About this file
This document is a Task Assignment Change Order (TACO) between the State of Florida Department of Environmental Protection (FDEP) and Professional Service Industries, Inc. (Intertek-PSI) for environmental remediation services at the Westwood Cleaners site in Tampa, Hillsborough County, Florida. The change order modifies the original Task Assignment DC004A under Contract HW686, with FDEP Project Manager Skylar Schwed and contractor Project Manager Nana G. Westmark designated as primary contacts.
The change order authorizes a reduction in total task assignment cost from $58,585.68 to $56,164.04, representing a cost decrease of $2,421.64. This reduction reflects the removal of costs associated with Subtasks 6 and 7 (4th Quarter Monthly O&M and RASS activities) due to unforeseen SVE system malfunctions that prevented completion of these subtasks. The task includes quarterly operation and maintenance activities, groundwater monitoring, utilities (cost-plus fixed fee capped at $12,840 with a $480 handling fee), and system troubleshooting and equipment repair. The completion date was extended from August 31, 2023 to September 29, 2023, providing an additional 29 days. Invoice requirements align with quarterly deliverables and equipment repair documentation, with three primary invoicing milestones totaling the final contract value of $56,164.04. The utilities component remains the only cost-reimbursement element; all other services operate on a fixed unit rate basis.
