About this file
This is a Past Performance Client Questionnaire template (Attachment M) for solicitation 36C25225R0004 related to an Expand Boiler Plant project (585-21-115) for the Department of Veterans Affairs. The questionnaire is designed to be sent by contractors to their references, who must complete and return it directly to the VA Contracting Officer.
The questionnaire includes a comprehensive evaluation matrix covering seven key performance areas: Quality, Schedule/Timeliness, Customer Satisfaction, Management/Personnel/Labor, Cost/Financial Management, Safety/Security, and General Performance. References rate the contractor's performance using a scale from Exceptional (E) to Unsatisfactory (U), with detailed rating definitions provided. Evaluation criteria include technical performance, schedule compliance, problem resolution, management effectiveness, cost control, safety practices, and emergency response capabilities. The form requires references to provide narrative feedback on strengths and weaknesses and indicate whether they would work with the contractor again. All responses must be submitted to Contracting Officer Kristi Kluck at the VA Network Contracting Office 12 in Milwaukee, WI.
