INITIAL SITE ASSESSMENT WORK PLAN
www.arcadis.com 2024.05.31_Prestige Cleaners_ISA Work Plan 4
INITIAL SITE VISIT
On May 8, 2024, Arcadis performed an initial site visit to observe the current layout and use of the site and surrounding properties, establish the immediate future use, verify the presence of utilities, potential drill locations/access, and complete applicable portions of the Site Visit Checklist. No on-site monitor wells were found during the visit. Arcadis spoke to the current property owner representative, Ms. Erin Feather (Tenant Relations Manager), prior to the visit to coordinate access. It was confirmed that municipal drinking water and sanitary sewer service is provided to the site; no septic systems currently exist. Natural gas is also supplied to the site.
The previous site layout from Site Screening Report is provided on Figure 2. There is currently no drycleaning equipment at the site and the unit is operated as a hair salon (Star Hair Salon). Current site photographs are presented in Appendix A and a copy of the completed Site Visit Checklist is presented in Appendix B.
INITIAL SITE ASSESSMENT WORK PLAN
A flexible assessment approach will be used to create a site-specific unsaturated soil and groundwater database capable of determining the absence/presence and extent of chlorinated drycleaning solvents above soil and groundwater CTLs. In addition, potential risks and exposure pathways will be identified and preliminary recommendations and design provided for remedial action, if justified. The proposed assessment activities will be completed in accordance with FDEP’s Standard Operating Procedures (SOP) (001/01), the site-specific HASP, Chapter 62-780 Florida Administrative Code (F.A.C.), and recent guidance provided in the July 13, 2022, Waste Cleanup Program Pollution Response Action Contract (WCP PRAC) meeting in Tallahassee, Florida.
Based on the reported historical contamination in the shallow groundwater, the reported depth-to-water (~5 ft bg) and site layout, multiple assessment methodologies are proposed. Unsaturated soil assessment will be completed by the installation of soil borings above the water table, with collection of soil samples for fixed-based laboratory analysis of VOCs and completion of a Modified Active Soil Gas Survey (MAGS).
The first phase of groundwater assessment will be completed as follows:
Installing a source area monitor well (MW001) near the former drycleaner machine location (currently unknown for the former Prestige facility) and a monitor well (MW002) near the historical temporary monitor well (TMW2) for fixed-based laboratory analysis of VOCs, Completing vertical profiling using direct-push technology (DPT) for fixed-based laboratory analysis of VOCs;
and, Installing piezometers to assess groundwater gradient conditions.
Following receipt and evaluation of the first phase of assessment, the scope of the next phase of investigation activities, such as additional monitor wells, will be developed in consultation with FDEP. The site assessment methods and techniques are described in detail below.
Site Preparation and Utility Locate Arcadis will contact the current site operator and current property owner to share proposed sampling locations and advise schedule/notification requirements prior to completing field work. Arcadis will oversee a subsurface private utility locate using ground penetrating radar (GPR) on the site and surrounding areas. The purpose of the
2024.05.31_Prestige Cleaners_ISA Work Plan 5 utility locate will be to establish/confirm the suspected locations of underground utilities (Sunshine State One Call will initially locate utilities on and surrounding the site). The initial site visit indicated the presence of underground water, sewer, electric, communications, and gas lines.
Soil Assessment and Sub-Slab Soil-Gas Sampling
Unsaturated Soil Assessment One continuous soil boring (SB001) will be installed in the rear of the facility to a depth of approximately 50 ft bls or into the basal confining clay at the bottom of the SAS to characterize site lithology. A lithologic log of the general geology at the site will be developed from the soil samples collected from this soil boring. Soil samples from each boring will be described by an onsite Arcadis geologist.
To confirm if past drycleaning operations at the site had an adverse impact to unsaturated soil quality conditions, up to 8 soil borings (SB001-SB008) for collection of soil samples are proposed for installation at the locations shown on Figure 3. The initial sample location will be placed in vicinity of the former temporary monitoring well (TMW2); additional boring/sample locations will be placed within and surrounding the building. Soil boring locations may be adjusted based on the GPR results and location of buried utilities/infrastructure.
The soil borings will be installed using a hand auger and DPT. A concrete coring machine will be used to bore through the concrete floor inside the building to access the underlying soils. Soil samples at all soil borings will be collected at 0.5 ft bls, 2 ft bls, and then at two-foot depth intervals to the water table (estimated to be at 4-7 ft bls).
