The plan shall indicate all categories of personnel employed and shall delineate lines of communication among them with contact information.

3.1.5 The Contractor shall implement warranties on new equipment, whether installed under this contract or by others under separate contracts and maintain records of same.

3.1.6 If the Contractor is unable to complete maintenance or repair tasks within the timeframe required by this contract (including SOPs) or stated in an accepted service order, the Contractor shall be responsible for providing and installing/connecting temporary systems to ensure the facility can continue to operate without interruption during the extended maintenance or repair time. All temporary equipment and/or items shall be approved by the Facility Manager prior to installation or connection to existing building systems, and the period of time the temporary measures will be in place shall be submitted in writing for approval by the Facility Manager.

3.1.7 NAMRU-6 operates automated systems which may be used to generate requirements for this Contractor, or which may require information from the Contractor. These systems include a customer service order request system in use at the Lima and Iquitos sites, and the Defense Medical Logistics Standard Support (DMLSS) system.

The Contractor is not responsible for using or providing direct data entry into either system, but the Contractor may be required to format reports and deliverables to ensure the NAMRU-6 staff can effectively update and operate the systems.

3.2 Preventive Maintenance (PM), also referred to as “Recurring Maintenance” or “Scheduled Maintenance.”

3.2.1 Tasks addressed in a PM will be completed under the PM, e.g. replace filters, tighten/replace belts, make minor adjustments, etc. Minor related repairs (requiring less than 2 hours of labor and $500 in materials) shall also be completed under the PM. For more significant repairs identified during PMs, the Contractor shall notify the Facility Manager and obtain concurrence with generation of a Service Order in the work management system. If Service Orders are required for deficiencies discovered during the PM, the Service Order number will be listed on the PM record, and the PM number will be listed on the Service Order record. The relationship between the two will be available by reviewing either record. The PM will not be closed or considered complete until all PM tasks are complete. Actions taken to correct deficiencies under the PM must be clearly identified in the PM record prior to completing the PM. No PM tasks or frequencies can be modified without prior approval of the COR and Facility Manager.

3.2.2 If during the PM, the Contractor identifies that the equipment or any component is near or at the end of its economic life and should be scheduled for replacement, the Contractor shall include that in the report to the COR and Facility Manager to be submitted upon completion of the PM. Identified equipment replacement will typically be performed under a separate construction/repair contract but could be performed under a separately negotiated modification to this contract.

3.2.3 The Contractor shall assign appropriately skilled and knowledgeable craftsman to perform the work.

3.2.4 If the SOP PM tasks are incorrect for the equipment, the Contractor shall notify the COR and Facility Manager to request a change to the SOP, and upon concurrence shall perform the correct tasks IAW manufacturer requirements.

3.2.5 The Contractor shall develop an annual PM schedule and provide it to the COR and Facility Manager for approval at the start of the Contract. Changes to the annual Schedule will be coordinated with the COR and Facility Manager. The Contractor shall ensure that Service on equipment is scheduled and performed during appropriate times of the year. For example, chillers will be serviced prior to the cooling season. Where schedules indicate yearly, annually, or similar terms to indicate performance at yearly intervals, the Contractor shall perform the service within the duration of each annual performance period, and the plan shall indicate the time at which the service is planned. Where schedules indicate performance at greater than yearly intervals (5 years for instance), the Contractor shall perform the service within the duration of this contract, and the plan shall indicate the time at which the service is planned. The SOPs provided in this contract shall be used as the basis for the required maintenance schedules.

3.2.6 The Contractor shall provide all tools, equipment, supplies, materials and parts to complete the service.

The Contractor shall ensure the work area is clean and orderly after completion of the service and dispose of all unusable materials and parts properly.

3.2.7 If the equipment is not operational the Contractor shall notify the Facility Manager. The PM will not be closed out if the equipment is not operational without approval from the Facility Manager.

3.2.8 All equipment data, such as gage readings, shall be recorded during each PM. These data points as well as craftsman comments or observations shall be recorded on the PM record and in the report submitted following the completion of the PM.

