About this file
This is a Best and Final Offer (BAFO) letter from the Arizona Department of Health Services (ADHS) to Advantage Staffing Services, LLC regarding Registry Staffing Services for the Arizona State Hospital. The letter, dated August 11, 2025, requests a third and final revised proposal to address deficiencies identified in the vendor's previous submissions for RFP No. BPM006391. The revised offer must be received by August 14, 2025, at 3:00 PM Arizona Time through the Arizona Procurement Portal (APP). The original solicitation required contractors to provide approximately twenty (20) Behavioral Health Technicians (BHTs) per shift on an as-needed, if-needed basis across all shifts to supplement the hospital's existing staffing resources.
The BAFO letter identifies specific deficiencies requiring correction: (1) reconciliation of pricing between Attachment H - Billable Rate Breakdown per Amendment One and the Items (F) Tab in the Arizona Procurement Portal, which must match in their entirety; (2) provision of an Employee Summary Sheet for each new staff member; (3) designation and certification of a CPI Trainer; (4) submission of shift differential rates in Attachment H; (5) inclusion of a conversion fee policy for permanent hiring; and (6) explicit confirmation that complete candidate profiles containing resume, CPR certification, CPI Blue Card, diploma, fingerprint card, and TB screening will be submitted for each individual. If the vendor does not submit a revised offer by the deadline, their immediate previous written offer will be accepted as their final offer per Arizona Administrative Code.

Hospital
Dear Supplier, In accordance with the Arizona Procurement Code, specifically Arizona Revised Statutes (A.R.S.) § 41- 2534 and Arizona Administrative Code (A.A.C.) R2-7-C315, Offer Revisions and Best and Final Offers, the Arizona Department of Health Services (ADHS) is requesting a third and final Best and Final Offer
(BAFO).
Following a review of your previous submissions, ADHS has determined that several requirements of the solicitation have not been fully addressed. This BAFO provides you with an opportunity to revise your proposal to correct the deficiencies identified below. A failure to substantially meet these mandatory requirements may affect your overall score, per the evaluation criteria.
The following items require your attention:
Pricing:
In order to continue the evaluation of your offer, both Attachment H – Billable Rate Breakdown per Amendment One (1) AND the Items (F) Tab in the Arizona Procurement Portal (APP) MUST match in their entirety. The discrepancy between these documents must be resolved for the evaluation to proceed.
Method of Approach (Section 5.3.1.3)
• Not Addressed - 4.2.2 (Employee Summary Sheet): The proposal confirms 24/7 availability with a one-hour response time. However, it does not mention providing an "Employee Summary Sheet" for each new staff member sent to the facility.
• Not Addressed - 4.8.1 (CPI Trainer): The proposal states that staff will receive CPI training, but it does not address the recommendation to designate and certify an employee as a CPI Trainer.
• Not Addressed - 4.10 (Shift Differential): The proposal mentions a shift differential policy and references an "Attachment H - Billable Rate Breakdown," but this mailto:sjenkins@advantagestaffing.co
