TMW-1) to provide groundwater gradient information. For purposes of this Work Plan, it is assumed that two shallow piezometers (PZ001 and PZ002) will be installed with 10-ft of 0.010-inch slot pre-pack screen, between 4 to 14 ft bg using DPT (Figure 3). Final location, dimensions, including depth, and screen interval will be determined based upon groundwater elevations encountered during the initial assessment activities. The piezometers will be developed and completed in a method to allow them to be converted to monitor wells in the future, if necessary. Arcadis will also survey the top of casing elevations of MW001, TMW-1, PZ001 and PZ002 relative to common point so that the groundwater gradient can be determined.

5.3.3 Monitor Well Installation and Sampling

Arcadis will hand-install a source area well inside the facility (MW001) into the water table to maximum depth possible at the location presented on Figure 3. The well will be constructed with 0.010-inch slot pre-pack screen, developed, and completed within a manhole. Arcadis will sample MW001 and existing well TMW-1 per SOP

001/01 methodology for laboratory analysis of VOCs using EPA Method 8260.

Based on the results of the initial assessment activities and vertical profiling results, the installation of additional monitor wells and sampling analyses in a subsequent phase will be developed in consultation with FDEP. Also, sampling for per- and polyfluoroalkyl substances (PFAS) will be screened per FDEP guidance and tasked separately.

6 REPORTING

An Initial Site Assessment Report (ISAR) including field logs, photographs, laboratory reports will be submitted to

FDEP within 30 days of receipt of laboratory data as an interim deliverable to document the fieldwork. The ISAR will provide a basis for the next phase of work which may include additional soil sampling, the installation of permanent monitor wells, a professional survey, etc. to be completed under a Task Assignment Change Order

(TACO).

A final Site Assessment Report (SAR) will serve as the final deliverable and will describe the results of the proposed site assessment tasks described herein will be delivered to FDEP within 60 days of completion of the field work. The SAR will contain field logs, laboratory reports, tables, and figures summarizing the results of assessment activities and include conclusions and recommendations based upon findings.

7 INVESTIGATIVE DERIVED WASTE

Investigative derived wastes (IDW - drill cuttings and purge water) generated during site assessment activities will be drummed, labeled and temporarily staged on-site in Department of Transportation (DOT)-approved, 55-gallon

Former Bartley's Laundry ISA Work Plan 09092022_ 7 steel drums. Arcadis will coordinate with a waste disposal contractor for non-hazardous disposal of the IDW and provide copies of final disposal manifests to FDEP.

8 QUALITY ASSURANCE & QUALITY CONTROL

All fieldwork will be performed in accordance with FDEP’s SOP (001/01), and the site-specific HASP included under separate cover. Additionally, sample analyses will be performed by NELAP-certified laboratories to method practical quantitation limits. Appropriate QA/QC samples will be collected during the assessment.

9 PROJECT SCHEDULE AND COST

Cost estimate/level of effort (CEF-LOE) worksheets and subcontractor bid copies for the implementation of this

ISAWP will be provided separately upon approval of this Work Plan. A proposed schedule for the site assessment activities, ISAR, and the final SAR document package, is presented in Appendix C.

$1,425.00

TOTAL $4,065.00

Scope of Services: Subsurface Utility Designation

All SurvTech SUE services abide by the standards set forth by ASCE under (CI/ASCE 38-02)

1. By use of Ground Penetrating Radar (GPR) and Electromagnetic Induction (EM), Attempt to horizontally locate and field mark (paint & flags) all public subsurface utilities found excluding service lines, gravity sewer lines and irrigation within the area outlined on the attached Exhibit “B”.

Deliverables shall be surface marking (paint and/or flags) of horizontal location of found subsurface utilities and a utility designation field sketch(s) of the project area reflecting all obtained relevant data for your use.

TERMS AND CONDITIONS

It is understood that the Arcadis U.S., Inc. is responsible to abide by Sunshine 811, Florida State Statutes Chapter 556.106 and all applicable laws, and regulations that pertain to the services provided.

SurvTech expects Arcadis U.S., Inc. to supply any existing information pertaining to the location of underground utilities in the project area. In addition to any information provided by Arcadis U.S., Inc., SurvTech will proceed with retrieving records from regulatory agencies including but not limited to: Sunshine One Call, local and state utility departments, and private utility providers.

SurvTech will ensure that all reasonable efforts are made to identify the location of each underground utility and provide the best available information within the project area but makes no guarantees that all utilities and structures will be found.

Limitations including but not limited to the lack of or inaccuracy of prints, and restrictions with GPR technology exist due to the complexity of site conditions and circumstances beyond the control of SurvTech.

SurvTech expects that the proposed work area to be free and clear of objects or obstacles that might prohibit access in and around the proposed work area. This would include clearing vegetation, machinery, debris, or any other object that would prevent access to the proposed work area. Failure to do so could result in time delay or incomplete coverage of the proposed work area.

Arcadis U.S. will be responsible for verifying any information provided by SurvTech that is obtained via contact with utility providers as per the Underground Facility Damage Prevention and Safety Act.

The surface designation provided by SurvTech is certified to be within two (2) feet of the true underground position of the utility relative to the mark as shown on the surface.

SurvTech shall not be held liable for any latent or unreasonably discoverable utilities or structures in the project area.

Furthermore, in the event of a claim regarding the services provided in the proposal, SurvTech shall have liability for reasonable and necessary defense costs to the extent caused by SurvTech’s negligence.

SurvTech shall NOT obtain depths while designating utilities. If depths are required, the client must contract for verification (soft dig) services. Soft dig (vacuum excavation) services are NOT included in this proposal.

Notes:

Paint marks will be color coded based upon suspected underground utility, according to the American Public Works Association Uniform Color Code Guide.

Poor conditions for GPR utilization include sites with high water tables (especially saltwater); heavily vegetated areas; heavily rooted soils; ground conditions such as clay and heavily fertilized soil, reinforced concrete; and/or detecting for very smal l, non-metallic lines, such as PVC (2” and smaller).
