Ms. Brittany Plyler Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road, MS 4520 Tallahassee, FL 32399-2400

Subject: Scope of Work and Level of Effort Annual NAM Groundwater Sampling Classic Cleaners (ERIC_4929) 3307 SE Federal Highway Stuart, Martin County, Florida 34997

Dear Ms. Plyler:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW) and proposed Level of Effort (LOE) for annual Natural Attenuation Monitoring (NAM) at the Classic Cleaners (Site) (ERIC_4929) located in Stuart, Florida.

On 9 June 2023, Geosyntec confirmed the Site property is still owned by Indian Street Shoppes, LLC, with whom the existing site access agreement was executed on March 24, 2016.

SCOPE OF WORK

Subtask 1 – Project Management

Tori Hood will serve as the Project Manager for the proposed SOW. Costs associated with this subtask are directly computed.

Subtask 2 – Annual NAM Monitoring

This subtask includes labor and expenses to conduct the following:

• Geosyntec will mobilize to the Site to conduct annual NAM groundwater monitoring of four (4) monitoring wells: MW001, MW002, MW006, and MW007 (figure attached).

o Exceedances of groundwater cleanup target levels (GCTLs) for tetrachloroethene

(PCE) and trichloroethene (TCE) were detected in MW007 in February 2023 and in MW002 in June 2022.

Classic Cleaners – Annual NAM 19 June 2023

/2023 Annual NAM_Classic Cleaners SOW o MW006 is a downgradient monitoring well that has not had volatile organic compounds (VOHs) above GCTLs since September 2020.

o MW001 is adjacent to the former drycleaning machine and had a PCE exceedance in February 2022.

o Field personnel will measure water levels in 10 site monitoring wells to evaluate groundwater flow direction.

Subtask 3: Investigation Derived Waste (IDW) Management & Annual NAM Report

Purge water generated during sampling activities will be containerized in one (1) 55-gallon drum.

It is assumed that the monitoring well analytical data can be used for waste profiling and an investigation derived waste (IDW) sample will not be required.

Based on the cost estimates provided in Attachment A for IDW disposal, Geosyntec has selected EnviroServe as the most cost-effective transporter and disposal option. We have assumed oversight for drum pickup will not be required.

Company Non-Hazardous PFAS Impacted Water (per 55-gal drum)

EnviroServe $215.00

Erwin $460.00

Hull $860

Geosyntec will prepare a Natural Attenuation Monitoring Report documenting the results of the NAM event. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the field activities completed. A final signed waste manifest for IDW transport and disposal will also be included in the NAM Report.

The report will be signed and sealed by a Florida-licensed professional geologist or engineer. Additionally, effort has been included to discuss conditional closure options with the Site owner prior to report submittal.

Hi Craig, Thanks for chatting with me earlier! Attached are those 2023 IDW quotes we talked about. This is what I was using to build our cost estimate for the transport & disposal of one (1) PFAS impacted groundwater drum at a site located at 3307 SE Federal Highway, Stuart, FL. I just want to confirm if there would be any additional fees associated with the T&D of PFAS impacted water.

Let me know – thanks!

Any terms proposed in Customer's acceptance of this proposal which add to, vary from, or conflict with the proposal or these terms and conditions, are hereby rejected. Any variance in these conditions is considered a change in the scope of work unless expressly otherwise stated in Hull’s proposal. If any of the waste contains materials which do not conform to the descriptions provided by Customer and/or in the Waste Profile Sheets ("non-conforming waste"), Hull’s may, at its option, properly dispose of it, return it to Customer or require Customer to remove and dispose of the non-conforming waste at Customer's expense and reimburse Hull’s for any expenses that it has incurred. The written notice must specifically state the portion in dispute and describe the dispute in such detail that Hull’s has full notice of the dispute. Any terms proposed in Customer's acceptance of this proposal which add to, vary from, or conflict with the proposal or these terms and conditions, are hereby rejected. Any such proposed terms shall be void and the terms in the proposal and these terms and conditions shall constitute the complete and exclusive statement of the terms and conditions of the contract between Hull’s Environmental Services, Inc. (“Hull’s”) and Customer.

Project Documents: Hull’s proposal includes and incorporates Hull’s Rate Schedule which is in effect at the time of performance of the work, all documents provided to Hull’s by or on behalf of Customer and all documents provided to Customer or its representative by or on behalf of Hull’s. The term “Customer” refers to the party with whom Hull’s is contracting. This document is incorporated by reference to the Contract as specified therein and is an integral part of the Contract.

Scope of Work and Price: All work performed hereunder shall be performed and invoiced in accordance with Hull’s written proposal, Hull’s Rate Schedule which is in effect at the time of performance of the work, the other Project Documents, and the terms and conditions stated herein as each may be applicable to the type of work performed. In the event that the scope of work, schedule, or material changes, Customer agrees to pay Hull’s on a time and material basis in accordance with Hull’s then current Rate Schedule or other unit rates, whichever is applicable, unless a revised proposal is prepared by Hull’s and accepted by Customer. Emergency response services shall be performed and invoiced in accordance with Hull’s current Rate Schedule. Unless expressly set forth, Hull’s proposal does not include state or local sales tax. If any such taxes are applicable and the client does not provide a Direct Pay or Tax Exemption Certificate for this work, such taxes will be added to the invoiced amount as a separate line item.

General Conditions of Work: Customer is responsible for furnishing to Hull’s all pertinent data and information concerning the work to be performed hereunder, the nature of the work site and the nature of the conditions to be remediated, including special hazards or risks involved with such work, premises, site or conditions. (d) the work site and all access ways shall be suitable for the size and weight of all vehicles and equipment utilized to perform the work; (e) all wastes shall conform to the representations of Customer and the Project Documents; (f) all non-emergency response related work will be performed Monday through Friday during daytime business hours between 8:00 a.m. and 4:00 p.m. (work performed outside of this time frame will be charged at 1.5 times the standard rates or as otherwise provided in Hull’s then current Rate Schedule; work performed on Sundays and holidays will be charged at two times the standard rates); (g) Customer is responsible for all damage to equipment and its components not caused by the direct fault of Hull’s; and (h) Customer is responsible for all costs associated with overloading of containers or trucks including citations, damages to equipment or property, loss of revenue, etc., unless loaded by Hull’s. Customer is expressly prohibited from allowing any other carrier to move Hull’s equipment without the prior written consent of Hull’s. The written notice must specifically state the portion in dispute and describe the dispute in such detail that Hull’s has full notice of the dispute.
