About this file

This is a Final Asbestos Abatement Report Template for Ellington Field Joint Reserve Base, a facility located at 14657 Sneider Street in Houston, Texas 77034, prepared in compliance with Texas Department of State Health Services (TDSHS) regulations and federal Environmental Protection Agency (EPA) standards. The template documents the completion of asbestos abatement work, including the removal of approximately 1,350 square feet of asbestos-containing floor tile and mastic. The report captures project duration, air monitoring results, and compliance with Texas Health and Human Services operational codes under Chapter 1954-Asbestos Health Protection, Texas Administrative Code Title 25 Part 1 Chapter 296, and Federal Code of Regulations Title 40 Chapter 1 Part 61-National Emission Standards for Hazardous Air Pollutants (NESHAP). Required documentation includes TDSHS notifications, air sample results analyzed using Phase Contrast Microscopy (PCM) and NIOSH Method 7400 Revision 3, chain of custody forms, air monitoring field forms, daily project notes, TDSHS certifications and licenses for prime and subcontractors, before and after photographs, and signed waste manifests.

The report confirms that all area air samples collected outside the work area were at or below the threshold value of 0.01 fibers per cubic centimeter (f/cc) or below the limit of detection, and all personal samples remained below the OSHA Permissible Exposure Limit for both Short Term Exposure (STEL) of 1.0 f/cc and Time Weighted Average (TWA) limit of 0.1 f/cc. The abatement contractor holds a TDSHS license and EPA waste transporter identification, with materials manifested and transported to an approved landfill in Houston, Texas. A TDSHS-licensed asbestos air monitoring technician conducted continuous monitoring during work performance, final visual inspection for cleanliness, and daily air monitoring throughout the abatement project. The template is designed to be completed by the prime contractor and reviewed by a TDSHS Asbestos Consultant, with all supporting documentation included as appendices.
