HW683 DC020B
This Task Assignment authorizes Geosyntec to conduct site assessment activities at the Allen’s Laundry & Dry Cleaning site in accordance with the Geosyntec scope of work, cost estimate, and schedule received on 04 October 2022.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2 – Field Preparation and Preliminary Assessment Tasks labor for field preparation, completion of one offsite access agreement needed for the Site Assessment, and preparation of a Site Health and Safety Plan is included in subtask 2. Geosyntec personnel will mobilize to the Site to observe the first phase of site assessment activities.
Geosyntec will observe utility locating, performed by GeoView, lithology boring by direct push technology down to 50 ft-bls or refusal, whichever comes first, water well sampling, and piezometer installation of 3 piezometers to determine the depth to water and groundwater flow direction.
Subtask 3 – Multi-Media Sampling (Soil, DPT groundwater, soil vapor) and Interior Monitoring Well Installation Geosyntec will remobilize to the Site to conduct soil sampling, DPT groundwater sampling, and MAGS well installation/testing. These activities have been assumed to need 9 field days with PDS and 1 day for MAGs testing.
Soil sampling will be performed at up to 24 locations (SB001 through SB020 and MAGS001 through MAGS004). A minimum of 7 samples will be collected from each of the 24 soil sample locations and will be collected by hand auger.
Four, 2-inch (in.) diameter Schedule 40 polyvinyl chloride (PVC) MAGS wells (MAGS001 through MAGS004) will be installed by hand auger to a depth of approximately 14 ft BLS. Each MAGS well will be installed with 1-ft of riser and 10-ft of 0.01-in. slotted well screen extending from approximately 1 to 14 ft BLS.
MEMORANDUM
Allen’s Laundry & Dry Cleaning October 5, 2022 Page Two
Each MAGS well will be tested for a period calculated to remove one, 10 to 15-ft radius of soil vapor volume using a blower. 1 hour is expected for each test. soil vapor will be monitored with an organic vapor analyzer equipped with a photoionization detector (PID) utilizing a 10.6 electronic volt lamp during testing.
DPT groundwater samples will be collected at 13 locations and will be collected at 4 ft intervals at every 10 ft resulting in 39 samples total.
One monitoring well (MW001) will be installed inside the facility adjacent to the former drycleaning machine. MW001 will be installed either manually with a hand auger or using DPT as part of the DPT groundwater assessment. The target depth of the monitoring well is approximately 18 ft-bls and will be made of 1 inch diameter PVC.
Exterior sampling locations will be surveyed throughout the week with a field GPS by Geosyntec personnel. Interior sampling locations will be transcribed using as-builts based on Site features.
Subtask 4 – Exterior Monitoring Well Installations, Groundwater Sampling, IDW Sampling Geosyntec will observe the installation of up to 4 exterior monitoring wells wo determine groundwater quality. Geosyntec will submit a WCD or TACO to FDEP for a focused utility locating at the drilling locations if spray paint is worn away or if there is any uncertainty in utility locations relative to drilling locations.
Additional upgradient, side gradient, and downgradient water-table monitoring wells will be installed using the hollow-stem auger drilling method. The wells will be constructed of 1-in.
diameter Schedule 40 PVC pipe.
Geosyntec will install one vertical extent monitoring well (MW005) to a depth of 45 feet.
Groundwater samples will be collected from the 5 site monitoring wells as well as depth to water measurements to evaluate groundwater flow direction. The monitoring wells will be sampled using a peristaltic pump.
One composite soil sample from the drums generated at each monitoring well location will be collected. It is assumed that 2 additional samples will be needed to characterize soil drums generated in subtask 2 and 3. Liquid drums will be characterized based on groundwater sampling results.
Subtask 5 – IDW Management and Surveying Geosyntec will review the generation of a waste characterization profile as well as the preparation of IDW manifests. Geosyntec will coordinate disposal arrangements for up to 35 drums of nonhazardous IDW.
Subtask 6 – Annual Reporting The preparation and submittal of a SAR documenting assessment activities and summarizing analytical results will follow the completion of field activities.
Prior to field work, Geosyntec will obtain an updated Access Agreement from the current real property owner of the Allen’s Laundry & Dry Cleaning (ERIC_4268) site.
Breck Dalton verbally approved the Site Assessment Work Plan on October 05, 2022.
The DEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $166,557.30 with 521 labor hours.
