The scope of work encompasses four primary subtasks: project management, first mobilization geoprobe activities (direct push technology, soil sampling, monitoring well installation, and magnetic acoustic gravity screening testing), groundwater sampling with investigative derived waste disposal, and site assessment reporting. The total task value is $73,008.46 on a fixed unit rate basis, with deliverables scheduled through December 31, 2026, and task completion by February 27, 2027. Cost breakdowns include $54,230.41 for subtask 2 (drilling and assessment), $4,889.48 for subtask 3 (groundwater sampling), and $13,888.57 for subtasks 1 and 4 (project management and reporting). Subcontractors were competitively bid, with drilling services awarded to ATI Companies at $15,830.00, surveying to United Survey and Mapping Inc. at $1,930.00, utility locating to GeoTek Services at $750.00, and waste disposal to Erwin at competitive rates ranging from $110-$125 per drum depending on waste type and location.

Re: Proposal for Initial Site Assessment Activities Revision 2 Sterling Cleaners Formerly 182 37th Avenue North (Currently 218 37th Avenue North) St. Petersburg, Pinellas County, Florida
ERIC_5319
Dear Ms. Uribe:
Professional Service Industries, Inc. (PSI), an Intertek Company, is pleased to provide this proposal for conducting initial site assessment activities at the former “Sterling Cleaners” located at 218 37th Ave. N in St. Petersburg, Pinellas County, Florida. Included with this proposal is a brief summary of the project background to date, proposed scope of work, cost estimate and a proposed schedule for completion. A Site Access form was signed by the property owner “Publix Supermarkets, Inc.” in 2023.
PROJECT BACKGROUND
The Northeast shopping Center was constructed in 1958 and drycleaning operations for Sterling Cleaners became active around 1970. A preliminary Contamination Assessment Plan for Sterling Cleaners was proposed in August 1992 after it was revealed that groundwater samples at a nearby site were found to contain elevated levels of drycleaning related compounds. In April 1996, an initial site screening was conducted which included soil sampling. The most contaminated soil sample, located near the former drycleaning machine, was found to contain 258.6 mg/kg tetrachloroethene. The site was determined to be eligible for state-funded clean up in May 1996 and was issued an Eligibility Letter with a site score of
65. In 1997, the drycleaning facility was permanently closed and a renovation that removed the southeast wing of the shopping center (including the site of the former drycleaner) began. During this renovation, limited remediation efforts (soil excavation) were made to remove soils with concentrations of tetrachloroethene (PCE) exceeding residential soil cleanup target levels from the former drycleaning location. An FDEP Rapid Risk Assessment was conducted in 2011 which confirmed the current site status.
FDEP – Sterling Cleaners
Revised March 19, 2026 www.intertek.com/building
FDEP approved funding to begin active site remediation in 2024 and Intertek-PSI submitted an initial Site Assessment Work Plan to begin active remediation in 2024.
PROPOSED SCOPE OF WORK
The proposal contained herein consists of conducting initial site assessment related work including drilling, sampling and well installation activities, along with generating a report deliverable. The results will be used to determine current soil, soil vapor and groundwater impacts related to the former drycleaning operations on this site. The assessment and sampling procedures will be conducted according to the FDEP Standard Operating Procedures and Drycleaning Solvent Cleanup Program protocols.
Specifically, the proposed scope of work consists of the following subtasks:
1. Project Management
This scope is provided for in the template worksheets.
2. First Mobilization Geoprobe (Direct Push): GPR utility location; Lithologic Boring, Soil Sampling, DPT Screen point Sampling; Monitoring Well Installation; Hand Auger MAGS Well Installation and MAGS Test; Survey Top of Casing elevation and locations.
a. Obtain groundwater well installation permits for the groundwater wells proposed in Figure 1.
b. Obtain a standard utility locate via Sunshine 811 for all locations; verify all soil locations using a GPR subcontractor.
c. Coordinate with the tenant/owner to provide site access.
d. PSI will subcontract drilling services in order to accomplish all drilling related activities.
e. Perform one (1) continuous lithologic profile boring up to 50 feet below land surface (ft-bls) or refusal depth with a DPT drill rig. Assess and classify soils and complete the boring with grout to the surface.
f. Advance seven (7) exterior soil borings to the water table (after hand auger clearance) with a DPT drill rig. Classify soils and collect discrete analytical samples at standard depth intervals (0-0.5 ft-bls, 0.5-2 ft bls, 2-4 ft-bls, 4-6 ft-bls, and 6-8 ft bls). Analytical soil samples will be collected to approximately ~1 ft above the water table. Soil analytical will include the full list of volatile organic compounds (VOCs) via EPA Method 8260 Full, and contingent leachable VOC’s via SPLP analysis. This proposal assumes 100% of the soil samples collected will need SPLP analysis provided. In reality, no more than 10% are expected and a Task Assignment Change Order will be submitted to reduce the quantity of SPLP analysis quantity to reflect the actual number of samples analyzed.
