SUBJECT: Negotiation Summary
HW680 SA293
DATE: January 24, 2024
Arcadis will conduct site assessment, as outlined in the Nov. 30, 2023 (updated 1/24/24) Proposal for Supplemental Site Assessment and Semi-annual Groundwater Monitoring Activities, DMS Facility- Starke letter. Activities will include GPR/utility locate and clearance, bush hogging of access pathways to monitor well locations, monitor well installation, professional survey of wells, generation of Off-site Notification materials, Semi-Annual Groundwater Monitoring event, and T&D of IDW.
Arcadis will generate brief field event summaries, and a Semi-Annual Groundwater Monitoring Report that will summarize the results of field activities and will include tables and figures, sample analytical reports, and field documentation.
Total task amount is $80,613.17. Arcadis’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. Due to issues with EnviroServe and drum removal, the second lowest IDW hauler was selected. Four drilling bids were sought, with two no bids due to equipment availability. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable is due May 3, 2024. Task completion date is July 14, 2024.

Mr. Jeff Newton Site Investigation Section Florida Department of Environmental Protection 2600 Blair Stone Road, MS 4515 Tallahassee, Florida 32399-2400
Date: November 30, 2023 Our Ref: TBD Subject: Proposal for Supplemental Site Assessment and Semi-annual
Groundwater Monitoring Activities DMS Facility - Starke 14281 US Highway 301 South Starke, Bradford County, FL
ERIC_6157 (56SL)
Dear Mr. Newton, Arcadis U.S., Inc. (Arcadis) is pleased to submit the following proposal/cost estimate to complete the following activities at the DMS Facility in Starke, FL (Site): off-site notification of impacted properties, monitor well installation, Site mowing, semi-annual groundwater monitoring, and preparation of a Semi-Annual Monitoring Report.
This proposal includes the scope of work pursuant to recent communications with the Florida Department of Environmental Protection (FDEP) and estimated costs with a schedule for completion. The following tasks were developed in accordance with Chapter 62-780 F.A.C. and the applicable standard operating procedures (SOP)
(DEP-SOP-001/01).
SCOPE OF WORK
Task 1: Project Management
Project management includes technical support and general administrative and health and safety tasks related to the items below.
Task 2: Off-Site Notification Package
Based on the analytical results from the supplemental site assessment and groundwater sampling events conducted between April 2023 and May 2023, an additional eight Off-Site properties not included within the current Temporary Point of Compliance (TPOC) boundaries have been identified. This task includes the effort to provide FDEP a plume map showing dashed lines around the monitor wells (MW-106 & MW-109 cluster) and the creation of a table showing all the previously notified properties within the historical TPOC extents, the newly impacted properties, and adjacent properties that can be inferred to be impacted. This information will be used in the future for the preparation of off-site notification update packages per Rule 62-780.220 F.A.C. A proposal for costs associated with the actual TPOC notification update package will be provided under separate cover when requested by FDEP.

Task 3: Monitor Well Installation
Based on the data collected during the May 2023 semi-annual groundwater monitoring event, Arcadis has identified the need for installation of additional monitor wells to further define the Constituent of Concern (COC) impacts associated with the Site. Three additional monitor wells, as depicted on the attached figure (Figure 1, Attachment 1), are recommended to be installed prior to the performance of the next semiannual monitoring event to delineate plume extents and to determine the distribution of COC mass more accurately within the constituent plumes. Specifically, the following monitor wells are proposed:
Two monitor wells (MW-115 (35-45) and MW-115 (45-55)) are proposed within the Lower Shallow Zone and Intermediate Zone downgradient of the MW-109 well cluster. Arcadis recommends installing the additional wells within the western right-of-way (ROW) of SE 44th Avenue.
One monitor well (MW-116 (40-50)) is proposed south of MW-106. Access and final location of this well will be coordinated with FDEP.
All monitor wells will be constructed of 2-inch Schedule-40 PVC, with the lower 10- feet comprised of 10-slot screen. The monitor wells will be installed with a 20/30 filter sand pack, a two-foot 30/65 choker sand seal, and grouted to surface. The monitor wells will be installed within 8-inch bolt-down manhole, in a 2-foot by 2-foot concrete pad. A summary of monitor well construction is provided in the table below:
Table 1 – Proposed Monitor Well Details
Well ID Aquifer Zone Well Diameter (inches)
Screened Interval (ft-bls)
MW-115 (35-45) Shallow Upper Aquifer Zone 2.00 35 to 45
MW-115 (45-55) Intermediate Aquifer Zone 2.00 45 to 55
MW-116 (40-50) Intermediate Aquifer Zone 2.00 40 to 50
Monitor well construction details proposed are based on available data, final construction details of the monitor wells will be determined based on soil lithology data (collected from the deepest boring at each proposed cluster) and field observations. Arcadis solicited four bids from qualified and licensed drillers to perform the work. Two of the drillers were unable to provide a bid, as they did not have a sonic drill rig. Based on cost and availability, Preferred Drilling Solutions (PDS) was selected as the winning bidder. Monitor wells will be developed for approximately 30-60 minutes, until the purged water is relatively clear and sediment-free. It is estimated that four drums of non-hazardous Investigation Derived Waste (IDW) groundwater and ten drums of non-hazardous IDW soils will be generated during the well installation/ development event. Arcadis solicited bids from three licensed waste haulers for the off-site transportation and disposal of IDW. Although EnviroServe was the low bid, recent service issues resulted in selecting Cliff Berry, Inc. as the winning bidder.
Prior to beginning intrusive work for the installation of the monitor wells, in addition to initiating utility marking via Sunshine 811, a Ground Penetrating Radar (GPR) survey will be performed to minimize the potential for utility strikes within the right-of-way and on the private property location. Arcadis requested bids from three GPR companies and selected Blood Hound as the winning bidder.
Following installation and development, the three new monitor wells will be surveyed by a surveyor licensed in the state of Florida. The surveyor will collect latitudinal and longitudinal coordinates and the top of casing (TOC)