Geosyntec may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Mr. Evan Miller Florida Department of Environmental Protection 2600 Blair Stone Road, MS 4520 Tallahassee, Florida 32399-2400
Subject: Scope of Work and Level of Effort – Site Assessment Allen’s Laundry & Dry Cleaning 7385 State Road 21
Keystone Heights, Clay County, Florida 32656 FDEP Site ID No. ERIC_4268
Dear Mr. Miller, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work and Level of Effort (LOE) for site assessment activities at the Allen’s Laundry & Dry Cleaning site (Site) located in Keystone Heights, Florida (ERIC_4268). Geosyntec prepared this LOE based on Geosyntec’s 2 September 2022 Site Assessment Work Plan (SAWP), approved by FDEP. A description of task breakdown for Site assessment field activities and report preparation is provided below.
SUBCONTRACTOR EVALUATION
Geosyntec obtained three bids from licensed and qualified utility locating, drilling, and IDW transportation and disposal firms; a cost comparison is provided in Attachment A. The drilling work was bid as a single project with three moblizations so that the total price was used in the evaluation. Geosyntec recommends the following subcontractors based on lowest cost among the qualified bidders:
• Underground utility locating – GeoView
• Piezomter installation, soil sampling, MAGs installation, DPT groundwater sampling – Preferred Drilling Solutions (PDS) (13 total field days)
• IDW transportation and disposal – Hulls Environmental Services
SurvTech Solutions, Inc. is recommended for surveying the five (5) monitoring wells. Copies of the bids from each subcontractor are included in Attachment A.
For ease of reference, Table 1 from the SAWP is attached as a summary of the field activities and samples to be collected.
Subtask 1 – Project Management
Fabio Fortes (Tampa Bay) will be the Geosyntec Project Manager. His labor hours are directly computed.
Subtask 2 – Field Preparation and Preliminary Assessment Tasks
Geosyntec will Subtask 2 includes labor for field preparation, completion of one offsite access agreement needed for the Site Assessment, and preparation of a Site Health and Safety Plan.
Geosyntec personnel will mobilize to the Site to observe the first phase of site assessment activities, which includes a private underground utility survey, lithology borings, sampling the onsite water well, and installing three piezometers.
Utility locating: Geosyntec will observe utility locating at the Site performed by GeoView.
Identified utilities will be mapped by Geosyntec staff using a Global Position System (GPS) unit following the private underground utility survey.
Lithology boring: One soil boring (LSB001) will be advanced using a direct push technology (DPT) drill rig to a depth of approximately 50-feet below land surface (ft BLS) or refusal, whichever is encountered first, to characterize the Site-specific lithology. This data will be used to assist in the selection of soil samples and DPT groundwater sampling intervals. No soil sampling for laboratory analysis was proposed from LSB001
Water well sampling: One groundwater sample will be collected from the onsite water well to verify the local water quality. Geosyntec will collect the sample from the spigot and analyze for EPA Method 8260 (full list) VOCs by a fixed-based laboratory.
Piezometer installation: Three (3) piezometers will be installed at the west, east, and southeast of the former drycleaning facility with the objectives of determining the depth to water and defining the local groundwater flow direction (SAWP Figure 4). The piezometers will be advanced using DPT to a target depth of approximately 20 ft BLS. The piezometers will be constructed of 1-inch diameter PVC and screened between 15 and 20 ft BLS. The top-of-casing elevation of each piezometer well will be surveyed by Geosyntec to an arbitrary site reference elevation. After the completion of this activity, the piezometer wells will be left in place, capped, and remaining annulus filled with bentonite so that they can be converted into shallow monitoring wells if needed in the future.
Geosyntec will review the depth of groundwater and lithology data to revise the depths of subsequent soil sampling, MAGS wells, and DPT groundwater samples following communication with FDEP.
Subtask 2 activities will be performed by 2 Geosyntec personnel over 1.5 days (8 hrs each) based on PDS estimate.
Subtask 3 – Multi-Media Sampling (Soil, DPT groundwater, soil vapor) and Interior Monitoring Well Installation
Geosyntec will remobilize to the Site to conduct soil sampling, DPT groundwater sampling, and MAGS well installation/testing. Based on the cost estimate provided by PDS, we have assumed these activities will be conducted over 9 field days with PDS and 1 field day for MAGs testing with Geosyntec personnel. Field days will be 8 hrs in length for 2 Geosyntec personnel; no weekend or night work will be conducted.