g. Submit soil samples to a NELAP certified laboratory.
h. Complete seven (7) exterior soil borings as MAGS wells.
i. The MAGS™ wells will be installed approximately 1-foot above the groundwater table and are anticipated to be screened from 1 foot bls to 3 feet bls. They will be constructed with a 2-inch diameter 2-foot lengths of 0.01-inch slot size screen with 1 foot of solid riser. The well will be packed with 20/30 silica sand followed
Revised March 19, 2026 www.intertek.com/building by a 30/65 silica sand seal and grouted to surface where it will be completed with a 2 foot by 2 foot concrete pad and an 8-inch manway cover.
i. Conduct groundwater screenpoint sampling using a DPT drill rig equipped with a screenpoint groundwater sampler at seven (7) locations. Collect groundwater samples from each screen interval (3-7 feet bls, 13-17 feet bls, 23-27 feet bls, 33-37 feet bls, and 43-47 feet bls) and submit to a NELAP certified lab for analysis of VOC’s via EPA Method 8260 Full.
j. Use a DPT drill rig (once utilities are cleared with a hand auger) to install up to four (4) shallow monitoring wells and 1 deep monitoring well.
i. Each shallow monitoring well will be installed with a DPT drilling rig and will be constructed with 1-inch diameter 10-foot lengths of pre-packed screen (0.01-inch slot size) from 3-13 ft bls and packed with 20/30 silica sand followed by a 30/65 silica sand seal and grouted to surface.
ii. One deep well will be constructed with 1-inch diameter 5-foot length of pre-packed screen (0.01-inch slot size) from 25-30 ft bls and packed with 20/30 silica sand followed by a 30/65 sand seal and grouted to surface.
k. Develop all wells until discharge water is visually clear of sediment.
l. Containerize investigative derived waste (IDW) in 55-gallon drum, label appropriately and stage on-site, pending waste characterization results.
m. PSI will subcontract a Surveyor to collect GPS coordinates and Top of Casing elevations.
n. Collect groundwater samples and elevation measurements from all four (4) monitoring wells.
o. Containerize investigative derived waste (IDW) in 55-gallon drums, label appropriately and stage on-site, pending waste characterization results.
p. Conduct a MAGS test and collect air samples at each MAGS well head for analysis by EPA
Method TO-15.
3. Second Mobilization: Monitoring Well Sampling / IDW Disposal /Update SAWP Figures
a. Coordinate with the tenant/owner to provide site access.
b. Mobilization to the site during each day of sampling activities.
c. Collect groundwater samples from up to 7 monitoring wells for Full List VOC’s via EPA
Method 8260.
d. Submit groundwater samples to a National Environmental Laboratory Accreditation
Conference (NELAC) approved laboratory for analysis of VOCs via EPA Method 8260. No blanks or duplicates will be included per the Waste Site Cleanup Section.
e. See Subtask 5 for PFAS analysis of groundwater samples collected in this Subtask.
f. Containerize investigative derived waste (IDW) in 55-gallon drum, label appropriately and stage on-site, pending waste characterization results.
g. A subcontractor will transport and dispose of IDW at an approved facility. A signed manifest will be provided by the waste management company prior to pick-up and disposal.
h. Update SAWP maps with all monitoring well and MAGs locations.
Revised March 19, 2026 www.intertek.com/building
4. Reporting: Site Assessment Report
a. A Site Assessment Report will be developed which will include figures, tables, and charts developed to document and summarize soil, soil vapor, and groundwater field parameters and analytical data for analyzed Constituents of Concern. RE: Utility Locating Services – 218 37th Ave N, in St. Petersburg, Florida
Mr. Newell, GeoTek Services is pleased to respond to your request for Utility Locating services at 218 37th Ave N located in St. Petersburg, Florida. We will scan a (10’ x 10’) area around the nineteen proposed bore locations with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, or vehicles. We will scan as close as reasonably possible to fixed objects such as curbs, vehicles, fences, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. Our utility locating quote for this project is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for ninety days.

Subject: Proposal to Perform a Utility Designation Study 218 37th Avenue North St. Petersburg, Florida GeoView Proposal Number: 12170p
Mr. Newell, The purpose of this letter is to transmit a proposal to complete a geophysical designation study at the subject site. This proposal is in response to your request for a proposal made during your recent conversations with our office. GeoView Associates, Inc. (GeoView) appreciates the opportunity to provide our services on this project. We look forward to hearing from you soon.

