About this file
This is a Method of Approach document submitted by Granite as part of a bid response for the Arizona Department of Transportation's (ADOT) On-Call Pipelining and Drainage Repair Services contract (BPM005628). The document outlines Granite's comprehensive process for executing pipelining projects from initiation through completion. Upon receipt of a task order, the company's Project Manager will conduct a site visit with ADOT representatives to establish scope limits, access requirements, water source and dump site locations, and traffic control needs. Following the site visit, a detailed cost estimate and schedule will be prepared. Once a Notice to Proceed is issued, the Superintendent will coordinate crew and equipment dispatch. The service scope includes pipe delivery, hydrovac cleaning, pipe distribution, pipe lining using 24-foot joints with bell and spigot connections sealed with rubber O-ring gaskets, annular space grouting at 300 PSI, and site cleanup with final inspection and acceptance by ADOT district personnel.
The approach emphasizes comprehensive safety management, including identification of routine and non-routine hazards through Job Hazard Analysis (JHA) documents and daily "Take 5" safety meetings conducted post-shift and when activities change. The company has identified eight primary safety categories specific to pipelining work: moving equipment, release of stored energy, falls, hazardous materials, picks and lifts, pinches and crunches, electrical hazards, and work environment concerns. All excavation work will be performed by a competent person in compliance with OSHA requirements. Pipe lining operations utilize a four-person crew with heavy equipment including backhoes, rough terrain forklifts, and F-550 flatbed trucks with gooseneck trailers. Grout is sourced from commercial suppliers and delivered in ready-mix trucks, with bulkheads constructed at pipe ends to contain grout during the pumping process.

METHOD OF APPROACH
ADOT Procurement - On-Call Pipelining and Drainage Repair Services (BPM005628)
The Offeror shall provide a brief narrative on the entire process of a pipelining project from start to finish.
Notice of Task Order Upon notice of a task order, Danny Hoback, Project Manager, will setup a field meeting to review the scope of work onsite with the ADOT representative. At the site visit Danny will work with the District to:
• Identify the limits of the scope of work
• Identify access requirements and restrictions
• Identify the location of water source and dump sites
• Determine the appropriate traffic control requirements for the work.
Upon completion of the site visit Danny will produce a detailed cost estimate and schedule for the individual scope of work.
Notice to Proceed:
Once a notice to proceed is issued, Danny will coordinate with Rafel Frisby, Superintendent, to dispatch crews and equipment.
Safety:
Sam Kennedy, Regional Safety Manager and Rafael Frisby shall meet onsite to identify routine and non-routine activities, including activities of all persons having access to the workplace, foreseeable hazards originating outside the workplace capable of adversely affecting activity within the workplace, and hazards inherent to our work that could adversely affect 3rd parties outside the workplace. Once the activities and hazards are identified, we will identify how we will eliminate, substitute, or control said hazards. This information will be shared through our Job Hazard Analysis document that will then be shared with craft at our daily safety work plan “Take 5” meetings. Take 5’s occurs post shift and anytime activities change. Craft will actively engage and contribute to the further identification, elimination, and control of hazards. We have prepared JHA’s for each of the activities, which are available on request.
Granite will identify and address all hazards related to pipe construction. Including but not limited to shoring, bracing, or sloping excavation to meet OSHA requirements. A competent person will be on-site for all excavations, and conduct inspections per OSHA guidelines.
Core to our program is a large focus on Stuff Than Kill You (STCKY). Identification of STCKY focuses on eight primary STCKY categories for our industry which are moving equipment, release of stored energy, falls, hazardous materials, picks/lifts, pinches and crunches, electrical, and work environment.
During work, employees are empowered to stop work if they identify a condition or behavior that is hazardous (“Speak Up, Listen Up” policy). When activities change, mid-shift Take 5’s are conducted. Our Front-Line Leaders continuously conduct inspections and formally document on a weekly basis. Front-Line Leaders also submit one “Observation” per day that describes either a positive or negative act (behavioral) or condition.
Step-by-step procedure for Pipelining:
1. Pipe Delivery - Pipe will be delivered to ADOT maintenance facilities and unloaded by Granite in an area designated by ADOT.
2. Hydrovac Pipes - Hydrovac of pipes is preferred to be done from the outlet end of pipe(s), however can be done from inlet side when roadside conditions make it too difficult for access.
METHOD OF APPROACH
ADOT Procurement - On-Call Pipelining and Drainage Repair Services (BPM005628)
3. Pipe Distribution - Granite will reload the stored pipe at ADOT maintenance facilities and haul to pipelining locations. This work is performed with a two-man crew, reach-forklift and F-550 flatbed truck pulling goose neck trailer.
4. Lining the pipe(s) - This process starts by choosing the inlet or outlet end that provides the most work area with the least amount of disturbance to the right of way. The pipe comes in 24 FT long joints with a factory milled bell and spigot that utilizes a rubber O-ring gasket to seal and lock the bell and spigot joints together. Joints are lubricated with a substance provided by pipe manufacturer made specifically to aid in “snapping” pipe joints together. Granite utilizes a combination of come-a-longs, chain hoists, straps, chains, and heavy equipment to ensure the pipe is snapped together properly. The pipe is then pushed through the existing pipe 24 FT at a time with a backhoe or rough terrain forklift until it sticks out 12” on each end. If more than 12” of pipe is sticking out it is cut off and used in other locations.
Two 2” PVC pipes are inserted into the resulting annular space on top of the newly inserted pipe to provide a means for the grout to be pumped into the annular space. Bulkheads are constructed at each end of the pipe to hold the grout in place. Lining of pipe is performed with a four-person crew consisting of Foreman with pickup, F-550 flatbed truck with gooseneck trailer, backhoe, truck driver, operator, and laborer(s).
5. Grout Annular Space – Grout is pumped into the resultant space between the existing pipe and the new liner pipe. A towable trailer mounted grout pump is connected by a hose to the 2” PVC pipe. Grout (300 PSI) is purchased from commercial sources and delivered in ready mix trucks to the pipe location. Grout is pumped into the annular space until it comes out of the observation ports on each end.
6. Cleanup & Inspection – Granite will cleanup and restore areas of disturbance that were created. ADOT district personnel will be requested to accept the completed work on the last day of work before we leave a project.