Soil Sampling: Soil sampling will be performed at up to 24 locations (SB001 through SB020 and MAGS001 through MAGS004) as presented on Figure 5 in the FDEP-approved SAWP. Based on an assumed depth-to-water (DTW) of 15 ft BLS, a minimum of seven soil samples (0.5, 2, 4, 6, 8, 10, and 15 feet below land surface [ft BLS] or water table) will be collected from each of the 24 proposed soil sample locations (168 total samples). Soil samples will be collected by hand auger or a DPT soil core. A concrete core drill will be used to access soil under the building slab (concrete) and driveway/parking lot (asphalt). Samples will be analyzed for the presence of VOCs via EPA Method 8260 by a National Environmental Laboratory Accreditation Program (NELAP) accredited, fixed-base laboratory. Soil samples will be screened with a photoionization detector (PID) organic vapor analyzer. Supplemental soil samples will be collected from each location and depth interval (via Encore samplers) for analysis of VOCs by the synthetic precipitation leaching procedure (SPLP) should VOC concentrations exceed the leachability-based soil cleanup target levels. For budgetary purposes, it is assumed that approximately half of the proposed soil samples will be analyzed by SPLP for VOCs. Exterior soil samples will be surveyed with a GPS (Subtask 8); interior sampling locations will be transcribed using as-builts based on Site features.
MAGS well installation: Four, 2-inch (in.) diameter Schedule 40 polyvinyl chloride (PVC) MAGS wells (MAGS001 through MAGS004) will be installed by hand auger to a depth of approximately 14 ft BLS (or 1-ft above the static water level if the depth to groundwater is different than historically observed). Each MAGS well will be installed with 1-ft of riser and 10-ft of 0.01-in.
slotted well screen extending from approximately 1 to 14 ft BLS. A 20/30 grade environmental sand filter pack will be placed from the bottom of the boring (top of water table at approximately 15 ft BLS) to the top of the well screen. The remaining annular space will be grouted to just below the top of casing. A temporary grout patch will be placed on top of a PVC cap that will be placed on each well to match existing grade and protect the well between installation and testing.
MAGS well testing: Each MAGS well will be tested for a period calculated to remove one, 10 to 15-ft radius of soil vapor volume using a blower. The test duration is expected to be approximately
1 hour. The vacuum and flow rates used at each MAGS well will be based on the results of a step test at one interior location. The step test will include a minimum of three steps at increasing vacuums to evaluate optimum airflow rates. Vacuum will be monitored at adjacent MAGS wells at approximate 15-minute intervals, as practical, during MAGS testing.
During testing, soil vapor will be monitored with an organic vapor analyzer equipped with a photoionization detector (PID) utilizing a 10.6 electronic volt lamp. Soil vapor samples will be collected at the end of each MAGS test using a batch-certified “clean” Summa canister provided by the analytical laboratory. The soil vapor samples will be sent to a fixed-based laboratory for analysis of VOCs by the TO-15 method.
DPT groundwater sampling: Geosyntec personnel will observe the collection of DPT groundwater samples at up to 13 locations (DPT001 through DPT013) as shown on Figure 6 of the SAWP.
Groundwater samples will be collected at 4-ft intervals approximately every 10 feet (i.e., 15 to 19 ft BLS, 25 to 29 ft BLS, and 35 to 39 ft BLS), resulting in 3 samples x 13 locations (39 samples).
Sampling intervals may be adjusted based on observed depth to water and Site lithology.
Groundwater samples submitted to a NELAP-approved fixed-based laboratory will be analyzed for the presence of VOCs by EPA Method 8260 (full list). No field duplicates will be collected.
Interior monitoring well installation: One monitoring well (MW001) will be installed inside the facility adjacent to the former drycleaning machine. MW001 will be installed either manually with a hand auger or using DPT as part of the DPT groundwater assessment. The monitoring well target depth will be approximately 18 ft BLS and will be constructed of 1-inch diameter PVC, screened between 13 and 18 ft BLS. A pre-pack screen will be used for hand auger installations and potentially for DPT installation, if warranted.
Field GPS and Interior Mapping: Exterior sampling locations (e.g., DPT groundwater, soil samples, select MAGS wells; approximately 31 locations) will be surveyed throughout the week with a field GPS by Geosyntec personnel. Interior sampling locations will be transcribed using as-builts based on Site features.