Proposal This document shall serve as a proposal for work to be performed by
GeoView Associates, Inc. (GeoView) for Intertek-PSI.
Scope of Work
The project site is located at 218 37th Avenue North in St. Petersburg, Florida. The purpose of the geophysical survey will be to determine the location of underground utilities within a 10 feet radius of 19 proposed borings The location of the study area is provided on Figure 1.
The geophysical survey will be conducted using ground penetrating radar (GPR) and conventional electromagnetic (EM) pipe and cable locators. The GPR survey will be conducted using a GSSI GPR system. The appropriate GPR antenna configuration and time range and processing parameters will be determined in the field by the project manager.
The EM survey will be conducted using a Subsite 95R and/or a Radiodetection RD (8000, 8100 or 8200) system. As appropriate, the EM survey will be conducted using both passive and active modes. The passive mode is used to find cables and conduits carrying live power (50-60 Hz) and communication (29-33Hz) signals. For the active mode survey, a current is transmitted directly or broadcast into using electromagnetic induction a metal pipe or conduit and the path of the pipe/conduit is identified by following the signal with a portable receiver.
The active mode is used to trace out cast iron pipes, metal water and sewer lines, utilities that have trace wires, and inactive power and communication lines.
Based on information provided by Intertek-PSI, it does not appear that the work at any of the sites will require MOT (traffic calming efforts). This proposal does not include the costs for the design or implementation of an MOT program.
Presentation of Results The location and estimated depths of underground utilities or any other identified structures shall be indicated on the ground surface using spray paint or other methods as specified by Intertek-PSI. No report shall be prepared.
Compensation Cost to complete the survey will be as follows:
Mobilization: $0 Field Investigation: $1500 Data Analysis and Report Production: Note Required
Total: $1,500
If a scaled AutoCAD site map is required to document the study results, an additional fee of $200 will be added. This price is inclusive of all charges associated with the project. Unless otherwise agreed upon, GeoView shall be compensated for all services within 30 days of the invoice date. GeoView shall be paid in full regardless of whether the results of the geophysical survey are what Intertek-PSI anticipated. If it is determined during the survey that the geophysical survey will not achieve the objectives of the project, Intertek-PSI will immediately be notified. If a decision is made to discontinue the survey, only charges for time and materials costs to that point will be submitted.
These prices are based upon GeoView being able to access the site during normal business hours, Monday through Friday. It is also based upon the assumption that no site-specific training or drug testing will be required. If there are any such requirements, additional costs will be incurred.
Requirements of Client Intertek-PSI will provide a scaled map and digital file, if available, of the project site showing the boundaries of the project site, areas of specific concern and pertinent landmarks. Intertek-PSI will also supply all available information regarding the presence and history of underground utilities at the project site (see limitations for further discussion). Unless otherwise requested, Intertek-PSI will be responsible for the coordination of site access, traffic control, clearing of onsite obstructions or any other logistical consideration necessary to conduct the survey.
If any test drilling, monitor well installation or other excavation activities are planned it will be the responsibility of Intertek-PSI to submit a notification (dig ticket) to the appropriate 811 One-Call agency. It is recommended that such notification be filed sufficiently in advance so that GeoView will have the opportunity to incorporate the 811 findings into their results.
Limitations The objective of the geophysical survey is to determine the presence and configuration of underground utilities. However, the geophysical response of these features may be highly complex depending upon on the physical characteristics of the underground utilities, the electrical properties of the surrounding soils, the depth of burial of the utilities, and the proximity of the utilities to each other.
GeoView shall conduct the geophysical survey using the most “up-to-date” geophysical equipment in a manner consistent with the level of care and skill ordinarily exercised by members of the geophysical profession practicing in the same locality under similar conditions. GeoView’s utility locating personnel are all certified GPR and conventional pipe and cable locator operators with extensive in-house training.
It is acknowledged that GeoView is not an 811 Sunshine One-Call or equivalent utility mark-out service. GeoView does not have access to any utility-related records for either public or private properties. It is the responsibility of Intertek-PSI to obtain such records and provide to GeoView prior to the site investigation. GeoView will use these records during the site investigation and report to Intertek-PSI any utilities that are shown on the site records but cannot be confirmed by GeoView. GeoView will not mark on the ground surface any utilities which it cannot confirm.
GeoView shall only be liable for the successful locating of underground utilities or any other underground structures whose presence is both known and indicated on site maps to be provided prior to the performance of the geophysical survey. Every attempt will be made to locate both known and unknown utilities within the survey area. However, GeoView will only be liable for utilities whose presence is known prior to the survey. Intertek-PSI accepts that due to the inherent limitations of non-intrusive geophysical technologies GeoView cannot be held liable for damages to utilities that cannot identified using non-intrusive technologies.
