HW683_DC018C

This Task Assignment authorizes Geosyntec to conduct Groundwater Monitoring and PFAS sampling at Former First Class Cleaners site in accordance Geosyntec’s scope of work, cost estimate and schedule received on , 2025.

Melissa Shook is the Project Manager.

Subtask 2 and 4 – Geosyntec will mobilize to the Site to collect depth-to-groundwater measurements from each accessible monitoring well (28 wells) within a 24-hour period, collect 16 groundwater samples via low flow methods for 2025 annual groundwater monitoring. The groundwater samples will be submitted for laboratory analysis of VOHs by Environmental Protection Agency (EPA) Method 8260B. Geosyntec will prepare a report summarizing sampling activities and results and will provide conclusions and recommendations based on data evaluation. The report will contain text, tables and figures summarizing field/laboratory data.

Subtask 3 - During the Subtask 2 mobilization, Geosyntec personnel will collect three groundwater samples from the monitoring wells shown in the table below for PFAS analysis. Two of the proposed wells (MW009 and MW012) are being sampled for VOH analysis and approximately 0.25 hours of additional time is allotted to sample these wells for PFAS whereas labor for MW037 is included for sampling since it is not being sampled in Subtask 2.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $1 with 11 labor hours.

First Class_2025 Annual GWM_SOW

December 16, 2025

Ms. Sarah Aduwa Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road Tallahassee, FL 32399-2400

Subject: Statement of Work – Annual Groundwater Monitoring Former First Class Cleaners 591 East Semoran Boulevard Casselberry, Seminole County, Florida

ERIC_5489

Dear Sarah:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this statement of work (SOW) and attached level of effort (LOE) cost estimate for the Florida Department of Environmental Protection (FDEP) to conduct annual groundwater monitoring at the former First Class Dry Cleaner Site (FDEP Facility ID ERIC_5489). The Site is located at 591 East Semoran Boulevard, Casselberry, Seminole County, Florida. Proposed sampling activities are in accordance with the recommendations provided to FDEP from the Annual Groundwater Monitoring Report 2024.

Groundwater samples were last collected at the Site in July 2024.

The purpose of the annual groundwater monitoring is to collect groundwater samples for analysis of volatile organic halocarbons (VOHs) and Per- and polyfluoroalkyl substances (PFAS) to evaluate groundwater quality at the Site. The attached LOE consists of four subtasks and is summarized below.

SITE ACCESS

1. The Site property, located at 591 SR 436, Casselberry, FL 32707 (Parcel ID 20-21-30- 300-0A1G-0000) is owned by 591 SR 436, LLC. The existing agreement for this property was authorized in July 2021 by Tracy K. King, representative of 591 SR 436, LLC, and is still valid. However, the existing agreement should be updated to the latest FDEP site access agreement language. Therefore, an updated Source-Property Access Agreement will be pursued but will not be required to be in place in order to collect samples.

December 16, 2025

2. Several properties owned by the City of Casselberry contain monitoring wells requiring sampling and/or water level gauging. These properties are 92 Wilshire Dr. Casselberry FL 32707 (Parcel ID: 21-21-30-501-0200-0040), 720 SR 436 Casselberry FL 32707 (Parcel ID 21-21-30-501-0200-0000), and Easements. The existing agreement for these properties was authorized in 24 June 2024 by James Newlon and is valid.

3. MW027, MW028, and MW029 are located within the property at 830 Sr 436 Casselberry Fl 32707 (Parcel ID 21-21-30-501-0100-0000), owned by Floorsystems Inc. as of 20 April 2024 and is valid.

Subtask 1 – Project Management Melissa Shook will serve as the Geosyntec project manager, and her hours are directly computed in the LOE/CEF.

Subtask 2 – Annual Groundwater Monitoring Geosyntec will mobilize to the Site to collect depth-to-groundwater measurements from each accessible monitoring well (up to 34 total wells) within a 24-hour period, collect 16 groundwater samples via low flow methods for 2025 annual groundwater monitoring. Refer to the sampling plan provided in Attachment A.

Groundwater levels will be measured on the first day. Each of the 28 monitoring wells are flush-mounted, in high-traffic areas, and/or prone to collecting stormwater inside the manhole.

