Re: Site Assessment Cost Estimate and Level of Effort Classic Cleaners ERIC_4750 2291 East Bearss Avenue Tampa, Hillsborough County, Florida

Dear Ms. Schwed:

Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to provide a Site Assessment for the Classic Cleaners ERIC_4750, Tampa, Florida.

Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion. According to the signed access form from May 2022, the same property owner is listed in the current property appraiser’s website for Hillsborough County.

PROJECT BACKGROUND

The Classic Cleaners facility, currently Sunshine Cleaners, is located at 2291 East Bearss Avenue, in the Meridia Square Shopping Plaza near the southeast corner of Bearss Avenue and North 22nd Street in Tampa, Hillsborough County, Florida. The subject site is situated approximately 1.3 miles east of Interstate I-275 and approximately 1.7 miles southwest of Tampa Palms, in central Hillsborough County. Classic Cleaners occupied the most northeastern tenant space of the multi-tenant commercial shopping plaza.

The drycleaner currently is a drop off drycleaning business. A stormwater retention feature for the plaza is situated along the southeastern portion of the property mailto:Skylar.Schwed@FloridaDEP.gov

FDEP – Classic Cleaners ERIC_4750

According to information available from the Hillsborough County Property Appraiser, the existing Meridia Square Shopping Center was constructed in 1990 and drycleaning operations for Classic Cleaners became active circa June 1990. The older machine was documented in the DSCP Application as having a leak beneath the solvent tank in September 1996 and was subsequently removed from the facility. The owner at the time then operated the cleaner as a drop-off facility until a new drycleaning machine could be installed.

Based on information from the FDEP database, drycleaning operations at the facility ceased in January 2012 when the cleaner converted to a dry drop-off facility.

A Site Screening Report dated May 30, 1997 identified PCE in one soil sample at a concentration of 20 mg/kg, exceeding the commercial direct exposure soil cleanup target level (SCTL) criteria. The sample was collected near the drycleaning machine from one foot below land surface (bls), potentially a composite sample as the report indicates it was collected from “in front and behind” the dryclean machine. Five additional soil samples were collected at different locations, the results of which, however, were not reported.

PSI visited the site on August 2, 2022, and met with Mr. Brad Ackerman, representative from property owner Meridia Associate, LTD, property manager, Mr. Jim Ward and the tenant, Mr. Jay Kim whom provided access inside the cleaner and the property.

As the Classic Cleaners has been accepted into the Drycleaning Solvent Cleanup Program, PSI was assigned to prepare a Site Assessment Work Plan. A Site Assessment Work Plan was submitted by PSI, revised per FDEP suggestions, resubmitted and approved in August 2022, and follows the guidelines presented in the July 2022 FDEP Contractor’s Workshop. Bids and quotations were solicited from various firms to provide services for the assessment. A copy of all quotations is attached.

PSI, therefore, proposes to conduct the Site Assessment at the Palm Dry Cleaners, as presented below.

PROPOSED SCOPE OF WORK

Subtask 1. Project Management

This scope is provided for in the template worksheets.

Subtask 2. Notification/ Utility Locate / GPR Survey / Land Survey / Existing Monitoring Well

Sampling

Prior to mobilizing field crews, PSI will provide several site assessment preparation elements.

2.1 Notification – The tenant and/or owner of the property will be notified of the planned field activities.

2.2 Utility locates – Sunshine 811 will be notified for utility locates.

2.3 GPR – A ground penetrating radar (GPR) survey will be performed to determine the presence and location of subsurface utilities leading up to the building.

2.4 Land Survey – A Specific Purpose Survey with locations of improvements will be performed by a

Florida Licensed Surveyor and Mapper.

Subtask 3. Initial Mobilization

PSI will conduct an initial mobilization to consist of the following items.

3.1 Lithologic Boring / Piezometers Installation – Continuous lithologic profile to 50 feet bls or refusal depth will be performed utilizing a DPT rig equipped with a macrocore soil sampling system. The target geologic formation will be the Hawthorn clay layer. Three piezometers will be installed to evaluate shallow groundwater flow direction. The piezometers will be converted to shallow wells.

3.2 Interior Soil Borings / Soil Sampling/ MAGS Well Installation - Soil borings will be installed inside the building to evaluate source areas and potential contaminant release points near the former drycleaning machine, former spotting board and floor drains. Two soil borings will be installed outside the building. Soil borings will be hand-installed with a core drill and hand auger to the water table and will be screened utilizing an OVA-PID. Sample depth intervals will follow FDEP protocols and include 0 - 0.5 feet bls, 0.5 - 2 feet bls, 2 - 4 feet bls, 4 - 6 feet bls, 6 - 8 feet bls and 8 - 10 feet bls. The water table is estimated to be approximately 10 feet bls. Soil samples will be analyzed for VOHs and VOAs by EPA Method 8260.

