About this file
This document set comprises a Task Assignment (TA) and a Task Assignment Change Order (TACO) Number 1 between the State of Florida Department of Environmental Protection (FDEP) and Professional Service Industries, Inc. (Intertek-PSI) for environmental remediation services at Pugh's Cleaners, a dry cleaning facility located in Eustis, Lake County, Florida. The work is conducted under Contract Number HW686, the FDEP Waste Cleanup Program Services contract dated December 21, 2021. The primary contact for FDEP is Mary Uribe (Project Manager, 850-245-8990), and for the contractor is Joshua Newell (Project Manager, 863-712-4342), with Douglas Coleman serving as the contractor's Contract Manager.
The scope of work consists of preparing a Soil Vapor Extraction (SVE) Interim Source Removal Plan Modification for the Pugh's Cleaners site, which has been impacted by tetrachloroethene (PCE) contamination from historical dry cleaning operations. The deliverable—Subtasks 1 & 2: SVE ISRP Modification—is due May 31, 2026, with a task completion date of July 17, 2026 (as extended by TACO 1). The fixed unit rate contract value is $6,710.38, with no changes to compensation in the change order; the TACO provides only a time extension of 78 days from the original completion date of April 30, 2026. Invoice requirements specify that the full amount of $6,710.38 is payable upon FDEP approval of the SVE ISRP Modification. The cost estimate reflects 45 hours of labor across multiple engineering and support staff, vehicle usage, equipment, and expendables, with no subcontractor costs or laboratory expenses included.

Re: Proposal for an SVE Interim Source Removal Plan Modification Pugh’s Cleaners ERIC_4814 215 South Bay Street Eustis, Lake County, Florida
Dear Ms. Uribe:
Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to provide a Soil Vapor Extraction Interim Source Removal Plan Modification with details and specifications for the addition of active and passive venting via air injection to the existing Soil Vapor Extraction system to enhance remedial efforts associated with soils impacted with Volatile Organic Halocarbons at the Pugh’s Cleaners site located in Eustis, Lake County, Florida. Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed work schedule. A Site Access Agreement was signed by South Bay Propety Holdings LLC in March 2023 and as of March 5, 2025, South Bay Property Holdings LLC is still the owner of the property per the Lake County Property Appraiser’s website.
FDEP – Quality Cleaners ERIC_4771 PSI Proposal 0552-446333r
December 2, 2025 www.intertek.com/building
PROJECT BACKGROUND
Dry cleaning operations at Pugh’s Dry Cleaners began in 1948 and utilized a petroleum-based solvent. In 1965, Pugh’s switched to tetrachloroethene (PCE) as the drycleaning solvent, which was stored in an AST located inside the building. Based on documented impacts from the drycleaner, the facility was accepted into the Florida Drycleaning Solvent Cleanup Program in August 1998, and assessment activities by others commenced in December 2002. The assessment results revealed impact at levels requiring remediation and a soil vapor extraction (SVE) system was implemented. PSI took over running the SVE system in 2011 due to contractor turnover.
The Department then requested that the SVE system be shut down due to the drycleaner’s continued use of PCE.
The property changed ownership in 2016 and switched from PCE to a non-chlorinated solvent. The SVE system was re-started in October 2016 and is currently operating. The groundwater remains impacted with PCE at elevated concentrations.
A groundwater remediation strategy was designed incorporating In Situ Chemical Oxidation to reduce concentrations of COCs in groundwater. The remediation activities were implemented with a Regenesis product Persulfox which was applied in 22 injection wells (screened intervals 30 – 40 feet, and 58 – 68 feet). Two separate injection activities were conducted, one in September 2020 and the second injection in December 2020. Post injection monitoring is continuing with some groundwater contaminant rebound occurring.
The drycleaning business has relocated and the building is currently being used as a drop-off only. This was confirmed in September 2023 and in January 2024. Based on the tenant space being vacant, we conducted additional soil sampling inside the building to potentially conduct an interim source removal inside. The FDEP Waste Site Cleanup Section has also recommended we proceed with this course of action. The soil confirmation sampling was recently completed (February 2024) and a report is being developed to present the results.
PSI developed an Interim Source Removal Plan for excavation of soils in February 2025 however after discussions with the FDEP and the complexity of excavation activities within the restroom area, structural evaluation and potential requirement for additional support of load bearing walls, the FDEP has opted not to proceed with interior excavations. Based on the current radial placement of existing SVE wells, location of soils with commercial/industrial SCTL exceedances, and in lieu of excavation activities, PSI recommends installation of additional SVE wells within the area exhibiting the greatest VOH impacts and installation of a 1 horsepower blower for use in active venting. SVE wells should be installed with the intent to be used for active/passive venting and SVE operation to allow the system to push/pull soil vapors while mitigating preferential pathways.
PSI proposes modification of the existing SVE Interim Source Removal Plan to enhance VOH remediation activities in the vadose zone. PSI has developed this cost estimate and level of effort to modify the existing plan.

PROPOSED SCOPE OF WORK
This proposal consists of modifying the existing SVE Interim Source Removal Plan at the referenced site.
Specifically, the proposed scope of work consists of the following subtasks:
1. Project Management
This scope is provided for in the template worksheets.
2. Interim Source Removal Plan (ISR Plan)
The SVE ISRP Modification will enhance soil vapor exchange within the area of greatest VOH impacts and will not directly address groundwater quality. The presumptive remedy for remediation of impacted soil at Pugh’s Cleaners site is soil vapor extraction, based on previous experience, feasibility, implementability, reliability, operations and maintenance (O&M) requirements, and cost.
Calculations will be generated to determine the design criteria and equipment selection for an air injection blower/compressor. Updated figures will include piping and instrumentation diagram (P&ID) and manifold detail with tie in locations and equipment placement.
Due to the change in operation and enhanced recovery efforts, the report will include operation parameters, proposed soil vapor sampling and analysis, treatment considerations and itemized cost estimates for project capital, and the implementation schedule. The design will be signed and sealed by a Florida Registered Engineer in accordance with Chapters 471, Florida Statues. The report will incorporate specific recommendations and provide opinions based on professional experience in order to achieve the most efficient endpoints. We understand that the Department’s goal is to achieve aggressive contaminant reduction in an efficient and cost-effective method.
The deliverable and invoice schedule are presented below and are based on receipt of a Task Assignment prior to December 15, 2025.

COST ESTIMATE AND SCHEDULE
The estimated cost to prepare the SVE ISRP Modification is shown on the attached Cost Estimate and Level of Effort. All PSI services will be conducted in accordance with the terms and conditions contained within the terms and conditions contained within DEP Contract No. HW686, Waste Cleanup Program Services, dated December 21, 2021. An invoice and deliverable schedule is presented as follows.
