It is the responsibility of the Offeror to submit complete proposal information and documents that are fully responsive to the submission requirements of the solicitation. Offerors are responsible for organizing submitted material logically as it relates to the requirements of this solicitation and to number all pages to facilitate evaluation. Materials not organized appropriately and in the sequence of subparagraphs set out below, may easily be overlooked during evaluations, possibly affecting an Offeror’s rating.
2.0 Volume I - Past Performance of the Prime Contractor, Technical Approach, and Management. Submit Past Performance Information of the Prime Contractor, Technical Approach, and Management in Volume I, under the tabs indicated below and number all pages.
Note: Do NOT provide any cost information in Volume I.
2.1 VOLUME I TAB-A: Past Performance of the Prime Contractor. The contractor noted on the SF 33 is considered the Prime Contractor and Offeror. The Offeror is to provide its past performance information regarding recent and relevant efforts and the quality of those efforts to demonstrate its accomplishments and capabilities to successfully perform and accomplish the effort required by this RFP. Past performance information includes the following: (a) Relevant Company Effort and (b) Quality of Past Performance.
Where the word "project” and/or “effort(s)" appears in RFP Sections L and M, it is understood to mean operation and maintenance (O&M) type work under a single contract. Examples of project effort submitted by Offeror may be projects performed for a State or Federal Government agency or for a commercial or private sector customer. If the Offeror is submitting projects or efforts performed under a “task order type contract”, each task order shall be considered a separate project. In other words, do not combine tasks order projects and present them as one project.
Where the word “recent” appears, it is understood to mean an O&M type project for which (1) the contract has a minimum period of one year and is 100% complete within 10 years of the RFP issue date; or (2) the contract is ongoing and at least one year of the contract is 100% complete by the RFP issue date.
“Relevant” past performance information is one indicator of an Offeror’s ability to perform the contract successfully. Where the word “relevant” appears, it is understood to mean a single project that is similar and logically connected to the context of this solicitation in terms of scope (dollar value, size, project type, contract type), magnitude of effort and complexity as the work described in this solicitation. For example, projects that include performance on laboratories, hospitals or similar institutions will be given more consideration. The description of work is found in RFP Section C.
2.1.1 (a) RELEVANT COMPANY EFFORT. The Offeror is to submit no more than five (5) examples of its most “recent” and “relevant” project efforts performed as a prime contractor to demonstrate its company’s accomplishments on contracts similar to the effort required in this solicitation. If the Offeror submits more than five projects, only the first five projects shall be evaluated. For each “recent” and “relevant” project example, the Offeror shall submit a separate and fully completed Prime Contractor Project Data Sheet (Attachment 1 at the end of this section), or at a minimum, provide the following information
(1) Company Name;
(2) Name of Project;
(3) Location of Project;
(4) Owner (i.e. customer work was completed for);
(5) Cost for this single contract;
(6) Narrative Description of the General Scope of Project/Contract as it relates to this solicitation. Include a copy of the information from the contract (such as the scope of work summary and/or table of contents) in order to clearly demonstrate the type of work included in the contract. This scope/summary of work shall not exceed three pages in total:
(7) Your Role on this Contract (Prime, Subcontractor, Joint Venture Partner) and the type of effort your company self-performed;
(8) Contract Type: (for example Firm Fixed-Price, Cost Plus Fixed Fee, Cost Plus Award Fee);
(9) Extent and Type of Work Subcontracted Out;
(10) Contract Start Date (Provide Month and Year); Contract Completion Date (Provide Month/Year); or One Year Completion Date (Provide Month/Year);
(11) Termination or Liquidated Damages Information; Comments regarding any known performance deemed unacceptable to the customer or not in accordance with the contract terms and conditions; and
(12) Name and contact information of Owner’s point of contact.
(b) QUALITY OF PAST PERFORMANCE.
