Re: Proposal for Annual Groundwater Monitoring and SVE System Removal and Demolition Haines City Quality Cleaners 511 Haines City Mall Haines City, Polk County, Florida

ERIC_5357

Dear Ms. Uribe:

Professional Service Industries, Inc. (PSI), an Intertek Company, is pleased to provide this proposal for conducting a groundwater monitoring event and removal and demolition of the existing SVE system at the Haines City Quality Cleaners, located at 511 Haines City Mall in Haines City, Polk County, Florida.

Included with this proposal is a brief summary of the project background, proposed scope of work, cost estimate and a proposed schedule for completion. A Site Access form signed by the property owner in April 2016 is on file with the Department stating that the Department and its contractors (which includes PSI) is allowed to enter the property and conduct a variety of activities including assessment, treatment of contamination, and monitoring tasks. The current site owner on the Polk County property appraiser website on March 7, 2025 is listed as “Haines City Mall LLC” whom acquired the property in September 2015. A Site Access Agreement was obtained from the owner in April of 2016 and remains current for the Haines City Quality Cleaners site.

PROJECT BACKGROUND

The Haines City Quality Cleaners operated at the current tenant space inside the Haines City Mall starting in 1984. The name of the cleaners at that time was Smith Cleaners who has since moved to a separate location in the mall and reportedly within the footprint of the current Publix supermarket.

The name of the drycleaners was changed to Haines City Quality Cleaners in August 1985, which operated until July 1997 when the drycleaning business moved across the street. The tenant space that is designated as Haines City Quality Cleaners in the Drycleaning Solvent Cleanup Program is Suite 1F of the

FDEP – Haines City Quality Cleaners

October 2, 2025 www.intertek.com/building

Haines City Mall which is currently occupied by an Enterprise Car Rental facility which has been in operation at this location since September 1998.

The FDEP tasked AECOM in 2016 to perform site assessment activities to identify the potential sources of drycleaning solvents that may impact the soil and groundwater quality, delineate the horizontal and vertical extents of impacts, characterize site lithology, and determine site specific aquifer characteristics to assist with transport modeling and possible remedial design. The assessment was performed between February 2017 and July 2018. The soil assessment indicated that no dry-cleaning chlorinated contaminants of concern were detected above SCTLs in any soil samples collected.

Groundwater assessment indicated chlorinated compounds above Groundwater Cleanup Target Levels (GCTLs) in several depth intervals as measured with direct push technology and a mobile lab. Eleven monitor wells were installed to delineate the groundwater impact from the former drycleaner which were installed to depths of 15, 35, and 45 feet bls and one deep monitor well (MW009) was installed to 75 feet bls.

A Modified Active Gas Sampling Study (MAGS) was also conducted as a part of the site assessment which revealed relatively low levels of PCE in the soil. Based on the MAGS tests, the site was suitable for soil vapor extraction technology to address the on-site contaminants. An Interim Source Removal Plan (ISRP) was prepared and submitted by AECOM in May 2019 to address the VOHs in the vadose zone.

The SVE system was constructed from June to July 2021 and started up on September 10, 2021. Due to contractor change, PSI took over the Operations and Maintenance (O&M) of the SVE system in January 2022. System modifications were initiated in May 2022 to re-install the SVE lines underground behind the facility due to construction of a car wash space for rental cars initiated by the tenant, Enterprise Car Rental.

A soil vapor system shutdown report was approved by the FDEP in August 2023. The SVE system has been inactive since August 2023 but remains on-site currently.

PROPOSED SCOPE OF WORK

The proposal contained herein consists of conducting annual groundwater monitoring consisting of groundwater sampling at select monitoring wells for VOH analysis via EPA Method 8260, along with along with the removal and demolition of the remaining SVE system. The results will be used to confirm current groundwater quality conditions. The sampling procedures will be conducted in accordance with the FDEP Standard Operating Procedures 001/01 and Drycleaning Solvent Cleanup Program protocols. Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets.

October 2, 2025 www.intertek.com/building

2. Annual Groundwater Monitoring

Specific components of the annual groundwater monitoring subtask are as follows:

a. Mobilization to the site.

b. Collect groundwater elevation measurements from eleven (11) monitoring wells.

c. Collect groundwater samples from

i. two shallow wells: MW002, MW004

ii. three intermediate wells: MW001, MW005, MW008

iii. one deep well: MW009

d. Submit groundwater samples from six (6) sampled wells to a National Environmental Laboratory Accreditation Conference (NELAC) approved laboratory for analysis of VOHs via EPA Method 8260. It is our understanding that the Waste Site Cleanup Section no longer requires QA blanks or duplicates.

e. Containerize investigative derived waste (IDW) in an existing, on-site, 55-gallon drum, and stage on-site, pending waste characterization results.

f. Sample IDW for waste characterization analysis.

g. A subcontractor will transport and dispose of IDW in an approved facility. A signed manifest will be provided by the waste management company after pick-up and disposal.

h. A groundwater monitoring report will be developed which will include groundwater elevation maps, groundwater quality data in tables, and conclusions and recommendations. As we understand, an ADaPT/GIS data package will no longer be required.

3. Removal and Transport of Remedial System and Disposal

Removal and disposal of the SVE system will be accomplished by completing the following tasks as described:

a) Remove & dispose of system compound wood fencing (posts to be cut flush).

b) Disconnect, transport and disposal of SVE system, provide disposal manifest upon final disposal.

c) Disconnect electrical from trailer, pull electrical leads from trailer to disconnect. Pull leads from meter/disconnect to pad mounted transformer.

October 2, 2025 www.intertek.com/building

d) Cut underground conduit and conveyance pipes level with grade and cap with asphalt or like material

e) Cut system tie-downs and cut anchor points level with grade.

f) Cut disconnect/meter conduits/stands etc. and dispose (no pole).

g) Grout-fill associated SVE wells (four wells, each being 4” diameter, 8 feet deep)

Quotes for potential subcontractor demolition, well abandonment, system removal, and transport/disposal are attached.

The deliverable and invoice schedule are presented below and are based on receival of a Task Assignment prior to October 30, 2025. The last groundwater sampling event occurred in October 2024.
