About this file
This document is a Task Assignment Change Order (TACO) issued by the State of Florida Department of Environmental Protection (FDEP) to amend an existing task assignment under Contract Number HW680. The parties involved are FDEP, serving as the state agency, and Arcadis U.S. Inc., the contractor. The change order is identified as TACO Number 1 for Task Assignment DC007C and relates to environmental remediation work at the Herman Jackson Cleaners facility located at 1981 San Marco Boulevard, Jacksonville, Duval County, Florida (FACID NO. 16-9502441; ERIC_4587). Theresa Pepe serves as the FDEP project manager, while Gus Suarez represents Arcadis U.S. Inc. as the contractor's project manager.
The task assignment requires Arcadis to conduct annual groundwater monitoring from select wells at the Herman Jackson Cleaners site according to a Work Plan dated January 17, 2025. The change order does not modify the contract price; the total task assignment value remains fixed at $14,323.56. However, the change order extends the task completion date by 32 days, from July 31, 2025, to September 1, 2025, to allow time for receipt and analysis of laboratory results. The extension was requested because Arcadis had not yet received laboratory reports required to complete the Annual Monitoring Report and Remedial Action Plan (AMR) deliverable, which was originally due on April 25, 2025. The deliverable consists of an Annual Monitoring Report/Injection/Disposal Well (IDW) manifest and project management activities, with payment contingent upon FDEP approval of these materials. Arcadis may invoice the full $14,323.56 upon submission of the approved monitoring report.

Theresa, We were able to sample MW-21, MW-8, and MW-12 as replacement wells, and completed the sampling on 4/11/25. We are currently waiting on lab results and expect to receive them on 4/23/25 at the earliest.
Because of this I would like to request a due date extension for the AMR which is due on 4/25/25.
Thanks, Mike