All soil samples (up to 4 samples/boring x 8 initial boring locations or a total of up to 32 soil samples) will be submitted to a fixed-based laboratory for confirmatory analysis of VOCs by EPA Method 8260. Each soil sample will also be collected in encore samplers in anticipation of SPLP analysis. Five SPLP analyses are included in the initial costs. Following the sampling, the boreholes will be converted to a MAGS well, or sealed with cement grout.
Modified Active Gas Sampling MAGS is proposed for completion inside and outside the building. MAGS will be completed in general agreement with the Modified Active Gas Sampling Manual, dated October 12, 2004. Six MAGS wells (MAG01 – MAG06), constructed of 2-inch diameter PVC with 0.010-inch slotted screen set approximately one foot below ground surface to approximately one foot above the water table, estimated to be 4-7 ft bls, resulting in an assumed screen interval of 1 to 4 ft bg. Final MAGS well construction will be field determined. One vapor sample will be collected from each MAGS well for analysis of VOCs by EPA Test Method TO-15. Proposed MAGS well layout locations are depicted on Figure 3.
Groundwater Assessment
Groundwater Vertical Profiling Groundwater assessment will be conducted utilizing vertical profiling by a direct-push drill rig to collect water samples for VOCs analysis. Seven vertical profile (VP) locations (VP001 – VP007) are proposed as shown on Figure 3. Assuming the water table will be encountered at approximately 5 ft bls and the base of the surficial aquifer is approximately 50 ft bls, samples starting at the water table and at 10-ft depth intervals to a total depth of
2024.05.31_Prestige Cleaners_ISA Work Plan 6
49 ft-bls (5-9, 15-19, 25-29, 35-39, and 45-49 ft bls) will be collected. Lithologic information from Soil Boring SB001 will be used to determine exact sample intervals. Each VP water sample will be submitted to a fixed-based laboratory for confirmatory analysis of VOCs by EPA Method 8260. Following sampling, each vertical profile borehole will be sealed with cement grout.
Piezometer Installation Arcadis will install three piezometers at select locations to provide groundwater gradient information. For purposes of this Work Plan, it is assumed that three shallow piezometers (PZ001 through PZ003) will be installed with 10-ft of 0.010-inch slot pre-pack screen, between 2 to 12 ft bls using DPT (Figure 3). Final locations, dimensions, including depth, and screen interval will be determined based upon groundwater elevations encountered during the initial assessment activities. The piezometers will be developed and completed in a method to allow them to be converted to monitor wells in the future, if necessary. Monitor wells and piezometers will be professionally surveyed so that the groundwater gradient can be determined.
Monitor Well Installation and Sampling Arcadis will hand-install a source area well (MW001) into the water table, as deep as possible, at the location presented on Figure 3. The well will be constructed with 0.010-inch slot pre-pack screen, developed, and completed within a manhole. A second monitor well (MW002) will be installed adjacent to the historical temporary monitoring well (TMW2) that was referenced in the DSCP documentation. MW002 will be installed with 10-ft of 0.010-inch slot pre-pack screen, between 2 to 12 ft bls using DPT (Figure 3). Arcadis will sample MW001 and MW002 per SOP 001/01 for laboratory analysis of VOCs using EPA Method 8260.
Based on the results of the initial assessment activities and vertical profiling results, the installation of additional monitor wells and sampling/analyses in a subsequent phase will be provided to FDEP as required. Also, sampling for per- and polyfluoroalkyl substances (PFAS) will be screened per FDEP guidance and tasked separately.

An Initial Site Assessment Report (ISAR) including field logs, photographs, laboratory reports, and results summary will be submitted to FDEP within 30 days of receipt of laboratory data as an interim deliverable to document the fieldwork. The ISAR will provide a basis for the next phase of work which may include additional soil sampling, the installation of additional monitor wells, etc. to be completed under a Task Assignment Change Order (TACO).
A final Site Assessment Report (SAR) will serve as the final deliverable and will describe the results of the proposed site assessment tasks described herein. The SAR will be submitted to FDEP within 60 days of completion of the field work. The SAR will contain field logs, laboratory reports, tables, and figures summarizing the results of assessment activities and include conclusions and recommendations based upon findings.
INVESTIGATIVE DERIVED WASTE
Investigative derived wastes (IDW - drill cuttings and purge water) generated during site assessment activities will be drummed, labeled and temporarily staged on-site in Department of Transportation (DOT)-approved, 55-gallon
2024.05.31_Prestige Cleaners_ISA Work Plan 7 steel drums. Arcadis will coordinate with a waste disposal contractor for non-hazardous disposal of the IDW and provide copies of final disposal manifests to FDEP.
