• Removal and disposal of conductors between the main disconnect and trailer (meter can has already been removed and the main disconnect is currently de-energized).

• Removal of any existing connections to anchors to the trailer (anchors to remain in place).

• The trailer does not have a tag so a lowboy/flatbed will be required.

• A weight ticket from the end disposal facility and invoice matching the quoted amount as the only required deliverables for these services.

• Up to 2 man-hours of onsite labor is requested for clearing of brush (palm tree, Brazilian pepper tree, and other vegetation as photographed last year).

Brush is to be removed with the trailer by the subcontractor.

Assumptions and Exclusions

• All power has been disconnected from the trailer.

• PRM will have full access to the trailer.

• No permitting is anticipated for the completion of these activities

• The chain link fence will remain and the gate opens towards a dumpster located south of the trailer.

• Temporarily move the dumpster out of the way and put it back in place once the trailer is removed.

• The system was last accessed in 2011.

• FDEP owner trailer with the understanding the system will be disposed of

Notes:

The following terms, conditions, and policies govern working with Microbial Insights, Inc. (hereafter "MI"). The submission of samples to MI without a valid contract is considered an acceptance or agreement to these terms. Please send contracts (i.e., purchase orders, task orders, etc.) to contracts@microbe.com for processing.

Failure to read these terms upon receipt of the quote does not exclude the client from compliance with these terms.

Payment Terms

Standard payment terms are Net 30 and apply to all projects except those governed by a contract or purchase order. Under no circumstance will MI accept "paid when paid" or PWP payment terms. Submission of samples to MI without a contract or purchase order in place assumes Net 30 payment terms.

New customers are required to prepay for Lab Services and Specialty Supplies.

If paying via Wire Transfer, please check with your financial institution to determine the fee they will charge you to perform the wire transfer. This fee amount must be added to the balance being paid to MI when initiating payment via wire funds transfer to ensure that MI receives proper payment.

Validity and Expiration

The pricing enclosed in this quote is valid for 60 days. The pricing of a quote may be ensured for an extended period of time with a contract or purchase order. Failure to procure a valid quote within 60 days of submitting samples to MI does not entitle the client to outdated pricing.

In order to receive the quoted pricing, the project must be invoiced to the company to whom the quote was issued.

Sampling Supplies - Shipping And Fees

MI can provide sampling supplies upon request at no additional charge. All sampling supplies will be shipped using ground service and we request sufficient lead time (7-10 days) for ground shipping. Sampling supplies may include but are not limited to BioFlo filters, 1L poly bottles, jars. swab sampling kits, and Whirl-Pak bags. Along with sampling supplies, MI can provide sampling protocols, chain of custody (COC) forms, sample labels, and a cooler(s).

Return shipping is not included in the quote and due to the short hold time of most analysis (24-48 hours), MI recommends shipping the samples to our lab with plenty of ice using an overnight service such as FedEx priority overnight.

Groundwater samples submitted using 1L bottles instead of BioFlo filters, will incur a $50 per sample filtration and waste fee in addition to the price of analysis.

Sample Cancellation Policy

MI understands that our clients may need to cancel an order or analysis of individual samples due to extenuating circumstances. However, because samples are processed promptly upon receipt, the following cancellation surcharges and fees may apply: There is no charge for samples or analyses canceled within 24 hours of sample receipt. If cancelation occurs from 24 hours to 3 working days following sample receipt, MI will invoice the client for 50% of the analysis price. After 3 working days, full charges will be applied regardless of the cancellation status. If samples are submitted to be held but later canceled, they will incur a $25 per sample processing fee plus any sample filtration fees incurred.

Results and Deliverables

Unless otherwise stated, the price quoted herein includes only the standard MI Level II report (.pdf) and standard MI EDD (.xlsx). Any deviation from the standard report will result in a surcharge. Requests for an MI Level III data package will entail an additional 15% surcharge. Allow an additional 1 - 2 weeks after the Standard Report is completed for the Level III data package to be sent. Requests for an MI Level IV data package will entail an additional 25% surcharge. Allow an additional 1 - 2 weeks after the Standard Report is completed for the Level IV data package to be sent. EDD formats different from the standard MI EDD, if available, will entail a 5% surcharge.

All further inquiries may be directed to the MI Project Manager who delivered this quote or to info@microbe.com.
