About this file

This document comprises a Task Assignment Change Order (TACO No. 2) issued by the State of Florida Department of Environmental Protection (FDEP) to Professional Service Industries, Inc. (Intertek-PSI) for work performed at the Coastal Cleaners site (ERIC_5272) in Dunedin, Pinellas County, Florida under Contract HW686, Task Assignment DC021A. The change order is authorized by Ben Grubbs (FDEP Project Manager) and Nana G. Westmark (Intertek-PSI Senior Geologist/Principal Consultant). Teresa Booeshaghi, Secretary or Designee for FDEP, executed the change order on February 12, 2024.

The change order reduces the total task assignment value from $188,978.70 to $187,047.88, reflecting a deduction of $1,930.82 due to the elimination of unnecessary costs for Remedial Indoor Waste (IDW) disposal ($750.00) and a power drop installation from Duke Energy ($1,005.29) that was not ultimately needed for the soil vapor extraction (SVE) system installation. The scope of work includes system installation, startup, construction completion report, one month of operation and maintenance, and carbon filter replacement with manifold reconfiguration. The task completion date remains May 13, 2024. Deliverables include initial system installation documentation (due 11/30/2023), system startup field notes (due 12/11/2023), construction completion report with one-month O&M checklist (due 01/22/2024), and utilities costs documentation (due 02/12/2024). Invoice payments are structured based on submittal and approval of these deliverables, with the final total cost not to exceed $187,047.88.

EXTERNAL MESSAGE

This email originated outside of DEP. Please use caution when opening attachments, clicking links, or responding to this email.

Hi Ben – PSI is pleased to submit a Change Order request for Coastal Cleaners ERIC_5272, DC 021A.

Briefly, we installed an SVE system at this site in Dunedin, Pinellas County, and we need to deduct costs for several items.

IDW was deducted and the costs for a power drop from Duke Energy (we ultimately did not need a new power supply).

DC 021 A-CO-1…………………………$188,978.70

Delta…………………………………..………($1,930.82)

DC 021 A-CO-2…………………………$187,047.88

If there is anyway I can get this quickly, I would appreciate it. My corporate office is REALLY pushing to get this $ off of our books.

Please contact me with any questions.

Kind Regards, Nana Westmark, PG, CHMM Senior Geologist / Principal Consultant Building & Construction Intertek-PSI
