Re: Proposal for Initial Site Assessment Activities – Rev 4 Clayton Plaza Cleaners 559 West Brandon Boulevard Brandon, Hillsborough County, Florida, 33511

ERIC_4751

Dear Ms. Uribe:

Professional Service Industries, Inc. (PSI), an Intertek Company, is pleased to provide this proposal for conducting initial site assessment activities at the former “Clayton Plaza Cleaners” located at 559 West Brandon Blvd. in Brandon, Hillsborough County, Florida, 33511. Included with this proposal is a brief summary of the project background to date, proposed scope of work, cost estimate and a proposed schedule for completion. A Site Access form was signed by the property owner “Clayton Ventures, LLC” in March 2024.

PROJECT BACKGROUND

The shopping mall containing the former Clayton Plaza Cleaners was constructed in 1967 and drycleaning operations became active around the same time. This drycleaning location was occupied by several different drycleaning tenants during its lifetime but was renovated to a furniture rental store (Rent King) around 2018 and has been occupied by that tenant since that time.

Initial site screening was conducted in 1996 which identified a soil sample which detected 0.033 mg/kg tetrachloroethene at ~6.5 feet below land surface, around the area of the dry clean machine. In October 1996, FDEP determined the site was eligible for the Drycleaning Solvent Cleaning Program with a site score of 65. A Rapid Risk Screening was performed at the site in November 2011 which identified non-residential tenants as potential receptors. The most recent potable well survey was conducted in 2018 which did not identify any small potable or Potable Water Supply (PWS) wells within ½ mile of the former drycleaner.

FDEP – Clayton Plaza Cleaners

Revised December 10, 2025 www.intertek.com/building

FDEP approved funding to begin active site remediation in 2024 and Intertek-PSI performed initial site reconnaissance in June 2024 and issued an initial Site Assessment Work Plan (SAWP) to begin Assessment in September 2024.

PROPOSED SCOPE OF WORK

The proposal contained herein consists of conducting initial site assessment related work including drilling, sampling and well installation activities, along with generating a report deliverable. The results will be used to determine current soil, soil vapor, and groundwater impacts related to the former drycleaning operations on this site. The assessment and sampling procedures will be conducted according to the FDEP Standard Operating Procedures and Drycleaning Solvent Cleanup Program protocols.

Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets.

2. First Mobilization Geoprobe (Direct Push): GPR utility location; Lithologic Boring, Soil Sampling, DPT Screen point Sampling; Monitoring Well Installation; Hand Auger MAGS Well Installation and MAGS Test; Survey Top of Casing elevation and locations.

a. Obtain groundwater well installation permits for the groundwater and piezometer wells proposed in Figure 1.

b. Obtain a standard utility locate via Sunshine 811 for all locations; verify all soil locations using a GPR subcontractor.

c. Coordinate with the tenant/owner to provide site access.

d. PSI will subcontract drilling services in order to accomplish all drilling related activities.

e. Perform one (1) continuous lithologic profile boring up to 50 feet below land surface (ft-bls) or refusal depth with a DPT drill rig. Assess and classify soils and complete the boring with grout to the surface.

f. Advance two (2) exterior soil borings to the water table (after hand auger clearance) with a DPT drill rig. Classify soils and collect discrete analytical samples at standard depth intervals (0-0.5 ft-bls, 0.5-2 ft bls, 2-4 ft-bls, 4-6 ft-bls, 6-8 ft-bls, 8-10 ft-bls, 10-15 ft-bls, and 15-20 ft-bls). Analytical soil samples will be collected to approximately ~1 ft above the water table. Soil analytical will include the full list of volatile organic compounds (VOCs) via EPA Method 8260 Full, and contingent leachable VOC’s via SPLP analysis. This proposal assumes no more than 10% of the soil samples collected will need SPLP analysis provided.

g. Submit soil samples to a NELAP certified laboratory.

h. Complete two (2) exterior soil borings as MAGS wells as specified by the SAWP.

i. Conduct groundwater screenpoint sampling using a DPT drill rig equipped with a screenpoint groundwater sampler at up to eight (8) locations. Collect groundwater samples from each screen interval (anticipated as ~4 intervals) and submit to a NELAP certified lab for analysis of VOC’s via EPA Method 8260 Full.

Revised December 10, 2025 www.intertek.com/building

j. Use a DPT drill rig (once utilities are cleared with a hand auger) to install up to 6 shallow shallow wells will be constructed and 1 deep monitoring well.

k. Develop all wells until discharge water is visually clear of sediment.

l. Containerize investigative derived waste (IDW) in 55-gallon drum, label appropriately and stage on-site, pending waste characterization results.

m. Install three (3) interior soil borings using a core drill and hand auger to clear utilities.

Classify soils and collect analytical samples at standard depth intervals (0-0.5 ft-bls, 0.5-2 ft bls, 2-4 ft-bls, 4-6 ft-bls, 6-8 ft-bls, 8-10 ft-bls, and 10-15 ft-bls). Analytical soil samples will be collected to approximately ~1 ft above the water table. Soil analytical will include the full list of VOCs via EPA Method 8260 Full, and contingent leachable VOC’s via SPLP analysis. This proposal assumes no more than 10% of the soil samples collected will need SPLP analysis provided.

n. Complete two (2) of the interior soil borings as MAGS wells.

o. Survey and collect groundwater elevation measurements of all 7 monitoring/piezometer wells:

p. Containerize investigative derived waste (IDW) in 55-gallon drums, label appropriately and stage on-site, pending waste characterization results.

q. Conduct a MAGS test and collect air samples at each MAGS well head for analysis by EPA

Method TO-15.