1) The Offeror shall submit evidence of the quality of its past performance on projects submitted in paragraph 2.1.1 (a). The Offeror shall provide a completed contractor performance evaluation, whether annual or final, found in the Contractor Performance Assessment Reporting System (CPARS) (www.cpars.gov) [formerly Past Performance Information Retrieval System (PPIRS) (www.ppirs.gov)]. If a completed contractor performance evaluation does not exist in CPARS (i.e., the past performance project is a non- U.S. Government project), the Offeror shall provide a completed Past Performance Questionnaire (PPQ) (Attachment 2 at the end of this section). If at time of evaluation, a completed contractor performance evaluation exists in CPARS that conflicts with a submitted PPQ, the completed contractor performance evaluation in CPARS will govern.
2) Any past performance surveys/assessment/evaluation forms, other than CPARS or the PPQ Form provided at the end of this section, are unacceptable submissions by the Offeror to evidence the quality of its past performance and will not be considered or evaluated.
3) Completed CPARS or PPQs that were submitted in response to previous solicitations may be submitted. However, Offerors shall not incorporate either by reference, and thus must resubmit the CPARS or PPQ with their proposal. However, this requirement does not preclude the Government, at its discretion, from utilizing previously submitted CPARS or PPQ information in the past performance evaluation. Notwithstanding the foregoing, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
4) If an Offeror has been required to take corrective action for any substandard performance, experienced other performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc., the Offeror should explain the circumstances and the corrective actions taken.
5) If a client will only provide completed PPQ directly to the U.S. Army Corps of Engineers, Mobile District, then ensure the client submits the PPQ before the proposal due date using the following electronic mailing address:
sophia.m.chin@usace.army.mil
Please include the solicitation number in the subject line of the email.
PPQs and/or CPARS received after the proposal receipt date will not be considered timely and will not be included in the Offeror’s proposal.
2.1.2 Joint Venture Submission Requirements. Offerors submitting proposals as a Joint Venture (JV) shall obtain a DUNS number and be registered in the System for Award Management
(SAM) in the name of the Joint Venture. The Offeror, that is the JV, must provide no more than five (5) “recent”, “relevant” projects performed by the JV that reflect the JV’s ability to perform the work required in this solicitation as required by Paragraph 2.1.1 subparagraphs (a) and (b).
2.1.2.1 If the Offeror, that is the JV, cannot provide a sufficient number of projects as the JV to meet the requirement, the Offeror must provide “recent”, “relevant” projects that it has performed as the JV, and then supplement the record with additional “recent”, “relevant” projects performed by any JV member in their individual capacities. In this instance, the total number of projects submitted shall be no more than five (5), regardless of the combination of JV projects and JV member projects.
2.1.2.2 If the Offeror, that is the JV, has no “recent”, “relevant” projects performed by the JV, then members of the JV must provide a combined total of no more than five (5) “recent”, “relevant” projects performed by either member of the JV that reflects the JV member’s ability to perform the work required in this solicitation as required by Paragraph 2.1.1 subparagraphs (a) and (b).
2.1.3 Verification: The Government reserves the right to verify the past performance record of cited projects or other recent projects not submitted by reviewing CPARS, or other Government appraisal systems, or to interview owner or references. The Government may check any or all cited references to verify supplied information. An Offeror will be afforded the opportunity to reply to or explain adverse performance information which the Offeror has not previously had an opportunity to address.
2.1.4 All Business Entities: To satisfy Paragraph 2.1.1 subparagraphs (a) and (b), only information pertaining to the corporate entity listed on the SF 33 will be considered in the evaluation. The individual experience of key personnel will not be considered. Information documented in the Federal Awardee Performance and Integrity Information System (FAPIIS) and CPARS related to a parent company, affiliates, divisions, subsidiaries, predecessor companies, and other related entities will be considered for responsibility in accordance with FAR 9.104-6 but will not be considered in the evaluation.
2.2 VOLUME I TAB-B: Technical Approach. Do not include cost information in the technical approach narratives. In a narrative form, as a minimum, submit your company’s technical approach to execute operation and maintenance and related repair and minor construction work. Technical approach narratives may be enhanced with charts, tables, or any logical means of conveying the proposed technical approach the Offeror will use to accomplish the work. Broad general statements that merely mimic the scope of work, repeat, paraphrase, or promise general performance are not sufficient.
