About this file
This is a Weekly Payroll Certification Form (LLC-25) issued by the Pennsylvania Department of Labor and Industry, Bureau of Labor Law Compliance, Prevailing Wage Division, for use on public works projects in Pennsylvania. The form is designed to document weekly payroll compliance for contractors and subcontractors working on state-funded construction projects, including the West Pump Station Replacement Project being undertaken by Northwestern Chester County Municipal Authority. The form requires contractors to report detailed employee payroll information including hours worked, work classifications, hourly rates, fringe benefits, deductions, and gross pay for each week of work. Contractors must certify compliance with prevailing wage requirements and predetermined rates as mandated by the Pennsylvania Prevailing Wage Act of 1961. The form must be notarized on initial and final submissions only, with all other weekly submissions requiring standard completion of payroll data.
The certification form requires contractors and subcontractors to declare that neither they nor any affiliated firms are debarred by the Secretary of Labor and Industry, and to confirm that no portion of the contract has been or will be subcontracted to debarred entities. Contractors must provide their legal name, business address, and identifying information of owners, partners, or officers. The form emphasizes that willful falsification of any statements may subject contractors to civil or criminal prosecution under the Pennsylvania Prevailing Wage Act. Fringe benefits must be documented separately, including medical or hospital care, pension or retirement, life insurance, disability, vacation and holiday benefits, and other specified benefits, with hourly contribution amounts clearly identified for both cash payments and benefit fund contributions.
