Re: Site Assessment Cost Estimate and Level of Effort – Revision 3 Quality Cleaners ERIC_4771 14837 North Florida Avenue Tampa, Hillsborough County, Florida

Dear Ms. Schwed:

Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to provide a Site Assessment for the Quality Cleaners ERIC_4771, Tampa, Florida.

Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion. According to the property appraiser’s website, the entity who is listed as owner in December 2022 is the same as on the Site Access form signed in May 2022.

PROJECT BACKGROUND

The Quality Cleaners is located at 14837 North Florida Avenue, Tampa, and is currently in use as a restaurant, Shelley’s Café. The site is located in Action Plaza on the east side of Florida Avenue approximately 1,500 feet south of Bearss Avenue in Tampa, Hillsborough County, Florida.

The site was also known as Premier Cleaners at one time. Documents available in OCULUS have used both Quality Cleaners as well as Premier Cleaners at various times. The site is more formally known as ERIC_4771, and for purposes of this report will be called Quality Cleaners. The drycleaning business started in 1983 using PERC, the year the strip plaza was constructed. According to the current owner of the property, all the dry equipment was removed by 2004.

The FDER(P) Site Investigation Section (SIS) conducted a groundwater study in the vicinity of Bearss Avenue and Florida Avenue in 1989 through 1991 to identify source(s) of volatile organic halogens (VOHs) detected in several area private wells.

mailto:Skylar.Schwed@FloridaDEP.gov

FDEP – Quality Cleaners, ERIC_4771

Four monitoring wells were installed by SIS to evaluate Quality Cleaners as a source of VOH groundwater contamination, labeled as NF-27S, NF-28S, NF-1S and NF-1D. Groundwater flow at the time of the SIS study was evaluated to be generally towards the south-southeast. A mounding effect was present at the Action Plaza which could be interpreted as localized radial flow.

The study concluded that VOH concentrations likely originated from Quality Cleaners. The study also concluded that VOHs present in the New Life Church supply well and other private wells located down gradient originated from Quality Cleaners. The study additionally presented documentation that a second VOH plume in the area originated from the McNatt’s Cleaners located in the Bearss Plaza, north north-west of the Quality Cleaners, located upgradient from the Quality Cleaners.

During the site visit in 2022 in preparation for the Site Assessment Work Plan, PSI identified two existing monitoring wells present at Action Plaza (NF-27S and NF-28S). Depth to water in NF-28S was measured in August 2022 at 8.35 feet below top of casing. An attempt was made to locate the monitoring wells (NF- 1S and NF-1D), installed at the New Life Church (currently La Verdad Church) located southeast of Action Plaza, however they could not be located. The water supply well present at La Verdad Church was observed to be out of service. Based on a review of the SIS soil boring logs depth to groundwater is estimated to range from 5 to 8 feet bls.

A Site Assessment Work Plan was submitted by PSI, revised per FDEP suggestions, resubmitted and approved in August 2022, and follows the guidelines presented in the July 2022 FDEP Contractor’s Workshop

PSI, therefore, proposes to conduct the Site Assessment at the Quality Cleaners, as presented below.

PROPOSED SCOPE OF WORK

Subtask 1. Project Management

This scope is provided for in the template worksheets.

Subtask 2. Notification/ Utility Locate / GPR Survey / Land Survey / Existing Monitoring Well

Sampling

Prior to mobilizing field crews, PSI will provide several site assessment preparation elements.

2.1 Notification - The tenant and/or owner of the property will be notified of the planned field activities.

2.2 Utility locates - Utility locates will be called in using 811 Sunshine.

2.3 GPR – A ground penetrating radar (GPR) survey will be performed to determine the presence and location of subsurface utilities leading up to the building.

2.4 Land Survey – A Specific Purpose Survey with locations of improvements will be performed by a

Florida Licensed Surveyor and Mapper. T

2.5 Existing Well Sampling - Two existing monitoring wells, NF-27S and NF-28S, will be sampled and analyzed for VOHs and VOAs by EPA Method 8260.

Subtask 3. Initial Mobilization

PSI will perform the following elements during the initial mobilization.

3.1 Lithologic Boring Installation - Continuous lithologic profile to 28 feet bls or the top of Hawthorn

Clay will be performed utilizing a DPT rig equipped with a macrocore soil sampling system.

3.2 Piezometer Installation – Two sets of piezometers will be installed.

One set of piezometers will be installed in a cluster configuration at the rear door to 16 feet bls.

The piezometers will then be converted to monitoring wells for later use in obtaining groundwater quality information.

3.3 Interior Soil Borings / Soil Sampling/ MAGS Well Installation – Seven soil borings will be installed inside the building near the former drycleaning machine, storage area and floor drain and will be hand-installed with a core drill and hand auger to the water table. Field screening will be conducted with an OVA-PID. Sample depth intervals will follow FDEP protocols and include 0-0.5 feet, 0.5 - 2 feet, 2-4 feet, 4-6 feet bls and 6-8 feet bls. Additional depth intervals may be added if the depth to water is deeper than the current estimate of 8 feet bls. Soil samples will be analyzed for VOHs by EPA Method 8260 and VOAs.

Five MAGS wells will be installed inside soil borings advanced inside the building and one outside the front door and positioned as close as possibly to the inside of the tenant space where the dryclean machine was formerly located. The MAGS wells will be installed approximately 1-foot above the groundwater table. Each MAGS well will be installed to approximately 7 feet bls and constructed with 2-inch diameter 5-foot lengths of screen (0.01-inch slot size) and 2 feet of casing.

The MAGS wells will be completed at the surface with a 2-inch bronze metal cleanout plug. The MAGS wells will be installed as permanent for later use as SVE wells, if necessary.

3.4 Interior Monitoring Well Installation - A two-inch diameter monitoring well will be installed in one of the soil borings inside the building near the former drycleaning machine location. The well be installed to approximately 13 feet bls (if possible) with 5 to 10 feet of screen (0.01-inch slot size). The well will be completed at the surface with a 2-inch bronze metal cleanout plug.

3.5 MAGS Testing - A well head assembly will be constructed to allow for vacuum, flow and temperature measurements from each MAGS point, and for each portion of the test. During each MAG point test, flow rate, well-head vacuum, and vacuum influence (measured from other MAG points, existing vacuum monitoring points and the horizontal SVE wells) will be measured approximately every 15- minute interval. The vacuum will be varied at the well head, starting from a low applied vacuum and incrementally increasing the vacuum. During each applied vacuum, the vapor flow rate will be measured for the purpose of determining the relationship of applied vacuum to flow rate. During this test, at each vacuum level, the vacuum influence will be measured from the nearby MAGs points or vapor monitoring points to assess the Radius of Influence (ROI). This test will also provide an indication of the maximum well head vacuum without excessive water entrainment. Vapor samples will be collected using SUMMA® canisters for analysis of volatile organic compounds using EPA Method TO-15. It is anticipated that one vapor sample will be collected for each well head test. The sample will be collected at the end of the test.

3.6 Groundwater Screenpoint Sampling- Geoprobe (Direct Push) - Based on groundwater sampling data provided from the existing on-site monitoring wells, soil assessment and MAGS testing data, groundwater screenpoint sampling will be used to determine additional horizontal and vertical groundwater contaminant plume configuration. A Direct Push (DPT) drilling rig will be used to evaluate vertical migration of potential contamination from the former drycleaner at up to eight locations. It is anticipated that two depth intervals will be evaluated from site lithology and the intervals will be determined from continuous macrocore samples at each location. Based on previous assessment information, the depth intervals will be anticipated at 10-14 feet bls and 24-

28 feet bls. The groundwater samples will be submitted to a fixed-base laboratory for VOH and VOA analysis on a standard turnaround time.

3.7 Exterior Soil Boring Installation/ Soil & Sediment Sampling – Exterior soil borings will be performed near the rear door, the stormwater inlet at the rear of the former Quality Cleaners and the dumpster.

Sediment samples will be collected from the stormwater sewer inlet basin and the stormwater outfall located west of the former Quality Cleaners. Soil samples will be collected from hand augered soil samples and macrocore sleeves and screened with an OVA-PID. As previously discussed, the water table is estimated to be approximately 8 feet bls. Soil samples will be retained for laboratory analysis from depth intervals 0 – 0.5 feet, 0.5-2 feet bls, 2-4 feet bls, 4-6 feet bls and 6-8 feet bls. Soil samples will be analyzed for VOHs and VOAs by EPA Method 8260.

3.8 Monitoring Well Installation – Five additional shallow monitoring wells will be installed, of which the locations and specifications will be based on field conditions. For budgetary purposes, it is anticipated that five shallow wells (about 15 feet deep) and one deep well (about 28 feet deep) will be installed. Each shallow monitoring well will be installed with a DPT drilling rig and will be constructed with 1-inch diameter 10-foot lengths of pre-packed screen (0.01-inch slot size) and packed with 20/30 silica sand followed by a 30/65 silica sand seal and grouted to surface. The deep well will be constructed with 1-inch diameter 5-foot lengths of pre-packed screen (0.01-inch slot size) and packed with 20/30 silica sand followed by a 30/65 sand seal and grouted to surface.

The wells will be completed at the surface with an 8-inch manway and a 2 ft by 2 ft concrete pad.

Each well will be developed until the discharge water is visually clear. All drill cuttings and development fluids will be containerized for later transport and disposal.

3.9 Monitoring Well Sampling and Analysis – The newly installed monitoring wells will be sampled using protocols in accordance with FDEP Standard Operating Procedures. The samples will be collected for laboratory analysis for parameters in EPA Method 8260 for VOAs and VOHs. The laboratory will use protocols approved by the National Environmental Laboratory Analytical Conference (NELAC). Equipment blanks and duplicates will not be collected for Quality Assurance/Quality Control purposes, per recent FDEP Guidelines. Top of casing elevations will be measured for all located wells to an established benchmark. Depth to groundwater will be recorded at each well so that a groundwater iso-contour flow map can be generated. Four newly installed monitoring wells will be sampled for the potential presence of PFAS compounds. We anticipate selecting a source area inside the building, outside the back door and two sidegradient or downgradient wells with subsequent analysis by the FDEP Central laboratory in Tallahassee.

3.10 Investigative Derived Waste (IDW) – All soil cuttings, development water and purge water will be containerized in 55-gallon drums, labeled and staged on-site for characterization. Once the results are received, the drums will be transported for disposal / treatment at an approved facility.

It is estimated a total of 6 drums will be utilized for the assessment.

Subtask 4. Data Evaluation

The data collected from the activities described above will be evaluated and recommendations for additional assessment will be presented to the FDEP for approval prior to proceeding with more field activities, potentially consisting of additional permanent monitor wells and subsequent sampling to complete the Site Assessment process.

Deliverables – The following deliverables will be submitted to the FDEP upon completion of the above described subtasks:

• Field notes, boring logs, construction logs, groundwater sampling logs submittal.

• Site Assessment Report which will include a conceptual site model.

• A separate email deliverable will be submitted upon receipt of the PFAS results if the analytical data reveals concentrations above the Health Advisory Level (HAL) or Provisional Groundwater criteria of 70 nanograms per liter (ng/L = parts per trillion). If the levels are below the HAL, the results will be submitted to the FDEP concurrent with the SAR.

• ADaPT deliverable - The laboratory data will be validated using the FDEP’s Automated Data Processing Tool (ADaPT) program to ensure that all required quality control targets were met.

The data will be uploaded to the FDEP portal.

• Investigative Derived Wastes (IDW) manifests.

Quality Assurance / Quality Control – All field work will be performed in accordance with the most current version of the FDEP Standard Operating Procedures guidelines. In addition, an FDEP approved fixed-base laboratory will perform all analyses associated with the horizontal and vertical extents of the associated potential contaminant plume at the subject site. According to 2022 Waste Site Cleanup Section guidelines, no field blanks, equipment blanks, or duplicates will be collected or analyzed.

Health and Safety – Drilling and sampling operations will be directed by a PSI field supervisor, and field personnel will be OSHA trained in accordance with 29 CFR 1910.120. Prior to subsurface drilling activities, PSI will notify the utility service alert in accordance with local practices.
