About this file

This is a Task Assignment (TA) document executed under Florida Department of Environmental Protection (FDEP) Contract No. HW686 between the State of Florida Department of Environmental Protection and Professional Service Industries, Inc. (Intertek-PSI). The task assignment is designated as DC018C and addresses groundwater and PFAS monitoring services at Hampton Cleaners, located at 5317 Gunn Highway, Citrus Park, Hillsborough County, Florida (ERIC_4713). The agreement was executed on August 27, 2025, with a completion date of November 27, 2026. Key personnel include Mary Uribe as the Department Project Manager and Joshua Newell as the Contractor's Project Manager.

The contracted scope of work comprises quarterly volatile organic hydrocarbon (VOH) groundwater monitoring and annual PFAS (per- and polyfluoroalkyl substances) monitoring at the site. Services include field sampling activities, laboratory analysis via EPA Method 8260 for VOH compounds and DEP Chemistry Lab analysis for PFAS, quarterly email interim deliverables, and a comprehensive annual groundwater monitoring report with engineering recommendations. The total task assignment value is $18,912.81 on a fixed unit rate basis, invoiced upon submittal and approval of deliverables. Invoicing schedule includes $5,964.46 for Q1 VOH and PFAS monitoring (due December 31, 2025), $2,562.94 each for Q2, Q3, and Q4 VOH interim deliverables (due March 30, June 30, and September 30, 2026 respectively), and $5,259.53 for the annual groundwater monitoring report or site rehabilitation completion report (due October 31, 2026). The level of effort and cost estimate worksheets detail labor costs, vehicle mobilization charges of $120 per day, laboratory analytical fees, equipment rental, and subcontractor costs for investigative derived waste disposal.

FDEP – Hampton Cleaners ERIC_4713 PSI Proposal 0552-444506 rev 3

July 25, 2025 www.intertek.com/building

PROPOSED SCOPE OF WORK

The proposal contained herein consists of conducting quarterly groundwater monitoring for VOH compounds and annual groundwater monitoring for PFAS compounds. Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets and is detailed in Subtask 1.

2. Quarterly VOH Groundwater Monitoring (Qtr 1) with Email Interim Deliverable

a. Mobilization to the site.

b. Replace caps for RW003 (6-inch), RW005 (6-inch) and RW006 (4-inch).

c. Collect DTW from up to 10 monitoring wells with a minimum of three shallow and three deep monitoring wells. (Proposed: MW004, MW005, MW006, DMW008, DMW009, MW010, MW011, MW013, and DMW015.)

d. Collect a groundwater sample for VOH analysis from shallow monitoring well MW010.

d. Submit groundwater sample to a NELAC-approved laboratory for analysis of VOHs via EPA Method 8260.

e. Develop an interim email deliverable with field notes, sampling forms, updated analytical table, lab report and recommendation for annual monitoring or another groundwater sampling event 90+ days later based on VOH results documented above or below GCTLs.

f. The investigative derived waste (IDW) that will be generated during quarterly groundwater monitoring events is limited to purge water from the monitoring wells which will be containerized in a 55-gallon drum and transported off-site for disposal at an FDEP approved facility, pending waste characterization analytical results. Analysis of IDW with waste profile and characterization is included in this subtask while Transport and Disposal efforts are covered in Subtask 7.

3. Annual PFAS Monitoring

a. Collect groundwater samples for PFAS compounds from the following:

i. Wells RW002, RW004, DMW009, MW010, and DMW015

b. Submit groundwater samples for PFAS to the DEP Chemistry Lab in Tallahassee, FL.

July 25, 2025 www.intertek.com/building

4. Quarterly VOH Groundwater Monitoring (Qtr 2) with Email Interim Deliverable

A minimum of two consecutive sampling events, more than 90 days apart, and with analytical results reported below GCTLs are required to close the site. MW010 has most recently been reported with GCTL exceedances and therefore requires a minimum of two additional quarterly sampling events to close the site. Based on this, PSI is proposing activities from Subtask 2 be duplicated with the exception of IDW waste sampling and characterization and replacement of well caps as provided below.

a. Mobilization to the site.

b. Collect DTW from up to 10 monitoring wells with a minimum of three shallow and three deep monitoring wells. (Proposed: MW004, MW005, MW006, DMW008, DMW009, MW010, MW011, MW013, and DMW015.)

c. Collect a groundwater sample for VOH analysis from shallow monitoring well MW010.

g. Submit groundwater sample to a NELAC-approved laboratory for analysis of VOHs via EPA Method 8260.

h. Develop an interim email deliverable with field notes, sampling forms, updated analytical table, lab report and recommendation for annual monitoring or another groundwater sampling event 90+ days later based on VOH results documented above or below GCTLs.

i. The investigative derived waste (IDW) that will be generated during quarterly groundwater monitoring event is limited to purge water from the monitoring well which will be containerized in a 55-gallon drum and transported off-site for disposal at an FDEP approved facility, pending waste characterization analytical results. Analysis of IDW with waste profile and characterization is included in subtask 2 while Transport and Disposal efforts are covered in Subtask 7.

5. Quarterly VOH Groundwater Monitoring (Qtr 3) with Email Interim Deliverable

In the event that one or both of the first two quarters are reported with GCTL exceedances, an additional sampling event will be required for site closure. At FDEP’s request, PSI is proposing additional quarterly sampling as needed. Efforts proposed in Subtask 5 are a duplicate of Subtask 4.

6. Quarterly VOH Groundwater Monitoring (Qtr 4) with Email Interim Deliverable
