About this file
This is an Asbestos and Lead Inspection Report prepared for the EHRM Infrastructure Upgrade project at the Hershel "Woody" Williams VA Medical Center in Huntington, West Virginia. The inspection was conducted by Southern Environmental Management and Specialties (SEMS), Inc. on February 27-28, 2024, at the request of Spees Design Build. The report documents the results of bulk sampling and analysis of 114 asbestos samples and 206 lead-based paint readings collected from various buildings and trailers across the medical center facility.
The asbestos analysis, performed using Polarized Light Microscopy (PLM) in accordance with EPA guidelines and Method 600/R-93/116, identified asbestos-containing material (ACM) in only one location: 12X12 Tan Floor Tile with Black Mastic in Building 1, Floor DOG Tower 3, Room GD 114E, which tested positive at 3% Chrysolite asbestos. This non-friable material requires removal by a licensed abatement contractor before any renovation or demolition activities. The lead-based paint inspection using X-Ray Fluorescence analysis identified 11 samples with lead concentrations above 1.0 mg/cm² from various locations including Building 8, Building 5, and Building 1. All lead-painted surfaces were found to be in good, intact condition, so immediate removal is not required; however, SEMS recommends periodic monitoring and adherence to EPA, HUD, and OSHA work practices if disturbance occurs. The report notes that materials sampled with asbestos results of 1% or less are not considered ACM.

ASBESTOS AND LEAD INSPECTION REPORT
1.0 INTRODUCTION
Southern Environmental Management and Specialties (SEMS) was retained by Spees Design Build, to conduct an asbestos and lead inspections in specified locations throughout the VA Medical
Center located at 1540 Spring Valley Drive in Huntington, West Virginia.
SEMS West Virginia Certified Asbestos Inspector, Rodney Lewis and technician, West
Anderson, conducted the inspections and sampling from February 27 to February 28, 2024, and provides the following report based on the observations made and sampling conducted.
2.0 PROCEDURES
2.1 Asbestos Inspection
Bulk samples were taken from suspected asbestos-containing materials in areas possibly effected by the VAWV EHRM Infrastructure Upgrade project and sent to CA Labs for analysis. Asbestos can only be positively identified using microscopical techniques. The samples collected in this survey were analyzed using Polarized Light Microscopy (PLM).
2.2 Lead-Based Paint Inspection
SEMS, Inc. also conducted a limited lead-based paint inspection in the specified areas of the VA
West Virginia Medical Center buildings using an RMD LPA-1 model X-Ray Fluorescence analyzer.
3.0 SAMPLE ANALYSIS
During the inspection, fourteen (14) homogeneous materials were identified in which one hundred and fourteen (114) samples, with layers, were taken.
Homogeneous materials are defined as a material uniform in color, texture, date of application use or system and appears identical in every other respect. In respect to sampling results, if one sample from a homogeneous material test positive, then all that material is considered positive, regardless of the other sample results.
The analysis procedure followed for asbestos determination was conducted following EPA guidelines and Method 600/R-93/116. Based on these guidelines, suspect materials are not considered asbestos-containing materials (ACM) if the results of the samples collected are
VA Medical Center, Huntington, West Virginia – Asbestos & Lead Inspection Page 2 determined to have asbestos in amounts of 1% or less. Those materials analyzed and determined to contain greater than 1% are considered ACM.

From the results above, the 12X12 Tan Floor Tile with Black Mastic in Building 1 Floor DOG
Tower 3 GD 114E sampled contains asbestos. Therefore, SEMS makes the following recommendations:
• Floor Tile Mastic: The floor tile mastic is a Category I non-friable Asbestos-Containing
Material. This material will have to be removed by a licensed abatement contractor before any renovation or demolition activities occur.
Located in Appendix B are copies of the asbestos laboratory analytical results.
4.2 Lead Results
Two hundred and twenty-seven (227) samples were taken from painted materials in the proposed remodeling areas, twenty-one (21) of which were calibrations. Therefore, two hundred and six lead readings were collected. Eleven (11) samples resulted in concentrations above 1.0 mg/cm².
An overview of the samples that returned a positive reading are presented in the table below.

