They may be customer requests approved by the Facility Manager, repair requirements identified during the performance of PMs, or any other similar work that is identified by the Contractor, the COR, and/or the Facility Manager. A request for maintenance or repair is incidental to the performance of operations and maintenance and included within the operations and maintenance contract requirements.

3.4.1 Schedule B identifies a Not to Exceed amount for unscheduled maintenance support for which the Contractor shall be responsible. Work against the unscheduled maintenance contract line item will be invoiced monthly based on actual effort completed on assigned service orders, to include labor, parts, and materials costs specific to each approved service order. The Contractor shall notify the COR immediately if/when the charges against any Not to Exceed line item reach 50% of the value of that line item, to provide adequate time for the Government to plan for future efforts. The Contractor shall then notify the COR immediately if/when the charges against any Not to Exceed line item reach 80% of the value of that line item. The Government will then either initiate a modification to add funds to the line item or will inform the Contractor to cease any work which would cause the Contractor to reach or exceed the obligated line-item amount. The Government shall be obligated only for the work that the Government actually directs the Contractor to perform, and that the Contractor actually performs.

3.4.2 Service Orders, including Customer Service Requests, will be assigned the appropriate Priority Code according to the following priority levels.

3.4.2.1 Priority 1: Emergency Work. Response to an emergency situation is defined as any condition that requires immediate action to eliminate life or serious injury hazards to personnel, prevent loss or damage to Government property, or restore essential services. The Contractor must respond within 15 (fifteen) minutes during duty hours.

The Contractor shall continue to work on the emergency until the problem is resolved or mitigated to a condition such that it can be classified as a lower priority. For non-duty hours or for Priority 1 calls at sites that are not regularly manned by the Contractor during business hours, the Contractor shall be on site within 1 (one) hour after notification by an authorized government representative.

3.4.2.2 Priority 2: Work of an urgent nature which must be initiated no later than the next regularly scheduled work shift. This priority is assigned to those pieces of equipment or systems for which there are temporarily sufficient alternatives. Priority 2 also includes work to correct an unsafe condition which is not an immediate hazard to personnel. Priority 2 service orders should be accomplished within two working days.

3.4.2.3 Priority 3: Any work to improve facilities building operation which can be planned for completion within ten working days.

3.4.2.4 Priority 4: Any work that is a valid requirement for proper operation of the facility, site, or specific systems, but that does not fall within the above categories. This will be completed within 25 calendar days. Major repairs and the construction of new systems, components, facilities and buildings is not considered unscheduled or demand maintenance and will not be performed as a service order but would require a modification to this contract.

3.4.3 Service orders assigned under the unscheduled maintenance line item will include “routine” maintenance, repair, and in some cases incidental construction of basic infrastructure and systems.

3.4.4 Service orders for specialty system repairs require a specialty technician or subcontractor to perform the work. Systems include, but are not limited to, systems such as fire alarm, access control and CCTV system, building automation system, fire protection system, chiller and boilers. Specialty technician or subcontractor must provide certification to demonstrate they are certified by manufacturer of the brand or similar to perform the work.

Special requirements for certifications and licenses may be included in the SOPs.

3.4.5 Service orders may also be issued for technical reachback, which will include telephone and/or online/email access to Contractor (inhouse and/or subcontract) subject matter experts for assistance to the NAMRU- 6 inhouse maintenance staff in troubleshooting maintenance and repair issues. Technical reachback will not include visits by Contractor personnel to the site. Technical reachback service orders will be limited to five (5) per annual performance period, unless that number is increased by a modification to this contract.

3.4.6 Unscheduled Maintenance Requirements: The NAMRU-6 inhouse maintenance staff will perform 75% of the “standard” service orders at the Lima site and 60% of the standard service orders at the Iquitos site. Standard service orders include basic repairs and services such as repair of drywall, repair overflowing toilet, etc. The Contractor will be responsible for the remainder of standard service orders. The Contractor will also be responsible for 100% of “specialty” service calls which typically require manufacturer certified technicians or repairs of similar complexity, such as repair of refrigerant leaks for HVAC equipment. The Contractor’s responsibility for unscheduled maintenance at the Lima and Iquitos sites will be limited to service orders assigned to the Contractor by an authorized government representative in the customer service order request system described below. The Contractor shall be responsible for all unscheduled maintenance requirements at the Puerto Maldonado site.

3.4.6.1 Service orders at the Lima and Iquitos sites will typically be given a priority and then be assigned to the Contractor in the system by the Facility Manager or his designee. An online customer service order request system is in use at the Lima and Iquitos sites. The online system will continue to be monitored and managed by NAMRU-6 staff. Service orders generated in the system may be assigned to the Contractor upon approval and prioritization by the Government within the limits described above. Upon receipt of a service order, the Contractor shall submit a level of effort estimate and obtain the Facility Manager’s approval prior to scheduling and performing the work within the timeframes required above (starting from the date/time of Government assignment of the service order).

3.4.6.2 At the Puerto Maldonado site, the Contractor shall assign priorities as described above to each service order request received, regardless of the source of the service order request. If the service order is estimated to require less than 4 labor hours and/or $500 in materials, the Contractor shall proceed with scheduling and performing the service order within the timeframes required above, with immediate notice to the Facility Manager.

If the requirement is covered by the scope of this contract, but the level of effort estimated to complete the service order exceeds 4 labor hours and/or $500, the Contractor shall submit the level of effort estimate and obtain the Facility Manager’s approval prior to scheduling and performing the work within the timeframes required above (starting from the date/time of Government approval of the service order).

3.4.6.3 The Contractor shall be responsible for documenting in their approved automated work management system, each service order or other unscheduled maintenance task performed at all sites. The Contractor shall submit a monthly report to the COR and Facility Manager, with a summary of all service orders performed during the month. The report shall provide for each service order, the information required by this specification, and shall include a list of (with reason for) any backlog of pending service orders. The Contractor shall coordinate with the COR and Facility Manager to determine an acceptable format for this report, which may include submission of or reference to copies of service order records for information captured in those records.

3.5 Exterior Grounds Maintenance. The Contractor shall provide all labor, management, supervision, tools, materials, and equipment necessary to provide grounds maintenance and landscaping services at NAMRU-6 sites in a sightly manner consistent with its intended purpose. The extent of required exterior grounds maintenance is dependent upon the site.
