About this file
This document is a procedural guide from Wisconsin State Agency outlining the process for recording alleged food harm incidents involving products supplied by IFD Foodservice Distributor, located in Eau Claire, Wisconsin. The procedure establishes a systematic approach for state agency personnel to document and report food-related incidents or injuries, requiring immediate incident response, detailed documentation, product isolation, and direct communication with IFD's inside sales team through specified email contacts.
The document does not represent a traditional contract but rather a service protocol for food incident reporting and management. It requires comprehensive documentation including incident details, location, time, product codes, lot numbers, and manufacturer information from case packaging. The procedure mandates isolation of affected product inventory and immediate reporting to IFD Foodservice Distributor via multiple contact methods including phone (800) 873-0131 and specific email addresses for inside sales representatives. The document includes detailed visual examples showing how to identify and record critical product information from case packaging, can lots, produce brands, and dairy carton markings, emphasizing traceability for food safety purposes. This revised March 2021 protocol appears to be part of a broader food service supply arrangement between Wisconsin state agencies and IFD Foodservice Distributor.
