Re: Proposal for Remediation System Decommission – Rev 1 Coin-O-Magic 11605 North 15th Street Tampa, Hillsborough County, Florida 33612 Facility ID No. 299500268, ERIC_4690

Dear Mr. Grubbs, Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to decommission services for the above referenced site. Included with this proposal is a brief discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion.

PROJECT BACKGROUND

Historical records indicate that this facility utilized perchloroethene (PCE) drycleaning from 1986 through 1994 at which time the drycleaning business was converted to a dry-drop-off and coin-operated laundromat. A Site Screening Investigation completed in August 1996 identified PCE impacts to soil ranging from 46 μg/kg to 83 μg/kg. Based on the Drycleaning Solvent Cleanup Program (DSCP) Application/Site Screening Report for Coin-O-Magic Laundromat in October 1996, the site was determined to be eligible for state-funded cleanup under the DSCP Program with a score of 96.

A Site Access Agreement between the property owner and the FDEP (and its contractors) was signed July 31, 2017. As of January 2017, no large Public Supply Wells have been identified within

FDEP – Coin O-Magic, Decommission

½ mile of the site, whereas one small potable private well is located within ¼ mile of the site. The private potable well provides water for the Hackworth Duplex located at 11720 North 15th Street, which is about 1,100 feet northwest of the Site.

A site assessment was conducted for the Drycleaning Solvent Cleanup Program beginning in May 2018, which consisted of a modified active gas survey (MAGS), a groundwater sampling event, soil screening, and a Site Assessment Report (SAR) was submitted on April 3, 2019. Results indicate that PCE concentrations were not detected above the soil cleanup target levels (SCTL’s). Air samples collected from MAGS test points revealed 4.3 mg/m3 PCE at VE001 and 3.7 mg/m3 PCE at VE003. Groundwater assessment showed low levels of VOH’s present in the saturated zone, however, none were detected above the groundwater cleanup target levels (GCTL’s). The highest dry cleaning contaminant of concern detected from that event was 1.2 μg/L of PCE at DP002 (21- 25 ft bls).

The site assessment results collected from inside and adjacent to the former drycleaner building suggested that a limited mass of residual PCE was present in the area around the former location of the dry-cleaning machine or the active lint trap system. Recommendations at that time included performing soil vapor extraction for vadose zone remediation.

The Interim Source Removal Plan for an SVE System installation was approved by the FDEP on April 22, 2020. The construction activities included; transfer and placement of the SVE trailer, replacement and retrofitting of the SVE wells, construction of the SVE conveyance piping, and electrical connections within the system compound, were completed from May 2021 to August 2021. The electrical service connection with Tampa Electric Company (TECO), including additional modifications to the temporary power pole, were completed in October 2021. The additional repairs to the trailer ventilation and control panel were also completed in January 2022. A Department-owned SVE remediation system trailer (property number 12809) was transferred to the site and was set-up at the northeast corner of the building.

The remediation equipment includes a 7.5 horsepower Amtek Rotron EN808 regenerative vacuum blower, a five-leg inlet manifold (three in use for this project), an 80- gallon aluminum vapor/liquid separator, a ½ horsepower liquid transfer pump, a 110- gallon closed top polytank, and an EOS Pro-2 PLC with cellular modem.

The SVE system operated from January 2022 to May 2023. A system shutdown evaluation was conducted from February 2023 to March 2023. Based on the results from the shutdown evaluation, FDEP recommended ceasing operation of the SVE system. The system electric service ended on May, 24, 2023 and the unit has been off since that time.

June 20, 2024 www.intertek.com/building

PROPOSED SCOPE OF WORK

The proposal contained herein consists of project management, removal and transport of the remedial system, waste analysis and disposal, removal of existing power pole, abandonment of all remedial conduits; abandonment of eight (8) monitoring wells and four (4) vapor extraction wells; and submittal of a summary letter report.

1. Project Management

This scope is provided for in the template worksheets.

2. Removal and Transport of Remedial System, Waste Analysis and Disposal, Power Pole Removal, and Abandonment of Remedial Conduits

The remedial system equipment trailer and associated components will be disassembled and transported to the FDEP equipment yard in Lakeland, Florida. All accompanying attachments, fittings, and essential components will be securely housed within the equipment trailer for safe transport. All anchor points and other supporting infrastructure will be removed or cut-below-grade. The remedial system equipment trailer is not currently street-ready and will need to be transported on a flatbed trailer or equivalent.

This remedial system does have a carbon vessel that contains activated carbon on site. A sample of the used carbon will be collected and submitted for laboratory analysis by EPA Method 8260 for halocarbons and non-halogenated VOCs. Following waste characterization and profile, the carbon will be transported and disposed of at an FDEP-approved facility.

Any SVE conduit lines running along the building interior/exterior (walls and ceilings) will be removed with their brackets/supports and any resulting penetrations will be repaired.

All building access penetrations related to the conduits will be patched/filled as well.

The existing power pole will be removed with electrical components once de-energized by the utility. A SVE well trench is present at the site and will be cut below grade, filled with flowable fill, then capped and patched at grade. Quotes for subcontractor disposal, system removal, and transport/disposal are attached.

3. Abandonment of Monitoring Wells and Vapor Extraction Wells

The proposed abandonment of monitoring wells on the site will include MW001, MW002, MW003, MW004, MW005, MW006, MW007, MW008 and vapor extraction wells VE001, VE002, VE003, and VE004. The abandonment will adhere to the regulatory standards outlined in Chapter 62-532 of the Florida Administrative Code (F.A.C.), ensuring compliance with FDEP guidelines for abandonment procedures. This will include applying

June 20, 2024 www.intertek.com/building for relevant well abandonment permits for groundwater wells only. Bids for licensed subcontractor drill services to complete the abandonment are attached.

4. Summary of Activities Report

A brief letter report will be developed which will describe the SVE system decommission activities, building or site repairs, disposal documentation, required permits, and include completed field documentation, as well as photo-documentation of site activities.

COST ESTIMATE AND SCHEDULE

The estimated cost of decommission is shown on the attached Cost Estimate.