Five MAGS wells will be installed inside five of the six soil borings inside the building and one location outside of the building (back door). The MAGS wells will be installed approximately 1-foot above the groundwater table. Each MAGS well will be installed to 8 feet bls and constructed with 2-inch diameter 7-foot lengths of screen (0.01-inch slot size) and 1 foot of casing. The MAGS wells will be completed at the surface with a 2-inch bronze metal cleanout plug. The MAGS wells will be installed as permanent for later use as SVE wells, if necessary.

3.4 Interior Monitoring Well Installation - A two-inch diameter monitoring well will be installed in one of the soil borings inside the building near the former drycleaning machine location. The well be installed to approximately 14 feet bls (if possible) with 10 feet of screen (0.01-inch slot size).

The well will be completed at the surface with a 2-inch bronze metal cleanout plug.

3.5 MAGS Testing - A well head assembly will be constructed to allow for vacuum, flow and temperature measurements from each MAGS point, and for each portion of the test. During each MAG point test, flow rate, well-head vacuum, and vacuum influence (measured from other MAG points, existing vacuum monitoring points and the horizontal SVE wells) will be measured approximately every 15- minute interval. The vacuum will be varied at the well head, starting from a low applied vacuum and incrementally increasing the vacuum. During each applied vacuum, the vapor flow rate will be measured for the purpose of determining the relationship of applied vacuum to flow rate. During this test, at each vacuum level, the vacuum influence will be measured from the nearby MAGs points or vapor monitoring points to assess the Radius of Influence (ROI). This test will also provide an indication of the maximum well head vacuum without excessive water entrainment. Vapor samples will be collected using SUMMA® canisters for analysis of volatile organic compounds using EPA Method TO-15. It is anticipated that one vapor sample will be collected for each well head test. The sample will be collected at the end of the test.

3.6 Groundwater Screenpoint Sampling - Geoprobe (Direct Push) - A DPT drilling rig will be used to evaluate horizontal and vertical migration of potential contamination from the former drycleaner at up to nine locations. It is anticipated that three depth intervals will be evaluated from site lithology and the intervals will be determined from continuous macrocore samples at each location. Based on previous assessment information, the depth intervals will be anticipated at 11- 15 feet bls, 21-25 feet bls and 31-35 feet bls. The groundwater samples will be submitted to a fixed-base laboratory for a standard turnaround time for VOH and VOA analysis.

3.7 Exterior Soil Boring Installation/ Soil Sampling – Exterior soil borings will focus on the backdoor area and one area to the side of the dryclean building. Soil borings will be installed to the water table immediately east of the building utilizing a hand auger and DPT drilling rig. A soil boring will be installed near the back door and the dumpster location of the facility. Soil samples will be collected from hand augered soil samples and macrocore sleeves and screened with an OVA-PID.

The water table is estimated to be approximately 10 feet bls. Soil samples will be retained for laboratory analysis from depth intervals 0-0.5 feet bls, 0.5-2 feet bls, 2-4 feet bls, 4-6 feet bls, 6-8 feet bls and 8-10 feet bls. Soil samples will be analyzed for VOHs and VOAs by EPA Method 8260.

Soil samples will be collected with an Encore sample such that SPLP analyses may be able to be run should the results indicate the concentrations of the contaminants of concern are above the Leachability criteria and below the Residential Exposure, of Chapter 62-777, FAC, and per FDEP approval.

3.8 Monitoring Well Installation - Geoprobe (Direct Push) – Subsequent to the preliminary groundwater screenpoint sampling data, it is estimated that additional cluster well sets will be required. Each cluster set will consist of a shallow, intermediate and possibly deep wells.

Although the exact depths of the wells will be determined by the groundwater testing of existing wells and the results of the lithologic profiling, for estimating purposes, the well screen intervals will be as follows:

• Shallow (up to 4 wells): 5- 15 feet bls;

• Intermediate (up to 5 wells): 20 - 25 feet bls and/or 30 – 35 feet bls;

• Deep (up to 3 wells): 40 - 45 feet bls; (depending on necessity)

Each shallow monitoring well will be installed with a DPT drilling rig and will be constructed with

1-inch diameter 10-foot lengths of pre-packed screen (0.01-inch slot size) and packed with 20/30 silica sand followed by a 30/65 silica sand seal and grouted to surface. Intermediate and deep wells will be constructed with 1-inch diameter 5-foot lengths of pre-packed screen (0.01-inch slot size) and packed with 20/30 silica sand followed by a 30/65 sand seal and grouted to surface. The wells will be completed at the surface with an 8-inch manway and a 2 ft by 2 ft concrete pad. Each well will be developed until the discharge water is visually clear. All drill cuttings and development fluids will be containerized for later transport and disposal.

