About this file
This file is a federal contracting form (FAR 52.209-7) titled "Information Regarding Responsibility Matters," issued in October 2018. It is a template used by offerors to disclose information related to their responsibility and integrity in federal contracting.
The form requires offerors with current active federal contracts and grants totaling more than $10,000,000 to represent that their information in the Federal Awardee Performance and Integrity Information System (FAPIIS) is current, accurate, and complete. Offerors must disclose whether they or their principals have been subjects of proceedings within the past five years that resulted in: (1) criminal convictions; (2) civil findings of fault and liability with monetary payments of $5,000 or more; (3) administrative findings of fault and liability with monetary fines or penalties of $5,000 or more, or reimbursement/restitution/damages exceeding $100,000; or (4) dispositions by consent or compromise with acknowledgment of fault. Offerors must also confirm whether they have provided requested information for each occurrence. The form mandates that offerors maintain active registration in the System for Award Management (SAM) at https://www.sam.gov to post required responsibility information in FAPIIS.