Geosyntec personnel will bail the manholes, measure and record groundwater levels, and re-secure the manholes. Additional time has been included due to the site-specific challenges, including (i) long distances between wells necessitating driving time and traffic navigation and (ii) flush-mounted wells located in vegetated and overgrown areas necessitating the use of a metal detector to locate wells.

Geosyntec will collect 16 groundwater samples from the monitoring well network using low-flow sampling techniques. The groundwater samples will be submitted for laboratory analysis of VOHs by Environmental Protection Agency (EPA) Method 8260B.

Geosyntec will use a metal detector and a GPS to locate several monitoring wells in overgrown vegetated areas. Geosyntec assumes the sampling event will take 2 people 3 days to complete and will require one truck per person.

Investigation derived waste (IDW) will be collected in two 55-gallon drums, which will be labeled, covered with plastic, and wrapped in orange construction fencing. Geosyntec obtained IDW transportation and disposal (T&D) estimates from Hulls, Erwin Remediation, LLC, and York

Environmental (Attachment B). It is assumed that 1 drum of IDW will be generated and that the waste will be disposed of as PFAS-containing waste due to historical PFAS concentrations at the site. Each company is a licensed waste hauler in Florida. Erwin Remediation was selected due to the lowest overall cost. No IDW sample will be collected for waste characterization, as Erwin Remediation has a current waste profile for First Class Cleaners.

Subtask 3 – PFAS Monitoring

During the Subtask 2 mobilization, Geosyntec personnel will collect three groundwater samples from the monitoring wells shown in the table below for PFAS analysis.

Geosyntec will prepare an email deliverable with attachments, including one figure with groundwater analytical results for PFAS (showing PFOA, PFOS, and the summation of PFOA and PFOS concentrations for each well sampled and no isoconcentration lines), certified analytical laboratory report, and field forms including equipment calibration logs, and groundwater sampling forms. The email deliverable will be submitted to the FDEP within two working days of receipt of the final certified analytical laboratory report from the FDEP laboratory.

Geosyntec assumes the final analytical laboratory report will be emailed to Melissa Shook (melissa.shook@geosyntec.com) by the FDEP laboratory after the report is finalized. The email deliverable will be submitted to Sarah Aduwa.

Additionally, the PFAS results will also be incorporated into Subtask 4. Costs for analysis and integration into the Subtask 4 deliverable are covered under Subtask 3.

Subtask 4– Natural Attenuation Monitoring Report Geosyntec will prepare a report summarizing sampling activities and results and will provide conclusions and recommendations based on data evaluation. The report will contain text, tables and figures summarizing field/laboratory data.

A total of three tables will be included: (i) well construction/groundwater elevation data,

(ii) groundwater analytical results, and (iii) natural attenuation data.

MW ID Screened Interval Sampling Rationale

MW037 5-15 ft BLS PFOA+PFOS P-GCTL exceedance in May 2023.

MW012 10-15 ft BLS Downgradient of MW037.

MW009 10-15 ft BLS Sidegradient of MW037.

A total of eight figures will be included: (i) site location, (ii) site features, (iii) groundwater elevation contours (one for each of three depth intervals), and (iv) VOH concentrations (one for each of three depth intervals).

The report will be signed/sealed by a Florida-licensed geologist/engineer.

Deliverable Schedule and Invoicing Note that this schedule reflects a two-week FDEP review of LOE. Geosyntec anticipates the following schedule of deliverables and invoices.

Good morning Larry, In light of the update you recently provided about LES, I was hoping you might be able to prepare an updated IDW disposal quote (previous version attached) for use as part of future proposals. Please see the below list of scenarios I need updated waste disposal costs for through the remainder of 2025. We will reach out again at the beginning of 2026 for updated rates.

Scenario 1

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has not been investigated for at the site. $260.00 per drum solid or liquid less than 50 miles- Add $95 per additional 50 miles

Scenario 2

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the laboratory's method detection limit but is below the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums- $380.00

Scenario 3

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums -$380.00

Scenario 4

FDEP PRP site where VOA concentrations are below hazardous levels for both soils and groundwater. $260.00 per drum solid or liquid less than 50 miles- Add $95 per each additional 50 miles

Please let me know if you have any questions or if you need anything else from me to support your efforts on this. If any of the above scenarios requires additional analytical results beyond those described, please let me know so I can include those in the overall cost evaluation. I really appreciate your help with this!

Thank you, Blaine Dawson, PG (FL,TN) Senior Geologist p g p g $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums- $380.00
