FACID NO. 29-9501239; ERIC_4725

Tampa, Hillsborough County FROM: Theresa Pepe, Project Manager SUBJECT: Task Assignment DC051B (HW680) DATE: December 3, 2025

This Task Assignment authorizes Arcadis to complete a Site Assessment at the Jaees Real Cleaners site. The initial fieldwork will be completed in approximately twelve days, and a Site Assessment Report (SAR) will be submitted as the final deliverable. The assessment may be expanded or reduced via change order depending on the complexity of the site. All work will be performed in accordance with the approved work plan.

The final task assignment total is $99,565.16. After requested revisions, total hours were reduced from 328 to 305 and total cost was reduced by $2,258.33. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

A minor error in the spreadsheet showing the wrong number of hours for PM was corrected in the final version.

Subtask 2: Site Assessment This subtask provides funds for soil, vapor and groundwater screening and monitor well installation at the facility. Arcadis will present the data in an interim deliverable after which a change order may be authorized for additional work, if needed. The Project Manager approved sampling locations with minor comments and the overall approach to the assessment presented in the July 2025 Work Plan.

INITIAL SITE ASSESSMENT WORK PLAN

www.arcadis.com 2025.07_Jaes Real Cleaners_ISA Work Plan 4 vapor point was installed in the former Jaes facility and the other two vapor points were installed in adjacent units to the north and south (DTLR and Metro by T-Mobile). Additionally, three indoor air samples and one outdoor ambient air sample were also collected for laboratory analysis. Analytical results indicated concentrations of PCE above the residential and/or commercial vapor intrusion screening levels (VISLs) in all three sub-slab vapor samples. PCE was also detected in the indoor air sample collected from the Metro by T-Mobile tenant space;

however, the reported PCE concentration was below the residential and commercial target indoor air concentrations (TIACs). Based on the March 2025 results, AEI recommended periodic inspection/maintenance of the floor concrete slab, as well as periodic indoor air testing to verify air quality at the investigated tenant spaces.

4 INITIAL SITE VISIT

On July 3, 2025, Arcadis performed an initial site visit to observe the current layout and use of the Site and surrounding properties, establish the immediate future use, verify the presence of utilities, potential drill locations/access, and complete applicable portions of the Site Visit Checklist. No on-site monitor wells were found during the visit. Arcadis spoke to the current property owner representative, Mr. Joseph Savaro (Property Manager), prior to the visit to coordinate access. It was confirmed that municipal drinking water and sanitary sewer service is provided to the Site; no septic systems currently exist. Natural gas is also supplied to the Site.

The historic Site layout from Site Screening Report appears to coincide with the field observations made on July 3, 2025. A single inactive drycleaning machine is currently located on the west side of the former boiler room. No other drycleaning equipment was observed at the Site. The Site currently operates as Real Dry Cleaners, and on-site activities are limited to pick-up/drop-off services. A photograph log with pertinent Site photos is presented in Appendix A and a copy of the completed Site Visit Checklist is presented in Appendix B.

5 INITIAL SITE ASSESSMENT WORK PLAN

A flexible assessment approach will be used to create a site-specific unsaturated soil and groundwater database capable of determining the absence/presence and extent of chlorinated drycleaning solvents above soil and groundwater CTLs. In addition, potential risks and exposure pathways will be identified and preliminary recommendations and design provided for remedial action, if justified. The proposed assessment activities will be completed in accordance with FDEPs Standard Operating Procedures (SOP) (001/01), the site-specific HASP, Chapter 62-780 Florida Administrative Code (F.A.C.), and recent guidance provided in the July 13, 2022, Waste Cleanup Program Pollution Response Action Contract (WCP PRAC) meeting in Tallahassee, Florida.

Based on the reported historical contamination in the shallow soil and groundwater, the reported depth-to-water (~26-32 ft bls) and site layout, multiple assessment methodologies are proposed. Unsaturated soil assessment will be completed by the installation of soil borings above the water table, with collection of soil samples for fixed-based laboratory analysis of VOCs and completion of a Modified Active Soil Gas Survey (MAGS).

