About this file

This document is a Task Assignment Change Order (TACO) issued by the State of Florida Department of Environmental Protection (FDEP) to Professional Service Industries, Inc. (Intertek-PSI), modifying an existing task assignment under Contract Number HW686. The change order amends the scope of work for site remediation activities at Pugh's Dry Cleaners (ERIC_4814), located at 215 South Bay Street, Lake County, Florida. The project manager for FDEP is Mary Uribe, and the contractor's project manager is Joshua Newell.

The change order modifies Subtasks 6 and 7 of the original task assignment, which involves site remediation, maintenance and operations, groundwater monitoring, and a Remedial Action Summary and Support (RASS) report. Subtask 6 amendments include measuring the distance from soil vapor extraction (SVE) wells to interior walls. Subtask 7 amendments include generating a new basemap with updated legend, overlaying SVE wells on soil boring figures based on interior measurements, generating a Region of Interest (ROI) figure contrasting with the soil contaminant plume, and adding supporting text. The amended total task value is $88,538.94, with a fixed unit rate of $77,818.94 and cost reimbursement of $10,720.00 for utilities. The task completion date remains July 31, 2025, with the final deliverable due May 30, 2025. All amendments were made at FDEP's request, and PSI's time, labor, and subcontractor costs have been reviewed as reasonable for the scope of services.