QUALITY ASSURANCE & QUALITY CONTROL
All fieldwork will be performed in accordance with FDEP’s SOP (001/01), and the site-specific HASP included under separate cover. Additionally, sample analyses will be performed by NELAP-certified laboratories to method practical quantitation limits.
PROJECT SCHEDULE AND COST
Cost estimate form/level of effort (CEF-LOE) worksheets and subcontractor bid copies for the implementation of this ISAWP will be provided separately upon approval of this Work Plan. Arcadis will proceed with scheduling field activities upon receipt of the task assignment from the FDEP.

Cascade will provide the following 1.VAPOR EXTRACTION POINTS: Core six (6) cores, at least 4-inches in diameter, through concrete slab approximately 6-inches in thickness. Install 2-inch 0.010 slotted PVC screen, 3-feet in length (assumed) in each borehole from 1-4 ft bls. Complete with steel manhole flush to grade.
2.MONITOR WELLS: Core one 6-inch diameter core through concrete and hand install one monitor well inside building (MW001). Complete well with 1-inch prepacked screens, up to 12 feet deep. Bring 5-foot length prepacked screens to provide more flexibility in depths. Complete well within steel manhole flush to grade and develop for 30 minutes. Using direct push, install a second monitor well (MW002) with 10-ft of 0.010-inch slot pre-pack screen, between 2 to 12 ft bls, with 1-inch diameter prepacked screen completed within steel manhole and 2’x2’ concrete pad. Assume 30 minutes for well development
3.LITHOLOGY SOIL BORING: Using direct push, install one boring (SB001) to 50 feet for lithology description using a Macro-Core equipped with a piston-rod assembly for depth-discrete samples.
4.SOIL BORINGS: Assist in coring through concrete and/or asphalt and install up to eight (8) soil borings (SB001 – SB008) for the collection of soil samples. Using a hand auger, advance soil borings to the top of the water table (estimated to be at 4-7 ft bls). Note, most soil borings are co-located with vapor extraction, vertical profile, monitor well, or piezometer locations.
5.VERTICAL PROFILE POINTS: Using direct push at seven (7) vertical profile locations, collect grab water samples at the following depths. One of the vertical profile boring locations will be the same as the soil boring described above.
6.PIEZOMETERS: Install three (3) piezometers screened 2-12 ft below ground, constructed with 1-inch diameter prepacked screen completed within steel manhole and 2’x2’ concrete pad. The piezometers will be installed at the locations shown on the attached figure. Assume 30 minutes for development at each piezometer.

Customer has requested an estimate for the following scope of work:
• Client has requested an estimate to horizontally locate detectable underground utilities within a 10' radius of multiple interior and exterior proposed soil bores in areas as shown on the provided exhibit and as directed on site.
Customer has requested approximate depths to be noted on site sketch. Please note that all depths are approximate and can only be verified using vacuum excavation.
· All estimated values are subject to change (+/-) once the technician arrives on site and conducts a proper site assessment. A Not to Exceed amount can be provided upon request, this will increase the estimate.
· Client will only be invoiced for time needed to complete the stated SOW with a two-hour minimum plus trip charge where applicable. Customer is advised if less time onsite is required, invoicing will be reduced to no less than the minimum fee.
Any additional time required on site to complete the task beyond the estimated / minimum time will be invoiced in 15-minute increments of the provided hourly rates, upon customer approval.
· Estimated time onsite does not include time for any site-specific training, escorting or ingress / egress time which if required, shall be an additional billable charge up to and including exceeding the estimate.
· BHUG will attempt to locate public utilities to the best of our ability when requested, with the information provided.
However, not having access to utility records, plans, maps, or utility enclosures may be cause for some utilities to be unaccounted for, therefor it is always the customer responsibility to call 811.
· The performance of BHUG’s services is limited to full and unobstructed access to include but not limited to mechanical rooms, manholes, hand holes, vaults, meter rooms, telecom rooms, fixtures (plumbing, electrical, communication), dispensers, fenced compounds, tanks and structures. It may be necessary to have parked vehicles or machinery moved to allow for a full scan and to access structures. Full cooperation from the on-site personnel is necessary to perform a complete survey.
· Any available as-builts, engineered or other record drawings, if available, should be supplied to us prior to providing an estimate and commencement of field work. Any drawings supplied to us after the estimate has been delivered to the customer will require a mandatory review of the estimate and potential revisions to the scope of work and associated pricing.