The maximum page limitation to include charts, tables, etc. is limited to 25 pages for Volume I Tab-B Technical Approach. Any information past this number will not be evaluated.
For this source selection, a technical approach for each of the following is to be discussed:
2.2.1 Provide a description of the Offeror’s capability to perform the operation and maintenance work required by this solicitation for a laboratory research facility. The discussion shall include but not be limited to the use of company resources in terms of labor, equipment, home office support, and software systems or processes.
2.2.2 Provide a description of the Offeror’s capability to perform the same or similar laboratory research facility operation and maintenance at multiple sites concurrently during the period of performance. The Offeror shall clearly demonstrate its capability to perform multiple operation and maintenance, repair, and construction work simultaneously in various geographic locations and shall address the Offeror’s personnel, processes, and resources that form the basis for the stated capability.
2.2.3 Provide a description of the Offeror’s plan to take over the maintenance of a laboratory research facility for the first time, addressing topics including but not limited to assessing requirements, developing a staffing plan, hiring a workforce, assuming control of existing inventory, the plan for learning existing systems and equipment, development/ implementation of an operation and maintenance management plan, etc.
2.2.4 Provide a description of the Offeror’s plan to manage an on-going, full service operation and maintenance program for a laboratory research facility. The narrative shall include, but not be limited to, discussions of the following.
(a) The Offeror’s proposed computerized management system
(b) Materials ordering, handling and tracking
(c) Plans and processes for meeting applicable regulatory requirements
(d) Safety
(e) Quality control
(f) Work control procedures, to include a description of work order processing from receipt of the request to closeout.
(g) The Offeror’s plan to minimize interruptions or disruptions to facility operations during normal operation and maintenance performance.
2.2.5 Provide a description of how the Offeror will respond quickly to different situations encountered in the operation and maintenance of a laboratory research facility. At a minimum, your technical approach is to:
(a) Discuss the proposed response and completion times for all priorities defined in this solicitation, to include examples that support the Offeror’s capability using the same methods and processes proposed.
(b) Discuss the methods that will be used to perform (initiate and complete a work order) within the times required in this solicitation. If the Offeror proposes shorter response or completion times than stated in this solicitation, the proposed times will be accepted as the basis for contract compliance.
2.3 VOLUME I TAB-C: Management Approach. Do not include cost information in the Management discussion. The Offeror shall discuss its management structure with a description of how the structure supports the requirements of the solicitation.
The maximum page limitation to include charts, tables, etc. is limited to 5 pages for Volume I Tab-C Management. Any information past this number will not be evaluated. Resumes shall not count against the page limit.
2.3.1 Project Management Structure. The Offeror shall clearly describe its management structure to include the roles and responsibilities of each level of management proposed (site and corporate) . A simple organizational chart shall be provided to show the lines of authority for the management structure proposed.
2.3.2 Personnel Qualifications. The Offeror shall provide resumes (highlighting experience directly related to the requirements of this solicitation) and job descriptions (role and responsibility) for those individuals performing the function of Project Manager, Safety Manager, and Quality Control Manager and any other primary personnel who will be directly, routinely involved above the foreman level to include all personnel authorized to conduct negotiations and/or commit the Offeror by signature on the basic contract or modifications.
Individuals may perform more than one assigned role (Safety/QC Manager for instance) if identified as such in the accepted proposal. The information shall show their areas of responsibility and relationship with the management structure. The use of subcontractors to fill primary roles in the project management structure is not allowed. The qualifications of the personnel should be provided to include their general experience, as well as their specific experience, in medical research facility operation and maintenance or operation and maintenance experience of a comparable degree of risk and complexity. For purposes of this proposal, information shall only be provided for the key personnel identified above. However, the Offeror should have on staff or have the ability to utilize acquire personnel of suitable background and experience to ensure that all the required disciplines are represented.
3.0 VOLUME II COST/PRICE PROPOSAL. The Price Proposal will consist of the Excel worksheets provided with this solicitation.