The first phase of groundwater assessment will be completed as follows:

Installing a monitor well (MW001) outside the facility, near the rear door, for fixed-based laboratory analysis of VOCs, Completing vertical profiling using direct-push technology (DPT) for fixed-based laboratory analysis of VOCs;

and, Installing piezometers to assess groundwater gradient conditions.

2025.07_Jaes Real Cleaners_ISA Work Plan 5

Following receipt and evaluation of the first phase of assessment, the scope of the next phase of investigation activities, such as additional monitor wells, will be developed in consultation with FDEP. The site assessment methods and techniques are described in detail below.

5.1 Site Preparation and Utility Locate

Arcadis will contact the current site operator and current property owner to share proposed sampling locations and advise schedule/notification requirements prior to completing field work. Arcadis will oversee a subsurface private utility locate using ground penetrating radar (GPR) on the Site and surrounding areas. The purpose of the utility locate will be to establish/confirm the suspected locations of underground utilities (Sunshine State One Call will initially locate utilities on and surrounding the Site). The initial Site visit indicated the presence of underground water, sewer, electric, communications, and gas lines.

5.2 Soil Assessment and Sub-Slab Soil-Gas Sampling

5.2.1 Unsaturated Soil Assessment

One continuous soil boring (SB001) will be installed in the rear of the facility to a depth of approximately 50 ft bls, or into the basal confining clay at the bottom of the SAS, to characterize site lithology. A lithologic log of the general geology at the site will be developed from the soil samples collected from this soil boring. Soil samples from each boring will be described by an on-site Arcadis geologist.

To confirm if past drycleaning operations at the Site had an adverse impact to unsaturated soil quality conditions, up to 9 soil borings (SB001-SB009) for collection of soil samples are proposed for installation at the locations shown on Figure 3. The initial sample location will be placed in vicinity of historical soil boring B1; additional boring/sample locations will be placed within and surrounding the building. Soil boring locations may be adjusted based on the GPR results and location of buried utilities/infrastructure.

The soil borings will be installed using a hand auger and DPT. A concrete coring machine will be used to bore through the concrete floor inside the building to access the underlying soils. Soil samples for soil borings located within the facility will be collected at 0.5 ft bls, 2 ft bls, and then at two-foot depth intervals to 12 ft bls. Exterior soil samples (SB001 and SB002) will be collected at 0.5 ft bls, 2 ft bls, and then at two-foot depth intervals to 10 ft bls, followed by five-foot intervals to the water table. All soil samples (up to 7 samples/boring x 7 interior/under overhang boring locations and up to 10 samples/boring x 2 exterior locations or a total of up to 69 soil samples) will be submitted to a fixed-based laboratory for confirmatory analysis of VOCs by EPA Method 8260. Each soil sample will also be collected in encore samplers in anticipation of SPLP analysis. Five SPLP analyses are included in the initial costs. Following the sampling, the boreholes will be converted to a MAGS point, or sealed with cement grout.

5.2.2 Modified Active Gas Sampling

MAGS is proposed for completion inside and outside the building. MAGS will be completed in general agreement with the Modified Active Gas Sampling Manual, dated October 12, 2004. MAG001, co-located with SB001, will be constructed with 2-inch diameter polyvinyl chloride (PVC) with 0.010-inch slotted screen set approximately 10 feet below ground surface to approximately one foot above the water table, estimated to be 26-32 ft bls, resulting in an

2025.07_Jaes Real Cleaners_ISA Work Plan 6 assumed screen interval of 10 to 25 ft bls. The remaining eight MAGS wells (MAG002 MAG009) will be installed using 2-inch diameter PVC with 0.010-inch slotted screen set approximately one foot below ground surface to approximately 11 ft bls, resulting in an assumed screen interval of 1 to 11 ft bls. Final MAGS well construction will be field determined. One vapor sample will be collected from each MAGS well for analysis of VOCs by EPA Test Method TO-15. Proposed MAGS well layout locations are depicted on Figure 3.

