About this file

This document comprises Task Assignment Change Orders (TACO #1 and TACO #2) issued by the State of Florida Department of Environmental Protection (FDEP) to APTIM Environmental & Infrastructure, LLC for environmental site assessment and remediation services at Care Cleaners of Jacksonville, Inc., located at 8011-1 Merrill Road in Jacksonville, Duval County, Florida. The work is performed under Master Contract HW689, with project designation ERIC_4516. The FDEP Project Manager is Gabriel Sostre (TACO #2) and Nicole Knutson (TACO #1), while the contractor's Project Manager is Eric Kramer.

The scope of work encompasses seven subtasks: Project Management (Subtask 1), Site Meeting and MAGS/Soil Sampling (Subtask 2), DPT Groundwater Field Activities (Subtask 3), Final Well Installations (Subtask 4), Site Assessment Report Preparation (Subtask 5), Additional PFAS Sampling and Reporting (Subtask 6), and WCD #3 equipment oversight (Subtask 7). TACO #1 established a total contract value of $106,876.86 with deliverables due by November 7, 2025, and completion by January 16, 2026. TACO #2, effective December 3, 2025, reduces the total contract value to $100,904.65—a decrease of $5,972.21 (5.58%)—and extends the completion date to March 6, 2026 (49 additional days). Cost adjustments in TACO #2 reflect hotel and per diem modifications, changes to subcontractor drilling costs, adjustment of groundwater samples and monitoring well installations, IDW disposal cost updates, and reductions in proposed expendables and equipment. Labor hours dedicated to the project were reduced to 509 total hours. Invoicing is conditioned upon FDEP approval of deliverables for each subtask.

Proposed Scope of Work Care Cleaners of Jacksonville, Inc.

Merrill Station Shopping Center 8011-1 Merrill Road Jacksonville, Duval County, FL 32277 Site Assessment and Reporting Task Assignment DC030A TACO #2 ERIC_4516 (Former Facility No. 169500253) Aptim Environmental & Infrastructure, LLC October 14, 2025

The following is a Task Assignment Change Order (TACO) #2 proposed for Task Assignment DC030A at the Care Cleaners located in Jacksonville, Duval County, Florida. The scope of work under TACO #2 includes reconciling Subtask 3 for invoicing and adjusts some of the final work scope, based on a review of the preliminary findings by the FDEP.

SUBTASK 1: Project Management

Project management hours are calculated based on the labor hours in the scope of work.

SUBTASK 2: Site Meeting & MAGS/Soil Sampling: No Changes

SUBTASK 3: DPT Groundwater Field Activities Subtask 3 adjusts the hotel costs to the actuals and removes one night that was not required. The Subtask adds per diem costs that were inadvertently omitted from the original task assignment.

Subtask 3 removes labor that was not incurred. The subcontracted drilling costs were reduced slightly to the actual costs. The Subtask also adds a groundwater sample that was inadvertently not incorporated into the original Task Assignment. Subtask 3 reduces the proposed expendables and equipment that was required during the drilling and sampling.

SUBTASK 4: Well Installations:

Based on a review of the initial analysis for the vapor, soil and groundwater reported to date, the FDEP approved the installation of one additional monitoring well to complete the assessment. The requested well included one shallow depth well, rather than the originally proposed four wells. The Subtask reduces the drilling costs to the actual amount. The Subtask also removes one hotel that was not required but adds per diem that was not originally included in the Subtask for the sampling event. Additionally, the IDW disposal costs have been updated to the estimated drum count which will be required for transportation and disposal. Additionally, Subtask 4 reduces the proposed expendables and equipment that was required during the drilling. Since the total well count will be reduced, the number of groundwater samples have been reduced to the actual number of wells to be sampled. Subtask 4 also changes the sampling labor to the scientist currently scheduled to perform the work, since there will be a cost savings to the FDEP.

SUBTASK 5: SAR Reporting: No Changes SUBTASK 6-PFAS Groundwater Sampling: No Changes

To allow for the sampling and reporting of the wells a deliverable extension for the Site Assessment Report (Subtasks 4,5,6 and 1) of 45 days is requested. The extension would move the deliverable date to December 22, 2025. The extension would adjust the Task Assignment Completion date to March 2, 2026.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