3/5 elevation from each monitor well. Bids were solicited from three qualified professional land surveyors, with Landmark Engineering & Surveying Corporation selected as the winning bid.
As the site is no longer occupied, routine maintenance of the Site is no longer performed. Prior to monitor well installation activities, Arcadis will perform the bush hogging of access pathways to monitor wells on the Site property in preparation for monitor will installation and groundwater monitoring activities. Bids were solicited from three equipment rental companies, with Lepp Rents selected as the winning bid.
Arcadis will provide FDEP with a brief field work summary memorandum (with field documentation and photographs) approximately 10 days following the completion of the monitor well installation event. Bids from the various subcontractors are included as Attachment 2.
Task 4: Semi-Annual Groundwater Monitoring Event
As performed during the May 2023 monitoring event, depth-to-water (DTW) levels will be collected from all accessible monitor wells (approximately 140 locations) utilizing a water level probe relative to each top of well casing. The data will be used to determine relative groundwater elevations and gradient conditions within each of the respective aquifer zones.
Following the completion of the groundwater elevation gauging, 86 (81 existing MWs sampled in May 2023 + 2 existing MWs recommended in September 2023 GMR +3 proposed MWs) Site monitor wells will be sampled during the event. The 83 existing monitor wells will be sampled using the low-flow sampling methodology (equipment volume purge) to improve efficiency and minimize the volume of IDW generated. As discussed with FDEP, the conventional well volume purge technique will be utilized to sample the three new/ proposed monitor wells. At these three locations a minimum of one well volume will be purged and then parameter readings will be collected at ¼-well volume intervals until stabilization has been achieved. As previously discussed with FDEP, five monitor wells (MW-107 (105-115), MW-108 (115-125), CMW-D (115-125), CMW-G (115-125)), will be sampled using bailers due to the small diameter of the wells and observed DTWs (approximately 60 ft bls).
Samples will be submitted to the FDEP Central Lab for analysis. All samples will be analyzed for CVOCs per USEPA Method 8260B, four samples will be analyzed for 1,4-dioxane by 8260 SIM-ID, and 31 samples will be analyzed for total organic carbon (TOC) by standard method SM 5310B. Monitor wells selected for 1,4-dioxane analysis were those with detections above 0.35 ug/L (Florida Department of Health Advisory Level) and monitor wells selected for TOC analysis were those that exhibited at least one CVOC compound above its’ respective FDEP GCTL during the May 2023 monitoring event. A total of six duplicate samples (4 – CVOC and 2 – TOC) will be collected for QA/QC analysis. This proposal assumes that approximately 3 drums of non-hazardous purge water will be generated during the implementation of Task 4. It is assumed that Arcadis can utilize the existing waste profile for the disposal of purge water.
Arcadis will provide FDEP with a brief field work summary memorandum (with field documentation and photographs) approximately 10 days following the completion of the sampling event.
Task 5: Reporting
A Semi-Annual Monitoring Report will be prepared, certified by a Professional Engineer or Geologist, and submitted to FDEP within 60 days of the receipt of laboratory data. This report will summarize the results of field activities completed at the Site and include tables and figures, sample analytical reports, field documentation, and recommendations for further assessment or closure options.
Use or disclosure of information contained on this sheet is subject to the restriction and disclaimer located on the signature page of this document.