Intertek-PSI accepts that a non-intrusive utility designation study does not replace the need to employ widely accepted industry safety standards for subsurface work. Concerning the installation of well or test borings this standard of care includes post-holing or hand auguring at the planned test locations to the minimum depth at which underground utilities are anticipated at the project site. If absolute confirmation is needed to a specified depth that underground utilities are not present at a planned work location GeoView can provide vacuum excavation services. Pricing for such services is available upon request.
When Intertek-PSI is working around indicated utilities, a minimum of 2 ft clearance from either side of the marked utility will be maintained. Unless otherwise indicated, a minimum vertical clearance of +/- 20 percent of the indicated depth shall also be maintained. Intertek-PSI maintains the responsibility for manually exposing any indicated underground utility for absolute confirmation.
Authorization and scheduling of the project is considered acceptance of these terms and conditions.
Other Terms and Conditions Per Master Services Agreement with Intertek-PSI.

General Description and Limitations of Geophysical Methods Ground Penetrating Radar (GPR)
The GPR survey will be conducted using GSSI GPR systems. The survey will be conducted with either a 900, 400 or 350 mega-Hertz antenna. The appropriate antenna will be determined in the field by the project manager. GPR will be able to provide both the position and estimated depth of both metallic and non-metallic underground utilities.
The GPR method uses electromagnetic signals to detect subsurface utilities.
The depth of GPR penetration is primarily limited by the attenuation of the signal within subsurface soils. Attenuation occurs when the electromagnetic signal is absorbed by soil materials, preventing the GPR from detecting deeper features.
Factors such as soil composition, moisture content, and conductivity significantly affect signal attenuation. For example, clay-rich soils or soils with high moisture content can dramatically reduce the effective depth range of GPR. Once the signal is fully attenuated, no further data can be collected from deeper layers, which may result in undetected utilities at greater depths.
The ability of GPR to accurately detect a utility depends on the contrast in dielectric constants (conductivity) between the utility and the surrounding soils.
Higher dielectric contrasts produce stronger signal reflections, enhancing the detectability of underground utilities. Metal pipes, such as steel, typically provide a strong contrast against sandy soils, making them easier to identify. In contrast, non-metallic pipes, such as PVC, ceramic, or concrete, present a lower dielectric contrast, which may result in weak or indistinguishable reflections. Consequently, utilities made from non-metallic materials may be difficult or even impossible to detect using GPR.
The size and depth of a utility also influence GPR's effectiveness. GPR resolution decreases by approximately 0.5 to 1 inch per foot of burial depth. For example, a small 2-inch diameter pipe may not be detectable at depths greater than 2 to 4 feet. Additionally, surface conditions, such as reinforced concrete, asphalt, or metal debris, may scatter or block GPR signals, further complicating utility detection. Adverse site conditions, including the presence of saltwater, highly conductive soils, or large subsurface obstructions, can also interfere with the accuracy of GPR data.
Conventional Electromagnetic Pipe and Cable Locators (EUL)
A Radiodetection System is used to locate underground utilities using conventional means. The system consists of a dual-function receiver and transmitter. The receiver can be operated in two modes: active and passive. In the passive mode the receiver detects the presence of underground utilities that are energized by a 60-Hertz or other known frequency. These utilities can either be those actually carrying electrical power or those utilities that are both metallic and sufficiently close to the electrical lines to have an electrical field induced within them.
In active mode, the EUL directly connects to a utility and induces a current to create a detectable electromagnetic field. The frequency of this field can be varied from 512 Hertz to 200 kilohertz. This method is highly effective for locating metallic utilities, such as steel pipes, metallic conduits, and cables. However, it cannot detect non-metallic utilities, such as PVC, concrete, clay, or HDPE pipes, unless a metallic tracer wire is present. When a utility transitions from a metallic to a non-metallic material, the EUL will only detect the utility up to the transition point. Furthermore, non-metallic pipelines equipped with metallic tracer wires, like some gas lines, may become undetectable if the tracer wire is broken, corroded, or discontinuous.
The EUL method also faces challenges in complex utility networks. In densely populated utility corridors, overlapping electromagnetic signals can cause interference, resulting in inaccurate or ambiguous utility locations. Multiple metallic utilities in close proximity may create false or ghost signals, complicating the interpretation of subsurface conditions and increasing the risk of misidentification. Depths of underground utilities can be determined when the EM equipment is being used in the active mode. Depths are typically accurate to within 10-15 percent of the total utility depth.
