Task Assignment Change Order Summary
This document is a Task Assignment Change Order (TACO #1) issued by the State of Florida Department of Environmental Protection (FDEP) to Professional Service Industries, Inc. (Intertek-PSI) for environmental remediation and monitoring services at the Custom Commerce site (ERIC_5234) located in Port Richey, Pasco County, Florida. The change order amends the original Task Assignment DC006C under Contract HW686 and was executed on September 25, 2024, with departmental approval from Jennifer Farrell and contractor authorization from Joshua S. Newell.
The change order reduces the total task value by $233.00 through the elimination of one analytical sample from Subtask 3 (2nd Quarter Monthly Operations and Maintenance), resulting in a revised total contract value of $63,009.18, comprised of $54,369.18 in fixed unit rate costs and $8,640.00 in cost reimbursement expenses for utilities. The scope includes twelve months of operations and maintenance activities, two semi-annual Remedial Action System Status (RASS) reports, one groundwater monitoring event, and associated deliverables with a completion date of February 28, 2025. Labor costs, travel, subcontractor services, laboratory analytical testing, field equipment, and utilities are itemized across ten subtasks, with quarterly invoicing requirements tied to FDEP approval of specific deliverables including monthly operations summaries, RASS reports, and analytical results.