3.2.9 Upon completion of each PM, test, and/or inspection, the Contractor shall submit to the COR and the Facility Manager, a report describing the work performed. The report shall include all PM reporting requirements identified in this contract (equipment data collected, etc.), and shall also include: the date(s) and time(s) the work was performed; the name of the personnel who completed the work (to include company name if performed by a subcontractor); the total number of hours spent in performance of the work; the total cost of materials used; the total subcontract cost incurred (if applicable); a narrative description of any minor repairs completed incidental to the PM with a summary of the time and materials expended specific to the repair as addressed above (separate from that required for the PM itself); a description of any major issues noted with the equipment, to include recommendations for major repair or scheduled replacement of the equipment or components; data required for entry of the PM record by NAMRU-6 personnel in the government system of record; and any other information pertinent to the work.

3.2.9.1 The Contractor shall submit a monthly report to the COR and Facility Manager, with a summary of all scheduled work performed during the month. The report shall provide for each PM, the information identified in paragraph 3.2.9 above. The Contractor shall coordinate with the COR and Facility Manager to determine an acceptable format for this report, which may include submission of or reference to copies of PM records for information captured in those records.

3.2.10 All PMs shall be completed in the month in which they are scheduled. The date scheduled shall comply with all regulatory requirements. Provide a copy of the schedule to the COR and Facility Manager no later than five working days before the end of the month prior to the month in which the PMs are to be performed. If a PM, test or inspection cannot be performed on the date scheduled, the COR and Facility Manager must be notified prior to the originally scheduled date. If access to the equipment must be restricted by NAMRU-6 for any reason, the Facility Manager will coordinate with the Contractor to reschedule the PM.

3.2.11 PM Requirements at Lima and Iquitos: NAMRU-6 maintains an in-house staff at the Lima and Iquitos sites, which will remain responsible for all daily and weekly, and some monthly PM tasks. The Contractor shall be responsible for all PM tasks with a frequency of quarterly or greater (semi-annual, annual, 5 year, etc.) and some monthly PM tasks as described in the SOPs. Required PMs are defined in the SOPs in the Technical Exhibits of this contract. The Contractor shall also be responsible for all required tests and inspections for facility equipment and systems, regardless of the required frequency. Processes and requirements for PMs included throughout this and other sections of the contract shall apply to PM requirements at Lima and Iquitos as applicable.

3.2.11.1 The Contractor shall coordinate with the Facility Manager on a monthly PM schedule to include all PMs, tests, and/or inspections required to occur within that month. The PM schedule shall be finalized no later than five working days before the end of the month prior to the month in which the PMs are to be performed. If a PM, test or inspection cannot be performed on the date scheduled, the COR and Facility Manager must be notified prior to the originally scheduled date. A request for maintenance or repair is incidental to the performance of operations and maintenance and included within the operations and maintenance contract requirements.

3.4.1 Schedule B identifies a Not to Exceed amount for unscheduled maintenance support for which the Contractor shall be responsible. Work against the unscheduled maintenance contract line item will be invoiced monthly based on actual effort completed on assigned service orders, to include labor, parts, and materials costs specific to each approved service order. The Contractor shall notify the COR immediately if/when the charges against any Not to Exceed line item reach 50% of the value of that line item, to provide adequate time for the Government to plan for future efforts. 3.4.2.2 Priority 2: Work of an urgent nature which must be initiated no later than the next regularly scheduled work shift. This priority is assigned to those pieces of equipment or systems for which there are temporarily sufficient alternatives. Priority 2 also includes work to correct an unsafe condition which is not an immediate hazard to personnel. Priority 2 service orders should be accomplished within two working days.

3.4.2.3 Priority 3: Any work to improve facilities building operation which can be planned for completion within ten working days.

3.4.2.4 Priority 4: Any work that is a valid requirement for proper operation of the facility, site, or specific systems, but that does not fall within the above categories. This will be completed within 25 calendar days. Major repairs and the construction of new systems, components, facilities and buildings is not considered unscheduled or demand maintenance and will not be performed as a service order but would require a modification to this contract.

3.4.3 Service orders assigned under the unscheduled maintenance line item will include “routine” maintenance, repair, and in some cases incidental construction of basic infrastructure and systems.

3.4.4 Service orders for specialty system repairs require a specialty technician or subcontractor to perform the work. Systems include, but are not limited to, systems such as fire alarm, access control and CCTV system, building automation system, fire protection system, chiller and boilers. Service orders generated in the system may be assigned to the Contractor upon approval and prioritization by the Government within the limits described above. Upon receipt of a service order, the Contractor shall submit a level of effort estimate and obtain the Facility Manager’s approval prior to scheduling and performing the work within the timeframes required above (starting from the date/time of Government assignment of the service order).