5.3 Groundwater Assessment

5.3.1 Groundwater Vertical Profiling

Groundwater assessment will be conducted utilizing vertical profiling by a direct-push drill rig to collect water samples for VOCs analysis. Seven vertical profile (VP) locations (VP001 VP007) are proposed as shown on Figure 3. Assuming the water table will be encountered at approximately 26 ft bls and the base of the surficial aquifer is approximately 50 ft bls, samples starting at the water table and at 10-ft depth intervals to a total depth of 50 ft-bls (26-30, 36-40, and 46-50 ft bls) will be collected. Lithologic information from Soil Boring SB001 will be used to determine sample intervals. Each VP water sample will be submitted to a fixed-based laboratory for confirmatory analysis of VOCs by EPA Method 8260. Following sampling, each vertical profile borehole will be converted to a piezometer or sealed with cement grout.

5.3.2 Piezometer Installation

Arcadis will install three piezometers at select locations to provide groundwater gradient information. For purposes of this Work Plan, it is assumed that three shallow piezometers (PZ001 through PZ003) will be installed with 10-feet of 2-inch diameter, 0.010-inch slot pre-pack screen, between 25 to 35 ft bls using DPT (Figure 3).

Final locations, dimensions, including depth, and screen interval will be determined based upon groundwater elevations encountered during the initial assessment activities. The piezometers will be developed and completed in a method to allow them to be converted to monitor wells in the future, if necessary. Monitor wells and piezometers will be professionally surveyed so that the groundwater gradient can be determined.

5.3.3 Monitor Well Installation and Sampling

Arcadis will install an exterior monitor well (MW001) near the rear door of the facility at the location presented on Figure 3. The well will be constructed with 2-inch diameter, 0.010-inch slot pre-pack screen, between 25 to 35 ft bls using DPT. Following installation, MW001 will be developed and finished at the surface with a flush-to-grade surface completion. Arcadis will sample MW001 per SOP 001/01 for laboratory analysis of VOCs using EPA Method 8260.

Based on the results of the initial assessment activities and vertical profiling results, the installation of additional monitor wells and sampling/analyses in a subsequent phase will be provided to FDEP as required. Also, sampling for per- and polyfluoroalkyl substances (PFAS) will be screened per FDEP guidance and tasked separately.

6 REPORTING

An Initial Site Assessment Report (ISAR) including field logs, photographs, laboratory reports, and results summary will be submitted to FDEP within 30 days of receipt of laboratory data as an interim deliverable to

2025.07_Jaes Real Cleaners_ISA Work Plan 7 document the fieldwork. The ISAR will provide a basis for the next phase of work which may include additional soil sampling, the installation of additional monitor wells, etc. to be completed under a Task Assignment Change Order (TACO).

A final Site Assessment Report (SAR) will serve as the final deliverable and will describe the results of the proposed site assessment tasks described herein. The SAR will be submitted to FDEP within 60 days of completion of the field work. The SAR will contain field logs, laboratory reports, tables, and figures summarizing the results of assessment activities and include conclusions and recommendations based upon findings.

7 INVESTIGATIVE DERIVED WASTE

Investigative derived wastes (IDW - drill cuttings and purge water) generated during site assessment activities will be drummed, labeled and temporarily staged on-site in Department of Transportation (DOT)-approved, 55-gallon steel drums. Arcadis will coordinate with a waste disposal contractor for non-hazardous disposal of the IDW and provide copies of final disposal manifests to FDEP.

8 QUALITY ASSURANCE & QUALITY CONTROL

All fieldwork will be performed in accordance with FDEPs SOP (001/01), and the site-specific HASP, to be submitted under separate cover prior to commencing field activities. Additionally, sample analyses will be performed by NELAP-certified laboratories to method practical quantitation limits.

9 PROJECT SCHEDULE AND COST

Cost estimate form/level of effort (CEF-LOE) worksheets and subcontractor bid copies for the implementation of this ISAWP will be provided separately upon approval of this Work Plan. Arcadis will proceed with scheduling field activities upon receipt of the task assignment from the FDEP.

Great, thanks Theresa. One more thing, GeoView is now saying a full day ($1,500) for GPR, instead of the original half day ($1,000). See attached. This will allow for clearing inside the red box for the southern borings recommended by FDEP and step outs if needed. Can we add that in now?

GeoView at $1,500 is still less than GPRS ($2,100) and Blood Hound ($2,112). Site Lines ($1,100) costs remain the same but are not available until 12/15.
