Place rock borrow onto the prepared foundation. Do not compact the first 4 feet of embankment. Place and compact subsequent lifts according to Subsection 204.10.
Monitor each embankment stage via settlement plate assemblies and piezometers according to Subsections 621.03A(b) and 271.04. Do not place additional embankment material until approved by the CO.
Construct remaining embankment in stages up to 4 feet thick, up to the proposed finished grade. Do not exceed the fill slopes noted in the plans. Install stabilization geogrid according to Subsection 207.04 a minimum of 2 feet below proposed subgrade.
Continuously monitor temporary culverts or diversions and adjust or reinstall at the beginning of each stage as directed by the CO.
Provide two-way traffic during all non-work periods. Construct ramps to the vertical profile according to the requirements in the plans or as approved. Provide barriers or vertical panels according to Section 635 to protect settling slopes during traffic.
F-6 DIVISION 200
Project: AK PFH 58(1), Neck Lake Road Reconstruction: NPOWI Hwy. to Marine Access Facility Jct.
Surcharge the proposed subgrade with an additional 3 feet of suitable material. Place roadway aggregate on the surcharge according to Section 302 to allow for extended periods of public traffic. Provide barriers or vertical panels to protect settling slopes during periods of public traffic.
Monitor and repair the embankment within the surcharge area during work and non-work periods as directed by the CO to provide for public safety and the minimum requirements of two-way traffic specified in the plans.
Remove surcharge embankments when either:
(1) Settlement data obtained from the settlement plate assembly and piezometers indicate primary consolidation is complete with written concurrence from the CO.
(2) 120 days after placement of surcharge embankment.
Incorporate the surcharge embankment material in the roadway embankments once the surcharge has been removed.
(c) Soft Ground Embankment Construction, Type 3. Install a minimum of one settlement plate assembly according to Subsection 621.03A(a) and at least one piezometer according to Subsection 271.03 prior to placing the first compacted embankment stage. Review each planned location with the CO before installation.
In areas of standing water, do not install settlement plate assemblies until the embankment material has reached an elevation of 12 inches above the static water level.
Place rock borrow onto the prepared foundation. Do not compact the first 4 feet of embankment. Place and compact subsequent lifts according to Subsection 204.10.
Monitor the embankment via settlement plate assemblies and piezometers according to Subsections 621.03A(b) and 271.04. Do not place additional embankment material until approved by the CO.
Construct the remaining embankment up to the proposed finished grade. Do not exceed the fill slopes noted in the plans. Install stabilization geogrid according to Subsection 207.04 a minimum of 2 feet below proposed subgrade.
Continuously monitor temporary culverts and adjust or reinstall at the beginning of each stage as directed by the CO.

Section 205. — ROCK BLASTING
Construction Requirements
205.04A Blasting Consultant. (Added Subsection.)
Provide a blasting consultant with at least 10 years experience in the field of rock blasting as a designer of blasts with controlled blasting techniques. Use a blasting consultant that is not an employee of the Contractor, an explosives manufacturer, or explosives distributor.
(a) Use a blast consultant to:
(1) Assist in the blast design.
(2) Attend the preconstruction conference.
(3) Make on-site inspections of the blast site before the submittal of each blast plan.
(4) Make on-site inspections during drilling, loading, and blasting of test sections.
(5) Make on-site inspections during drilling, loading, and blasting of production sections.
(6) Approve all blasting plans and modifications. Include with each blast plan submitted to the CO written evidence that the blasting consultant has reviewed and approved the blasting plan.
(b) Provide the following to the CO before the preconstruction conference:
(1) A resume of the proposed blasting consultant. Include educational background related to blasting and a detailed resume of experience in the blasting field.
(2) A list of at least five highway rock projects that demonstrate successfully completed work on which the blasting consultant has worked in the capacity of a consultant in blast design.
List a description of the projects, details of the blast plan, and modifications made during the project. List names and telephone numbers of project owners with sufficient knowledge of the project to verify the submitted information.
(3) A cost breakdown of the individual items included in the lump sum blasting consultant item for use in making progress payments.
Based on the documentation provided, the CO will notify the Contractor of the acceptability of the proposed blasting consultant under the terms of the contract within 14 days.

205.05 Blasting Plans. Amend as follows:
Add the following to the first paragraph:
All blasting plans and modifications to the plans must be signed by the CO. Do not begin drilling the blast pattern or deliver explosives to the project until the general blasting plan and site-specific blast plan are accepted.
Add the following to paragraph (a):
Allow 7 days for review and approval of the general blasting plan. Submit revisions and updates to the CO within 48 hours of any change in the general blasting plan.
Add the following to paragraph (b):
(11) Any rockfall containment designs and procedures; and
(12) Include an attenuation study for affected structures and indicate that the peak particle velocity versus peak frequency will not damage each structure when structures are within an area of ground vibration and/or air-blast overpressure concern.
205.06 Pre-blast Condition Survey and Blast Monitoring and Control. Amend as follows:
Delete the text of paragraph (c) and substitute the following:
(c) Control ground vibrations and air-blast over-pressures with properly designed delay sequences and maximum allowable charge weights per delay. Verify allowable charge weights per delay by conducting representative trial blasts and measuring ground vibrations and air-blast over-pressure levels. The attenuation study will enable successful prediction of the peak particle velocity in any component (longitudinal, transverse, or vertical) anywhere on the surface of the structure(s). Conduct test blasts with blast plan modifications that limit ground vibrations and air-blast over-pressures to levels that will not cause damage to nearby buildings, structures, utilities and/or natural features as determined by the Vibration Specialist. Submit a copy of the attenuation study results and predicted peak particle velocity versus peak frequency for each structure.
Add the following:
Submit a copy of the interpreted seismograph and air-blast records to the CO with a certifying signature by the Vibration Specialist. If actual peak particle velocity versus peak frequency measurements exceed those predicted for a structure, the site-specific blast plan will need to be

adjusted for the actual structural response to the blasting. The plan may include adjustments to pattern, loading, timing, flyrock measures, etc.
Record each blast using digital video equipment from two locations that clearly show the entire proposed blast site, preferably from locations on both sides of the blast.
205.09 Reporting: Add the following to (a):
(9) Record each blast using digital video equipment from two locations that clearly show the entire proposed blast site, preferably from locations on both sides of the blast.