3. Second Mobilization: Monitoring Well Sampling / IDW Disposal /Update SAWP Figures

a. Coordinate with the tenant/owner to provide site access.

b. Mobilization to the site during each day of sampling activities.

c. Collect groundwater samples from up to 7 monitoring wells for Full List VOC’s via EPA

Method 8260.

d. Submit groundwater samples to a National Environmental Laboratory Accreditation

Conference (NELAC) approved laboratory for analysis of VOCs via EPA Method 8260. No blanks or duplicates will be included per the Waste Site Cleanup Section.

e. See Subtask 5 for PFAS analysis of groundwater samples collected in this Subtask.

f. Containerize investigative derived waste (IDW) in 55-gallon drum, label appropriately and stage on-site, pending waste characterization results.

g. A subcontractor will transport and dispose of IDW at an approved facility. A signed manifest will be provided by the waste management company prior to pick-up and disposal.

h. Update SAWP maps with all monitoring well and MAGs locations.

4. Reporting: Site Assessment Report

a. A Site Assessment Report will be developed which will include figures, tables, and charts developed to document and summarize soil characteristics, groundwater field parameters, and analytical data for analyzed Constituents of Concern. The report will document all assessment activities and results as approved and performed in accordance

Revised December 10, 2025 www.intertek.com/building with the Task Assignment. Included with the report will be conclusions and recommendations as provided by a Professional Engineer licensed in the State of Florida.

5. PFAS Groundwater Sampling and Analysis, and Summary Deliverable

PFAS groundwater sampling will be conducted concurrently with the VOH sampling but will be addressed through Subtask 5 and include:

a. Collection of four (4) groundwater samples from permanently installed monitoring wells for PFAS analysis.

b. Preparation of sample coolers and ship to the FDEP Central Laboratory in Tallahassee for analyses for PFAS analyses.

c. Containerize purge water into 55-gallon drum(s) as necessary, label appropriately, and upon waste characterization, have transported to an approved facility for treatment. The manifest will be submitted to the Department upon receipt.

RE: Utility Locating Services – Clayton Plaza Cleaners in Brandon, Florida

Mr. Coleman, GeoTek Services is pleased to respond to your request for Utility Locating services for Clayton Plaza Cleaners located in Brandon, Florida. We will scan a 10’ x 10’ area around the twenty proposed bore locations with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, or vehicles. We will scan as close as reasonably possible to fixed objects such as curbs, vehicles, fences, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our utility locating quote for this project is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for ninety days.

SCOPE OF WORK

Soil Boring: We understand the scope of work on this project is to search for underground utilities in a 10’ radius around 20 soil bores. We will attempt to trace any utilities for which structures are visible from the work area. Utilities will be marked on the surface using paint, pin flags, or other appropriate means. The client will be responsible for providing drawings or notifying GPRS of any utilities entering the work area for which there are no apparent surface features or structures visible from the work area. To avoid additional charges, the areas should be laid out, marked, and cleared of obstructions before our arrival. Please visit www.simspec.org for an overview of our industry-leading best practices that will be applied to this project.

• GPRS is not responsible for housekeeping. Any debris, equipment, or other obstructions in the area at the time of scanning could potentially block out needed data.

• A thorough utility search can only be completed if GPRS is given access to all utility structures, interior and exterior. This service is never a replacement for the use of the state One Call system (811).

• All of our technicians have OSHA-10 safety training or greater. Site-specific safety training is not included in this quote. Please notify us if this project requires additional safety training.

EQUIPMENT

• Underground Scanning GPR Antenna. This GPR Antenna uses frequencies ranging from 250 MHz to 450 MHz and is mounted in a stroller frame that rolls over the surface. Data is displayed on a screen and marked in the field in real-time. The surface needs to be reasonably smooth and unobstructed to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the efficacy of GPR.

The total effective scan depth can be as much as 8’ or more with this antenna but can vary widely depending on the soil conditions and composition. Some soil types, such as clay, may limit maximum depths to 3’ or less. As depth increases, targets must be larger to be detected, and non-metallic targets can be challenging to locate. The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

• Electromagnetic Pipe Locator. This receiver can passively detect the signals from live AC power or radio signals traveling along some conductive utilities. Operators can connect a transmitter directly to accessible metallic pipes, risers, or tracer wires to generate a current at a specific frequency. The receiver can then detect the resulting signal along the pipe or tracer wire. Various factors may impact this device's effectiveness, including (but not limited to) access to the utility, conductivity, grounding, and interference from other utilities.

The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

• Traceable Rodder. The rodder consists of a copper wire encased in fiberglass. This device is pushed through a pipe with direct access, such as a sewer line at a cleanout or a storm drain catch basin. Operators then induce a current on the wire and trace the signal from the surface. The maximum traceable depth is 10’ depending on the soil conditions, and the maximum distance is 200’. Inserting the rodder into deeper pipes within manholes may not be feasible depending on site conditions. GPRS will not access electrical conduits. The signal is not detectable through metallic pipes. For more information, please visit: Link

• GPS. This handheld unit offers accuracy down to 4 inches; however, the accuracy achieved will depend on the satellite environment at the time of collection and is not considered survey-grade. Features can be collected as points, lines, or areas and then exported as a KML/ KMZ or overlaid on a CAD drawing. For more information, please visit: Link