All positive lead-painted surfaces are considered in good, intact condition so no removal or hazardous control measures are currently needed.
SEMS recommends that all components containing lead-based paint be periodically monitored for changes in condition which could cause the paint to present a health hazard.
Should the painted substrate or paint itself be removed, SEMS recommends a licensed lead abatement contractor perform the lead removal activities.
Any work activities which disturb the lead paint should be performed following EPA, HUD, and OSHA work practices.
5.0 STANDARD OF CARE
Services performed by SEMS are conducted in a manner consistent with state-of-the-industry practices, recognizing that even the most comprehensive sampling may not detect all the areas exceeding the evaluation criteria in the structure/building. Therefore, outside the scope of work for this investigation, SEMS cannot act as an insurer or certify that the remainder of the structure is free of additional hazardous materials. No expressed or implied representation or warranty is included, except that the services were performed within the limit of the scope of work authorized by the client and the encountered site conditions.

Materials Characterization - Bulk Asbestos Analysis Laboratory Analysis Report - Polarized Light
SEMS, Inc Ioannis Petikas 11628 S Choctaw Drive Baton Rouge, LA 70815
VA Huntington, WV
CBR24031325
Summary of polarizing light microscopy (PLM / Stereomicroscopy bulk asbestos analysis) using the methods described in 40CFR Part 763 Appendix E to Subpart E (Interim and EPA 600 / R-93 / 116 (Improved). The sample is first viewed with the aid of stereomicroscopy. Numerous liquid slide preparations are created for analysis under the polarized microscope where identifications and quantifications are preformed. Calibrated liquid refractive oils are used as liquid mouting medium. These oils are used for identification (dispersion staining). A calibrated visual estimation is reported, should any asbestiform mineral be present. Other techniques such as acid washing are used in conjugation with refractive oils for detection of smaller quantities of asbestos. All asbestos percentages are based on calibrated visual estimation traceable to NIST standards for regulated of asbestos. Traceability to measurement and calibration is achieved by using known amounts and types of asbestos from standards where analyst and laboratory accuracy are measured. As little as 0.001% asbestos can be detected in favorable samples, while detection in unfavorable samples may approach the detection limit of 0.50% (well above the laboratory definition of trace).
Vermiculite containing samples may have trace amounts of actinolite-tremolite, where not found be PLM should be analyzed using TEM methods and / or water separation techniques. Suspected actinolite/vermiculite presence will be indicated through the sample comment section of this report.
Fibrous talc containing samples may even contain a related asbestos fiber known as anthophyllite. Under certain conditions the same fiber may actually contain both talc and anthophyllite (a phenomenon called intergrowth). Again, TEM detection methods are recommended. CA Labs PLM report comments will denote suspected amounts of asbestiform anthophyllite with talc, where further analysis is recommended.
Some samples (floor tiles, surfacings, etc.) may contain fibers too small to be detectable by PLM analysis and should be analyzed by TEM bulk protocols.
A "trace asbestos" will be reported if the analyst observes far less than 1% asbestos. CA Labs defines "trace asbestos" as a few fibers detected by the analyst in several preparations and will indicate as such under these circumstances.
Quantification of <1% will actually be reported as <=1% (allowable variance close to 1% is high). Such results are ideal for point counting, and the technique is mandatory for friable samples (NESHAP, Nov. 1990 and clarification letter 8 May 1991) under 1% percent asbestos and the "trace asbestos". In order to make all initial PLM reports issued from CA Labs NESHAP compliant, all <1% asbestos results (except floor tiles) will be point counted at no additional charge.
CA Labs is accredited by the National Voluntary Accreditation Program (NVLAP) for selected test methods for airborne fiber analysis (TEM), and for bulk asbestos fiber analysis (PLM). All analysts have a college degree in a natural science (geology, biology, or environmental science) or are recognized by a state professional board in one these disciplines .Extensive in-house training programs are used to augment education background of the analyst. The group leader of polarized light has received supplemental McCrone Research training for asbestos identification. This report is not covered by the scope of AIHA accreditation. Analysis performed at CA Labs, LLC 12232 Industriplex, Suite 32 Baton Rouge, LA 70809.
