About this file

This is a federal contracting representation and certification form (FAR 52.209-7) regarding responsibility matters that contractors must complete when submitting offers on federal contracts.

The form requires offerors to disclose whether they currently hold active federal contracts and grants with a total value exceeding $10,000,000. If an offeror indicates they have such contracts or grants, they must certify that information entered in the Federal Awardee Performance and Integrity Information System (FAPIIS) is current, accurate, and complete as of the offer submission date. Specifically, offerors must report any proceedings at the federal or state level occurring within the past five years that resulted in: (1) criminal convictions; (2) civil findings of fault and liability with monetary payments of $5,000 or more; (3) administrative findings of fault with penalties of $5,000 or more or reimbursement/damages exceeding $100,000; or (4) dispositions by consent or compromise with acknowledgment of fault that could have led to the aforementioned outcomes. Offerors are required to maintain active registration in the System for Award Management (SAM) and post all required responsibility information in FAPIIS. This template is used for DoDEA Procurement Transportation Division solicitations and is marked as Controlled Unclassified Information (CUI).
