About this file

This is a Method of Approach document submitted as part of an on-call pipelining and drainage repair services procurement by the State of Arizona Department of Transportation (ADOT). The document outlines Granite's comprehensive approach to drainage excavation projects, beginning with a field meeting upon notice of task order where the project manager identifies the scope of work, access requirements, utility locations, traffic control needs, and potential obstructions. Following receipt of a notice to proceed, the contractor will dispatch crews and equipment tailored to site-specific conditions, typically including foremen, laborers, equipment operators, and machinery such as excavators, loaders, backhoes, and compactors. The methodology encompasses a step-by-step process including utility identification through Arizona 811 tickets and hand-location within two feet of construction areas, clearing and grubbing limited to necessary vegetation removal, site access establishment within the ADOT right-of-way, excavation and embankment work using appropriate equipment, and compaction through moisture application and vibratory equipment.

The document emphasizes safety as a core program component, with the Regional Safety Manager and Superintendent conducting on-site hazard identification meetings and developing Job Hazard Analysis documents shared through daily safety briefings. The contractor implements a "Stuff That Can Kill You" (STCKY) program addressing eight hazard categories including moving equipment, falls, hazardous materials, and electrical hazards, with a "Speak Up, Listen Up" policy empowering employees to halt work upon identifying hazardous conditions. All excavations will comply with OSHA standards with competent persons on-site conducting regular inspections. Site restoration will return disturbed areas to conditions similar to surrounding areas, though seeding and re-vegetation are excluded from the contractor's scope.

METHOD OF APPROACH

ADOT Procurement - On-Call Pipelining and Drainage Repair Services (BPM005628)

The Offeror shall provide a brief narrative on the entire process of a drainage excavation project from start to finish.

Notice of Task Order Upon notice of a task order, Danny Hoback, Project Manager, will setup a field meeting to review the scope of work onsite with the ADOT representative. It is imperative that a field meeting is conducted as the scope of work for drainage excavation varies drastically from location to location. At the site visit Danny will work with the district to:

• Identify the limits of the scope of work.

• Identify access requirements and restrictions.

• Identify the location of water source and dump sites.

• Determine the appropriate traffic control requirements for the work.

• Identify existing utilities or other potential obstructions.

Upon completion of the site visit, Danny will produce a detailed cost estimate and schedule for the individual scope of work.

Notice to Proceed:

Once a notice to proceed is received, Danny will coordinate with Rafel Frisby, Superintendent, to dispatch crews and equipment. The crew size and pieces of equipment will be dependent on the individual location, but will generally include a combination of, Foremen with pickup, Grade checker, Laborers, Equipment Operators, Truck Drivers, Pickups, Excavators, Compactor, Loaders, Backhoe, skid steer, dozer, Telehandler with bucket, Equipment Transport trucks and trailers.

Safety:

Sam Kennedy, Regional Safety Manager and Rafael Frisby shall meet onsite to identify routine and non-routine activities, including activities of all persons having access to the workplace, foreseeable hazards originating outside the workplace capable of adversely affecting activity within the workplace, and hazards inherent to our work that could adversely affect 3rd parties outside the workplace. Once the activities and hazards are identified, we will identify how we will eliminate, substitute, or control said hazards. This information will be shared through our Job Hazard Analysis document that will then be shared with craft at our daily safety work plan “Take 5” meetings. Take 5’s occurs post shift and anytime activities change. Craft will actively engage and contribute to the further identification, elimination, and control of hazards. We have prepared JHA’s for each of the activities, which are available on request.

Granite will identify and address all hazards related to drainage excavation. Including but not limited to preventing collapse; bracing and shoring and providing safe access. All excavations will be conducted in compliance with OSHA and Granite safety requirements. A competent person will be on-site for all excavations, and conduct inspections per OSHA guidelines.

Core to our program is a large focus on Stuff Than Kill You (STCKY). Identification of STCKY focuses on eight primary STCKY categories for our industry which are moving equipment, release of stored energy, falls, hazardous materials, picks/lifts, pinches and crunches, electrical, and work environment.

During work, employees are empowered to stop work if they identify a condition or behavior that is hazardous (“Speak Up, Listen Up” policy). When activities change, mid-shift Take 5’s are conducted. Our Front-Line Leaders continuously conduct inspections and formally document on a weekly basis. Front-Line Leaders also submit one “Observation” per day that describes either a positive or negative act (behavioral) or condition.

METHOD OF APPROACH

ADOT Procurement - On-Call Pipelining and Drainage Repair Services (BPM005628)

Step-by-step procedure for Drainage Excavation:

In general, most drainage repairs are completed with excavation, embankment, and grading. The process for this type of drainage excavation will include:

Identify Existing Utilities - Granite will establish an Arizona 811 ticket for each project location prior to construction. Upon utility marking, Granite will identify and locate utilities by hand within 2FT of proposed construction.

Clearing and Grubbing - Granite will clear only the brush and trees necessary to perform its work. Brush and trees that require removal will be scattered at the project site in a manner that blends with existing conditions or habitat or if required can be chipped or hauled to a disposal site. This work will be completed with a backhoe, loader, and water truck.

Site Access - Granite will establish access to the drainage excavation within the ADOT ROW and impacting the least amount of area as possible. This will be done with a backhoe, water truck, and loader.

Excavation, Embankment, Grading - In most instances the drainage repairs will be completed solely with excavation, embankment, and grading. In these locations where excavation or embankment are required, Granite will excavate or grade existing materials in ROW to fill in eroded areas. If locations do not have enough existing material, Granite will haul embankment material as needed. Excavation, embankment, grading may be done by excavator, loader, backhoe, or combination of these depending on the locations need. Compaction will be achieved by moistening the embankment utilizing a 3600-gallon water truck and compacting by vibratory compactor or compaction wheel attached to excavator.

Cleanup Granite will restore the locations it creates disturbance to. Will restore to similar surrounding conditions except for seeding and re-vegetation.
