FACID NO. 26-9501255; ERIC_5413

Port St. Lucie, St. Lucie County FROM: Theresa C. Pepe, Project Manager SUBJECT: Task Assignment DC031B (HW680) DATE: October 17, 2023

On August 10, 2023, the Waste Site Cleanup Section approved an Interim Source Removal Work Plan for installation of a soil vapor extraction (SVE) system. Arcadis will construct and install a new treatment system rather than relocate/reuse an existing system. Additionally, three months of Operation and Maintenance (O&M) will be funded under this task assignment. All work will be conducted in accordance with the approved ISR Work Plan and the October 11, 2023, Scope submitted by Doug McGlone of Arcadis.

The final task assignment total is $158,374.55. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated based on a percentage of total labor hours.

Subtask 2: Construction An Access Agreement with the current real property owner was executed in December 2019. No changes in ownership have occurred since that date.

The system will be installed according to the approved ISR Work Plan with the goal of reaching contamination beneath the building slab. Construction and delivery of the new equipment will take approximately eight weeks. Fieldwork construction/installation will take approximately five days. IDW will be disposed of by a licensed waste hauler.

Rivergate Drycleaners, ERIC_5413

LMS Manufacturing (LMS) was chosen as the subcontractor to build the equipment based on their low bid of three bids solicited.

Petroleum Resources & Development (PRD) was chosen as the subcontractor to install the SVE based on their low bid of three bids solicited.

Vehicle and some equipment on the proposal was changed to the weekly rate rather than daily in order to save costs. No other changes were required.

Subtask 3: System Startup and Reporting System startup and As-built drawings will be provided under this subtask.

Groundwater sampling, vapor sampling and testing of system components will also be conducted. Costs for some equipment was reduced from the original proposal. No other changes were required.

Subtasks 4: Three months O&M This subtask provides costs for one quarter of O&M including vapor sampling as specified in the ISR Work Plan. An email update will be submitted as a deliverable.

This subtask was reduced from six months of O&M to three months. No other changes were required.

Subtask 5: Utilities This subtask includes one quarter of electrical power to run the remedial system plus associated handling fees. Arcadis will only invoice for actual charges and any remainder will be adjusted via change order. This subtask was reduced from six months of O&M to three months. No other changes were required.

Date: October 6, 2023 Our Ref: 30177630 Subject: Proposal for Soil Vapor Extraction Installation and Startup Rivergate Dry Cleaners (ERIC_5413) 1193 SE Port Saint Lucie Boulevard, Port Saint Lucie, Florida

Dear Ms. Pepe, Arcadis U.S., Inc. (Arcadis) is pleased to submit this proposal/cost estimate for remedial action construction activities at the Rivergate Dry Cleaners Site, located in Port Saint Lucie, Port Saint Lucie, Florida. This proposal includes the costs to procure soil vapor extraction (SVE) equipment, install the SVE piping and wells, startup, and perform three months of operation, maintenance, and monitoring (OM&M) of the SVE system. The SVE system design was approved on August 10, 2023, by the Florida Department of Environmental Protection (FDEP). This proposal includes a description of the scope of services, and an estimated cost and schedule for their completion.

The CEF is included as Attachment 1.

TASK 1: PROJECT MANAGEMENT

As specified by FDEP, project management is calculated automatically in the CEF as the first subtask.

TASK 2: SVE SYSTEM CONSTRUCTION

This task includes the equipment procurement, construction contractor contracting, and electrical installation activities for a completed SVE system at the Site. The scope of work was included in a request for proposal (RFP) documents prepared and submitted to three qualified subcontractors. A copy of the RFP documents and bids for the equipment and construction contractors are included in Attachment 2. The following activities are included in this task:

• Arcadis will procure the subcontractors (LMS Manufacturing, LLC (LMS) – Equipment and Petroleum Resources & Development (PRD) -installation contractor). Arcadis will provide technical support and oversight activities for each contractor. A brief description of the scope of work for each contractor is included below:

The selected Remedial Action Construction Contractor, PRD, estimated up to five working days, are required to complete the proposed scope of work. The deliverable for Task 2 will consist of a Construction Completion Letter.

