About this file

This document is a Task Assignment Change Order (TACO #1) issued by the State of Florida Department of Environmental Protection (FDEP) to Professional Service Industries, Inc. (Intertek-PSI) for environmental services at the Pure Cleaners site (ERIC_5494) located at 954 West State Road 434 in Longwood, Seminole County. The contract is identified as HW686, with Task Assignment number DC005C. The change order was signed on December 2, 2024, and involves modifications to the original task assignment dated August 29, 2024.

The TACO authorizes PSI to conduct four quarters of groundwater monitoring and two groundwater monitoring reports consisting of volatile organic hydrocarbon (VOH) data and per- and polyfluoroalkyl substances (PFAS) data. The change order reduces one VOH sample from Subtasks 3, 4, and 5 while adding PFAS monitoring labor and materials under Subtask 7. The amended total task value is $30,619.21, representing an increase of $2,707.82 from the original task amount of $27,911.39. The task completion date was extended from December 31, 2025, to January 17, 2026, adding 17 days. Deliverables include quarterly groundwater sampling reports with due dates ranging from December 2, 2024, through October 31, 2025, with invoicing contingent upon FDEP approval of laboratory data, field notes, and summary reports. The contract operates under a fixed unit rate structure with invoice amounts ranging from $3,579.86 to $10,600.35 per deliverable.
