Subject: Negotiation Summary

HW686 DC008A

This Task Assignment authorizes PSI to conduct groundwater monitoring activities in accordance with the April 4, 2022, Scope of Work, and Cost Estimate.

Subtask 1: Project Management

An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Annual Groundwater Monitoring, Soil Monitoring and IDW Treatment

PSI will collect groundwater samples from monitoring wells. Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). Surface water samples and sediment samples from Lake Killarney will be collected and tested. In addition, soil borings will be installed inside the building, and samples collected at 3 depth intervals at 6 different locations selected by the

FDEP. Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum, which will be transported and disposed of at an approved facility.

Subtask 3: Reporting

PSI will submit the Supplemental Assessment Report and ADaPT and GIS deliverables to FDEP.

PSI confirmed on April 4, 2022, that an Access Agreement with the current real property owner of the Fashion Cleaners (ERIC_5015) site has been executed.

The original Task Assignment totaled $17,456.38 with 116 labor hours but after negotiations and corrections with the contractor costs are reflected as below:

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task

Assignment. The final cost for this Task Assignment is $17,363.95 with 114 labor hours.

PSI may invoice following the schedule established in the Deliverables and Invoice Schedule

Attachment, which accompanies the Task Assignment Notification Form.

Re: Proposal for Supplemental Soil Assessment and Annual Groundwater Monitoring Priceless Dry Cleaners 220 North Orlando Avenue Winter Park, Orange County, Florida FDEP Facility ID# 48 9502059 / ERIC_5015

Dear Ms. Murphy:

Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to provide Supplemental Soil Assessment and Annual Groundwater Monitoring services for the Priceless Dry Cleaners at 220 North Orlando Avenue, Winter Park, Florida. Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion.

PROJECT BACKGROUND

The Priceless Dry Cleaners opened in October 1991 and utilized perchloroethene (PCE) as their drycleaning solvent. Based on documented impacts from drycleaning fluids, the facility was accepted into the Florida Drycleaning Solvent Cleanup Program in June 1997, and assessment activities commenced in August 2001.

A Bioremediation Pilot Study was conducted in April 2006 with two injection areas, one in the source area and one downgradient.

In September 2007, a Remedial Action Plan was developed and implemented to address soil contamination through soil vapor extraction. PSI was tasked with providing remediation system start-up on January 31, 2011 and the system ran until April 2012 when the motor seized and was not repaired.

The system was subsequently transferred to the Department’s Lakeland warehouse for storage.

Groundwater monitoring has occurred generally on an annual basis which continues to reveal a groundwater plume migrating to the southwest and off-site. In 2014, groundwater monitoring was mailto:Skylar.L.Murphy@FloridaDEP.gov

PSI Proposal Number: 0552-370178-Rev 1 FDEP – Priceless Cleaners

March 31, 2022, Rev April 4, 2022 www.intertek.com/building deferred until the adjacent property was re-developed. The southwest adjacent property was redeveloped as a shopping center in 2014 with the grocery store, Trader Joes, as the anchor tenant.

Subsequent to the adjacent property reconfiguration, PSI provided oversight for the installation of a 46-foot deep monitor well (MW019) on the Trader Joes property in July 2015. The monitor well was installed as a replacement for MW006, MW011 and MW016, which were destroyed during site redevelopment.

Results of the annual groundwater monitoring program reveals a narrow contaminant plume migrating towards Lake Killarney. Results from 2017 also indicated vertical migration of PCE near MW015, located on-site at the former Priceless Cleaner.

The tenant apparently moved in 2019 and it was considered an opportune time to collect additional data inside the former drycleaner. Therefore, in July and August 2019, Supplemental Assessment was conducted which consisted of soil confirmation samples and a MAGS study. Results revealed remaining impacts to the soil. An Interim Source Removal Plan was prepared to address the remaining soil impact through a source removal. However, the implementation of the plan was put on hold pending the owner demolishing the building. The property owner’s representative has notified the FDEP of the demolition schedule which is planned for late April 2022. The FDEP has requested additional soil sampling to further refine the soil “cut lines”. This information will then be used to revise the Interim Source Removal Plan.

PSI has, therefore, developed the proposal contained herein to provide additional soil sampling as well as to continue the groundwater monitoring program. The recommended network is based on previous results and in accordance with our most recent groundwater report. PSI also recommends collecting surface water and sediment sampling at Lake Killarney located approximately 500 west of the former drycleaner. In previous annual events, we have included the surface water and sediment sampling to monitor the lake as it is considered a potential receptor.

PROPOSED SCOPE OF WORK

The proposal contained herein consists of conducting soil, surface water, sediment, and groundwater sampling and analysis, along with a report deliverable. Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets.

2. Supplemental Assessment

a. Mobilization to the site.

b. Collect groundwater samples and groundwater elevation measurements from the following wells:

i. Shallow wells: MW014 and MW017,

ii. Intermediate wells: MW004, MW007, MW008, MW009, MW015, MW018 and

MW019,

iii. Deep wells: MW005.

March 31, 2022, Rev April 4, 2022 www.intertek.com/building

c. Collect surface water samples and sediment samples from Lake Killarney located approximately 500 feet to the west. Two surface water samples will be collected; at the surface and approximately 2 to 3 feet below the surface. Two sediment samples will also be collected; at the surface and about 2 to 3 feet below the sediment surface.

d. Soil borings will be installed inside the building and samples collected at 3 depth intervals at 6 different locations selected by the FDEP (see attached site sketch prepared by FDEP engineer):

i. 0 - 0.5 feet bls

ii. 0.5 – 2 feet bls

iii. 2 – 4 feet bls

e. Collected samples will be laboratory analyzed for Volatile Organic Halocarbons (VOH) by

EPA Method 8260. The analysis will be performed using a National Environmental Laboratory Accreditation Conference (NELAC) approved laboratory.

f. The investigative derived waste (IDW) will be containerized in a 55-gallon drum. Once characterization of the IDW has been performed, the drum will be transported off the site to an approved facility. A copy of the manifest will be provided to the Department upon receipt.

3. Supplemental Assessment Report

A Supplemental Assessment Report will be developed which will include current and historical analytical data, with a comparison to state standards and consist of current groundwater elevation and quality data, development of ADaPT package, and recommendations. Conclusions and recommendations will be made, which will include any modifications to the proposed annual groundwater sampling event.

March 31, 2022, Rev April 4, 2022 www.intertek.com/building

COST ESTIMATE AND SCHEDULE

The estimated cost to perform the Supplemental Site Assessment is shown on the attached Cost Estimate and Level of Effort with the report deliverable. All PSI services will be conducted in accordance with the terms and conditions contained within DEP Contract No. HW 686, Waste Cleanup Program Services, dated December 21, 2021. We understand that the assessment is time critical in order to allow the owner to proceed quickly with the planned building demolition and for PSI to proceed with the interim source removal. For planning purposes, the table below outlines our anticipate schedule.

Good Morning Natacha – Hope you are doing well these days.

The PSI Team (PSI and Chastain Skillman) have been awarded one of the FDEP “Drycleaning Contracts” and we are anxious to get started.

We are inviting your firm to provide costs for pick-up, transport, and disposal to an approved facility of Investigation- Derived Waste (IDW) on a per 55-gallon drum basis from drycleaning sites and state funded sites, and site investigation sites that are in the programs.

If you are interested in providing a quote, we would appreciate it. I am attaching a form in which to submit back to us, along with the General Conditions.

If you could provide these prices to us by January 21, 2022, we would appreciate it.

Thank you, Nana
