STANDARD OPERATING PROCEDURE (SOP) – QUARTERLY PREVENTIVE

MAINTENANCE ACTIVITIES FOR ABOVEGROUND DIESEL TANKS &

ASSOCIATED EQUIPMENT IN NAMRU-6 LIMA, IQUITOS AND PUERTO

MALDONADO SITES

General Requirements for Lima, Iquitos and Puerto Maldonado

NAMRU-6 has the following aboveground diesel tanks and associate equipment in Lima, Iquitos and Puerto Maldonado sites:

- Lima site:

• One 8,000 gallons double wall steel aboveground diesel tank, built under UL-142 and UL 2085 standards, manufactured in USA.

• One 100 gallons day tank for HVAC heater

• One 100 gallons day tank for Boiler

• Two 1,000 gallons day tanks for Generators

• Delivery and transfer pumps for the filling point

• Two submersible pumps and fuel pipes to distribute fuel from 8,000 gallons diesel tank to day tanks

• Electrical and control panels for leak detection, TS-550 console

• Cathodic protection and associated electrical panel

• Electric and control panels for operation of pumps

• Slab and concrete foundations where the tank is placed and perimeter metallic fencing to delimit the area

• Fuel spillage collection concrete vault with valve

• Emergency portable tank and manual pump and hose for spillage collection

• Solenoid valves in each day tank and level sensors

- Iquitos site:

• One 1,000 gallons day tank for Generator

- Puerto Maldonado site:

• One 100 gallons day tanks for Generator

• Diesel pipes from 100 gallons day tank to Generator

Contractor shall provide the names and certification of the technicians performing preventive maintenance tasks. Only qualified technicians shall perform preventive and corrective maintenance tasks. The diesel fuel type in the tanks, categorized in Peru, is DB-5.

Contractor shall perform quarterly and annual preventive maintenance activities to all of the tanks & associated equipment specified in this document. It is the responsibility of the contractor to perform a full inspection of the systems and collect all required information to provide a quality preventive maintenance. In addition, contractor shall review operation manuals for the

Section J - Exhibit 11.2.2 SOP-FAC 002 equipment of the systems under this contract and include any recommended preventive maintenance activities by the manufacturer not mentioned in this document.

Contractor shall elaborate, review and update the drawings in AUTOCAD of the diesel tank systems in Lima, Iquitos and Puerto Maldonado sites on an annual basis.

The tanks & associated equipment in Iquitos and Puerto Maldonado sites are only day tanks and pipes to store diesel for generators. The diesel system in Lima site is complex and consist of a main 8,000 aboveground diesel tank and submersible pumps mounted in the tank that fill the four day tanks (one for HVAC heater, one for Boiler and two for generators). These pumps work alternately according to the requirement of the day tanks and while one is working, the other is on standby mode. Each of the day tanks have a level measurement system, which determine the max and min filling levels in the day tanks. Solenoid valves in each tank run the submersible pumps to fill the day tanks. The pumps can also operate from the control panel (HMI touch operator screen) installed in the proximity of the main aboveground diesel tank. In addition, a console for the leak detection system and solenoid valves are installed in the system to advise when there is a fuel leak in the diesel system. A chatodic protection system protects only a portion of new diesel steel pipes recently installed from the main tank to a concrete box near the electrical and control panels. The filling of the day tanks is done in automatic mode.

More information of the operation of the automatic filling system can be found in the O&M manuals.

Contractor shall include all materials, tools and personal protective equipment necessary to perform preventive and corrective maintenance work. Contractor must use the necessary and appropriate equipment to delimit the work area and not allow the pedestrian traffic. The contractor’s staff shall use the appropriate PPE for the activity to perform. NAMRU-6 will not provide any materials, tools nor PPE to the contractor to perform their work.

The contractor shall provide a weekly progress report to the NAMRU-6 facilities department, as well as a completion report when completing the maintenance work to include: activities performed with pictures, recommendations, conclusion and future needed corrective maintenance tasks. When a corrective maintenance work is required, the contractor shall submit a report detailing the problem encountered, with a quotation of the solution.

The preventive maintenance work will be coordinated between NAMRU-6 facilities staff and the staff assigned by the contractor.

Contractor shall perform operational testing of the systems at the completion of each preventive maintenance work. NAMRU-6 facilities department will assign a staff to participate in the operational testing and contractor shall prepare an operational testing form (checklist) to be signed by the facilities staff once testing is perform and accepted.

It is the responsibility of the contractor to dispose properly out of NAMRU-6 facilities all consumable materials and garbage generated by the contractor during the performance of the maintenance activities.

Contractor shall perform quarterly fuel tests for verifying the quality of diesel fuel DB5.

Contractor shall take special attention to detect water on fuel. Contractor to provide a written report of the test and certification provided by the company performing the test. In attachment one at the end of this document, find an example of the minimum parameters to include in the test (viscosity, sulfur, particle count, API gravity-density-relative humidity, V40, Biodiesel, PQI, etc.).