3.1 In submitting the Price Proposal, the Offeror will be required to fully complete the worksheets and include prices, quantities, and lists of labor classifications, etc. The Offeror is responsible for the accuracy of all data input, formulas and calculations. Offerors are cautioned to use the most current cost proposal worksheets information in the solicitation or in the latest amendment. The Offeror may not alter a price requirement, etc., within the worksheets, as it may prevent the effective evaluation of your price proposal.
3.2 Bid Schedule Notes.
3.2.1 Cost Line Item Numbers (CLINs) in Bid Schedule 1 are numbered using the following numbering convention. The first digit represents the bid schedule number. The second digit represents the site number (1 for Lima; 2 for Iquitos; 3 for Puerto Maldonado). The third digit (1 through 5) represents the 12-month period. The fourth digit (1-2) represents the type of CLIN (1 for Labor Rates and 2 for Prime Contractor Markups). The final character represents the sub line item (A through T, depending on the number of sub-items). For example, CLIN 1111A is the CLIN for Bid Schedule 1, Lima, Operation & Maintenance, Base Year, G.C. Site Supervisor.
3.2.2 CLINS in Bid Schedule 2 are numbered sequentially by site and reflect the proposed cost to operate and maintain the identified site during the identified 12-month period. The first digit represents the bid schedule number. The second digit of each CLIN represents the site number (1 for Lima; 2 for Iquitos; 3 for Puerto Maldonado) and the third and fourth digits (01-05) represent the 12-month period. The final character represents the sub line item (A through D, depending on the number of sub-items). For example, in Bid Schedule 2, CLIN 2101 is the CLIN for the cost of preventive maintenance of the Lima site during the base year of this contract.
3.2.3 The unit prices for the sub-items under each CLIN in Bid Schedule 1 shall represent the cost to the prime contractor for those sub-items, not including the prime's General and Administrative Overheads ("G & A", "Home Office", "Branch Office," etc., as applicable) and not including "Profit" or "Fee," which are reflected in the “Prime Contractor Markup” CLIN as described below.
3.3.4 Prices for labor CLIN sub-items in Bid Schedule 1 shall include all burdens, such as but not limited to labor rates accounting for any applicable Host Nation labor laws, codes, and standards; labor taxes and insurance as applicable; health, morale and welfare costs as appropriate; direct retirement costs if applicable; other applicable fringe benefits and a small tools allowance for trades. Do not include subcontractors' markups to the prime contractor in the labor rates. Labor rates should reflect only the burdened hourly cost for the identified discipline.
3.3.5 Individual unit prices for all CLINs in both bid schedules shall be provided for each site per the bid schedules.
3.3.6 The percentages for sub-items in Bid Schedule 1 under CLINs with the fourth character “2” (prime contractor mark-up) shall consist of the prime contractor's percentage markups for "General and Administrative Expenses", "Profit" (or "Fees") and "Subcontract Handling Fee", to be applied to specific CLIN sub-items as described in the bid schedule. Handling fees shall not include any duplication of direct charges, nor shall they be inflated to increase anticipatory profit. The handling charges shall be the maximum handling charges to be used for any modifications to this contract. These handling charges may be negotiated downward with award of individual modifications.
3.3.7 The hourly unit prices/rates listed on Bid Schedule 1 establish the base line contract rates for the base period (1st twelve months) of this contract.
3.3.8 The contract rates in Bid Schedule 1 shall be the maximum rates to be used for any modifications to this contract and shall serve as the basis for the pricing developed for each requirement in Bid Schedule 2. It is possible that not all disciplines will be required for performance of this contract, but the negotiated rates for the disciplines will apply if that discipline is required for performance of the contract or any modification thereto.
3.3.9 The Bid Schedules are set up based on sites as follows:
Site 1 – Lima Site 2 – Iquitos Site 3 – Puerto Maldonado

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT
YOUR EVALUATION OF THE CONTRACTOR’S PERFORMANCE
RATING DEFINITION NOTE
(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating.
Also, there should have been NO significant weaknesses identified.
(VG) Very Good
Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
(NA) Not Applicable
No information or did not apply to your contract
Rating will be neither positive nor negative.