TASK 3: SYSTEM STARTUP AND REPORTING

Following the completion of the remedial construction activities, Arcadis will perform system startup activities and balance the system for optimal operation. System startup will include:

• Prior to SVE startup, a groundwater monitoring event will be conducted to establish baseline VOC concentrations in monitor wells MW001, MW002, MW002I, MW003, MW003I, MW004, MW004I, and MW005I by USEPA Method 8260B.

• One day system startup activities (Day 1) as specified in the ISRWP.

• Testing of each system component and telemetry system.

• Measuring influence (wellhead vacuum/pressure and depth to water) in select monitoring wells.

• Recording system status (pressure, temperature, flow rates, etc..).

• Collection of air samples from each SVE well (2 samples), system influent (1 sample), and effluent (1 sample) for laboratory analysis by TO-15 for VOHs. This will be completed on the first day of system startup.

A summary table for sampling activities associated with Task 3 is included below:

Event Samples

Baseline GW 8-Samples (8 monitor wells)

Startup 4 total samples (SVE-1, SVE-2, Influent, and Effluent)

Following system startup, Arcadis will prepare a Remedial Construction Completion System Startup Report. The Report will include a description of the work completed, field modifications, field notes, equipment installed, and

As-Built Drawings which will be signed and sealed by a registered Professional Engineer licensed in the State of Florida.

TASK 3 AND TASK 4: GENERAL OM&M ACTIVITES

The SVE system’s performance will be monitored monthly for three months (Yr 1 Qtr 1). The following will be performed during each monthly event:

• The ROI of the SVE system will be measured by sequentially isolating the SVE wells and measuring the vacuum influence of the nearby SVE or monitor wells.

• Vapor extraction flow rate and vacuums will be recorded using meters and gauges located within the system.

Adjustments will be made as necessary to optimize system performance.

• In accordance the approved ISRWP, the SVE system’s vapor emissions (influent and effluent (post carbon)) will be sampled at the end of Week 1 and end of Month 1, and then quarterly thereafter (5 samples)

• The extraction blower will be inspected and adjusted for proper operation, filter replacement and amperage monitoring.

• The SVE system will be operated to minimize the capture and entrainment of groundwater.

• The system interlocking controls and fail-safe mechanisms will be inspected for proper operation.

• During each OM&M visit the SVE wells will be evaluated for mass recovery. Each SVE well will be operated for a minimum of 30 minutes prior to PID reading collection.

Recovered vapors from the SVE system will be monitored monthly, when in operation. SVE vapor samples will be collected from the system influent on a quarterly basis visit to calculate mass removal.

TASK 4: SVE SYSTEM OM&M YEAR 1 QUARTER 1

Task 3 consists of system monthly OM&M visits for one quarter and quarterly reporting. The proposed vapor sampling will be sampled for volatile organic compounds (VOCs) for EPA Method TO-15. A summary table for the quarter is included below:

Month Site Visits Samples

1 2 – Week 1 and Month 1 4-Vapor - Influent and Effluent

2 1-System O&M 1-IDW- GAC

3 1-System O&M 1-Vapor - Influent

• Four site visits are proposed during the first quarter of O&M (Week 1, Month 1, Month 2, and Month 3).

• Five vapor samples are proposed per the table listed above.

October 6, 2023

Use or disclosure of information contained on this sheet is subject to the restriction and disclaimer located on the signature page of this document. 4/5

• After the first month, if the vapor emission rate of HAP is less than 2.7 and 6.8 pounds per day for individual and aggregate HAPs, respectively, then a letter requesting discontinuation of vapor phase treatment will be submitted to the FDEP. Following approval, only a single effluent vapor sample will be collected on a quarterly basis.

• A GAC sample is proposed for TCLP VOC analysis for IDW profiling.

A Quarterly email update consisting of tables and figures will be submitted at the completion of the quarter (i.e., Month 1 through Month 3).

TASK 5: UTILITIES MANAGEMENT

This task will be used to manage and process FPL invoices for six months of system operation included in this proposal. Based upon typical utility invoices for a system of this size, it is anticipated that the utility budget be estimated at $400 per month. A monthly handing fee of $20 per month is also included. Actual utility costs will be invoiced, and any differences will be adjusted through Task Assignment Change Orders.

SCHEDULE

Arcadis will begin implementing the remedial action construction upon receipt of a Task Assignment (TA) from the FDEP. The following deliverables and estimated timeframe for completion are included below:
