About this file

This document collection consists of Task Assignment Change Orders (TACOs) and related contract amendments executed between the State of Florida Department of Environmental Protection (FDEP) and Professional Service Industries, Inc. (Intertek-PSI) for environmental services at the Westwood Cleaners site in Tampa, Florida. The project is identified as ERIC_4706 under Contract Number HW686. The primary parties involved are FDEP (represented by Project Manager Mary Uribe) and the contractor PSI-Intertek (represented by Project Manager Joshua Newell). These documents represent amendments to the original task assignment and reflect ongoing modifications to project scope and costs.

The contract encompasses eight subtasks for Operations and Maintenance (O&M) services, Remedial Action Summary Statement (RASS) reports, and utility management at the contaminated site. The original task value was $71,496.70, which was increased through TACO 1 to $71,496.70 and further amended through TACO 2 to $72,638.82, reflecting a cost reduction in Subtask 3 (IDW and lab shipping fees reduced from $8,813.95 to $8,630.95) and an increase in Subtask 5 (poly tank drainage work increased from $9,798.95 to $11,036.05). Deliverables include quarterly O&M activities, system evaluation, analytical sampling, RASS report preparation, and utility billing management, with final deliverables due by May 30, 2026 and task completion scheduled for July 17, 2026. Labor costs are calculated using fixed unit rates with overhead at 151.02%, while subcontractor costs for utilities, IDW disposal, and analytical services are itemized separately. A Work Change Directive (WCD 1) authorized additional work for pumping and containerizing approximately 220 gallons of water from a filled poly tank into DOT-compliant drums, with an estimated cost increase of $1,325.14.

Negotiation Summary

HW686 DC004E CO 2

April 3rd, 2026

This TACO amends Subtasks 3 and 5 for Task Assignment DC004E.

Reductions in Subtask 3 are due to the IDW and lab shipping fees being reduced.

Subtask 3 was reduced from $8,813.95 to $8,630.95.

The increase for Subtask 5 was to rectify the WCD signed on 11/12/2025 regarding draining the poly tank. Subtask 5 was increased from $9,798.95 to $11,036.05.

Total task amount changed from $71,496.70 to $72,638.82. PSI’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date is 05/30/2026 and has not changed. Task completion date is 07/17/2026 and has not changed.
