Subject: Proposal for Site Meeting, Desk Top Document Review, and Semi- Annual Groundwater Monitoring Activities Deerwood Subdivision Site 14281 US Highway 301 South Starke, Bradford County, FL

ERIC_6157 (56SL)

Dear Mr. Jensen, Arcadis U.S., Inc. (Arcadis) is pleased to submit the following proposal/cost estimate to complete site recon, including a site meeting with the former contractor (AECOM), a desktop review of site documents, and performance of a semi-annual groundwater monitoring event at the Deerwood Subdivision Site (Site). As discussed with the Florida Department of Environmental Protection (FDEP), preparation and submittal of a Semi- Annual Monitoring Report for the Site will be tasked separately.

This proposal includes the scope of work pursuant to recent communications with FDEP and estimated costs with a schedule for completion. The following tasks were developed and in accordance with Chapter 62-780 F.A.C.

and applicable standard operating procedures (SOP) (DEP-SOP-001/01).

SCOPE OF WORK

Task 1: Project Management

Project management includes technical support and general administrative and health and safety tasks related to the items below.

Task 2: Site Recon and Site Meeting

Arcadis will schedule a site meeting with the former contractor (AECOM) and FDEP. The purpose of the site visit shall be physically locating monitoring wells and obtaining a better understanding of Site and off-site conditions and potential health and safety concerns. Arcadis staff to perform the site visit shall include the project manager, engineer, and field technician.

In addition, Arcadis will conduct a conference call with AECOM to discuss Site history and current conditions.

Arcadis will request AECOM provide site files to Arcadis, including data base, CAD, survey, access agreements and any other relevant files not available through Oculus. Specifically important for the efficient development of the Semi-Annual Monitoring Report will be receipt of live file versions of excel and CAD drawings.

Task 3: Desktop Review of Site Documents

A desktop review of site documents available in Oculus and soon to be submitted final AECOM Semi-Annual Monitoring Report will be performed to understand previous site activities and current site conditions. This review will be used in conjunction with the analysis of data collected during proposed groundwater monitoring event, to determine a conceptual path to closure for the Site, what potential data gaps exist, if additional site assessment (groundwater or soil) is required and if additional remediation may be warranted. These topics will be discussed in detail within the proposed Semi-Annual Monitoring Report.

Task 4: Semi-Annual Groundwater Monitoring Event

Prior to initiation of field work, Arcadis will prepare a Health and Safety Plan (HASP) to identify task-specific hazards, hazard controls, and monitoring and safety requirements for all phases of work anticipated.

As performed during the August 2019 monitoring event, groundwater elevations will be collected from 121 monitor wells utilizing a water level probe relative to each top of well casing. These data will be used to determine relative groundwater elevations and gradient conditions in the respective aquifer zones.

Following the completion of the groundwater elevation gauging, 61 Site monitor wells will be sampled using low-flow techniques and in accordance with the current version of DEP-SOP-001/01 FS2200. Samples will be submitted to the FDEP Central Lab for analysis. All samples will be analyzed for CVOCs per USEPA Method 8260B, five samples will be analyzed for 1,4-dioxane by 8260 SIM-ID and 24 samples will be analyzed for total organic carbon (TOC) by standard method SM 5310B. Monitor wells selected for 1,4-dioxane analysis are those with detections above 0.35 ug/L and monitor wells selected for TOC analysis had at least one CVOC compound above the FDEP GCTL during the August 2019 sampling event. A total of eight duplicate samples (6 – CVOC and 2 – TOC) will be collected for QA/QC analysis. This proposal assumes that three drums of non-hazardous purge water will be generated during implementation of Task 4. It is assumed that Arcadis can utilize the existing waste profile for the disposal of purge water.

Arcadis will provide FDEP with a brief field work summary memorandum (with field documentation and photos) approximately 10 days following completion of the sampling event.

Task 5: Reporting

Reporting will be tasked separately.

SCHEDULE

Arcadis will begin coordinating schedules immediately following receipt of a signed Task Assignment. The field work summary memorandum will be submitted to FDEP within 10 days of completion of field work.