Data management and evaluation: Geosyntec has included costs to generate the following to facilitation discussison with FDEP:
• One figure and table to summarize VOC laboratory analytical results from the DPT groundwater samples;
• One figure and table to summarize VOC laboratory analytical results from soil samples;
and
• One figure and table that summarize VOC laboratory analytical results from MAGS samples.
These data will be provided to FDEP for evaluation to select the locations of proposed monitoring wells to be installed in Subtask 4; data gaps will be addressed during Subtask 4, if practical. If fixed-based laboratory analytical results of soil and DPT groundwater samples indicate that additional sampling will be required to complete delineation, Geosyntec will propose additional sampling locations and corresponding depths.
Subtask 4 – Exterior Monitoring Well Installations, Groundwater Sampling, IDW Sampling
Monitoring well installations: Once the final monitoring well strategy has been established with FDEP, Geosyntec will re-mobilize to the Site to install and sample the monitoring well network.
Geosyntec personnel will observe the installation of up to 4 exterior monitoring wells to evaluate groundwater quality; the tentative locations were presented on Figure 7 of the SAWP. We have assumed the existing utility survey will be sufficient for siting the proposed monitoring well locations; however, should the spray paint be worn away or if there is any uncertainty in utility locations relative to drilling locations, Geosyntec will submit a WCD or TACO to FDEP for a focused utility locating at the drilling locations.
Additional water-table monitoring wells MW002 through MW004 will be installed upgradient, side gradient and downgradient of the facility using the hollow-stem auger drilling method. The wells will be constructed of 1-in. diameter Schedule 40 PVC pipe with 10 ft of 0.010-in. slotted well screen, filter pack consisting of 20/30 silica sand, and at least a 0.5-ft thick, fine sand (30/65 silica sand) seal. The remaining annular space will be grouted to land surface with Portland Cement Type I/II and monitoring wells will be completed at the surface with a flush-mount 8-in. manhole and a locking monitoring well cap.
Geosyntec will install one vertical extent monitoring well (MW005) to a depth of 45 feet, if DPT groundwater sampling results warrant vertical delineation. The well will also be constructed with a 10-ft screen depending upon the lithologic information from LSB001.
The monitoring wells will be developed until the discharge is relatively sediment-free and sampled no sooner than 24 hours after development. The top-of-casing elevation and location of the monitoring wells will be surveyed by a licensed Florida surveyor.
Groundwater sampling: Geosyntec personnel will collect groundwater samples from the 5 Site monitoring wells. Depth to water measurements (DTW) will be collected from the monitoring wells to evaluate groundwater flow direction. The monitoring wells will be sampled using a peristaltic pump in general accordance with FDEP Standard Operating Procedures (SOPs), assuming the DTW and drawdown do not exceed the suction limit of the peristaltic pump. The samples will be stored on ice and shipped to a NELAP approved, fixed-based analytical laboratory under chain of custody for analysis under standard turnaround times. The groundwater samples will be analyzed for VOCs by EPA Method 8260. Screening for per- and polyfluoroalkyl substances (PFAS) will be conducted at up to five monitoring wells under a separate task assignment.
IDW Sampling: Geosyntec will collect one composite soil sample from the drums generated at each monitoring well location (4 total) and assumes two (2) additional samples will be needed to characterize soil drums generated in Subtasks 2 and 3. Geosyntec will characterize liquid drums based on the groundwater sampling results. The IDW samples will be analyzed for VOCs and RCRA metals
Subtask 4 activities will be performed by 1 Geosyntec staff member assuming 2 days for drilling oversight, and 2 days for well sampling, IDW sampling, and close out tasks at the Site. We have assumed 8-hour days.
Subtask 5 – IDW Management and Surveying
Geosyntec will review the generation of a waste characterization profile as well as the preparation of IDW manifests. Geosyntec will coordinate disposal arrangements for up to 35 drums of non-hazardous IDW. Analytical costs are included to establish the soil and groundwater profiles required by Hulls Environmental Services, the selected waste hauler. Field oversight will be provided for IDW pickup by a Geosyntec staff member.
Subtask 5 also includes costs for a Florida professional land surveyor to survey the five (5) newly installed monitoring wells. We have assumed field oversight of surveying will not be required.
Subtask 6 – Annual Reporting
Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a SAR documenting assessment activities and summarizing analytical results. This subtask includes costs to generate a SAR that summarizes the analytical results of the assessment activities outlined above and complies with the requirements of Chapter 62-780.600 FAC.