3.9 Groundwater Sampling – Groundwater samples will then be collected from the newly installed monitoring wells and analyzed for VOHs and VOAs by EPA Method 8260. Natural Attenuation and Remedial Parameters may also be collected from an upgradient well, a source area well and a downgradient well, if necessary. Groundwater sampling will be conducted in general accordance with FDEP’s Standard Operating Procedure (SOP) DEP-SOP 001/01 for Field Activities. Equipment blanks and duplicates will not be collected for Quality Assurance/Quality Control purposes per recent guidance from the Waste Site Cleanup Section. Groundwater purging and sampling will be conducted in accordance with the FDEP SOP requirements.

Top of casing elevations will be measured for all located wells to an established benchmark. Depth to groundwater will be recorded at each well so that a groundwater iso-contour flow map can be generated.

3.10 Data Evaluation – The data collected from the activities described above will be evaluated and recommendations for additional assessment will be presented to the FDEP for approval prior to proceeding with an additional mobilization.

3.12 Investigation Derived Waste (IDW) – All soil cuttings, development water and purge water will be containerized in 55-gallon drums, labeled and staged on-site for characterization. Once the results are received, the drums will be transported for disposal / treatment at an approved facility.

It is estimated a total of 5 drums will be utilized for the assessment.

3.13 Deliverables – The following deliverables will be submitted to the FDEP upon completion of the site assessment:

• Site Assessment Report which will include a conceptual site model,

• Separate email with PFAS results. If the PFAS levels are above the EPA Health Advisory Level

(HAL) of 70 nanograms per liter (ng/L = parts per trillion), an email summary with map and brief information will be submitted within 48 hours of laboratory results. If the results are below the HAL, the email submittal will be provided concurrently, but separately, from the Site Assessment Report.

• ADaPT deliverable,

• Investigative Derived Wastes (IDW) manifests.

3.14 Quality Assurance / Quality Control – All field work will be performed in accordance with the most current version of the FDEP Standard Operating Procedures guidelines. In addition, an FDEP approved fixed-base laboratory will perform all analyses associated with the delineation of the contaminant plume at the subject site. Please note that recent FDEP guidelines do not require the collection or analysis of trip blanks, equipment blanks, or duplicate samples.

3.15 Health and Safety – Drilling and sampling operations will be directed by a PSI field supervisor, and field personnel will be OSHA trained in accordance with 29 CFR 1910.120.

3.16 ADaPT – The laboratory data will be validated using the FDEP’s Automated Data Processing Tool

(ADaPT) program to ensure that all required quality control targets were met. The data will be uploaded to the FDEP portal for review and approval.

RE: Utility Locating Services – Classic Cleaners, Tampa, Florida

Ms. Westmark, GeoTek Services is pleased to respond to your request for Utility Locating services at the Classic Cleaners site located in Tampa, Florida. We will scan the area shown in your PDF drawing with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings, or vehicles. We will scan as close as reasonably possible to fixed objects such as curbs, vehicles, fences, etc. Our deliverable will be marking paint and/or flags indicating the horizontal location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our utility locating quote for this project is $1,500.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Proposal

This document shall serve as a proposal for work to be performed by GeoView Associates, Inc. (GeoView) for Intertek-PSI.

Description of Site

The project site is referred to as the Classic Cleaners (current Sunshine Cleaners) Site and is located at 2291 Bearss Avenue in Tampa, Florida. The purpose of the geophysical survey will be to determine the location of underground utilities within the designated area of the project site. The area of investigation is approximately 1 acre in size and is shown on Figure 1 (page #). The investigation will only be performed in the areas outside of existing buildings.

Scope of Work

GeoView will provide geophysical surveying services at the project site. The purpose of the geophysical survey will be to determine the location of underground utilities or other underground structures within the designated area of the project site. The fieldwork is estimated to take 1 day to complete. This estimate is based upon the site conditions being as described by Intertek-PSI.

The geophysical survey will be conducted using ground penetrating radar (GPR) and conventional electromagnetic (EM) pipe and cable locators. The GPR survey will be conducted using either a Mala and/or a GSSI GPR system. The appropriate GPR antenna configuration and time range and processing parameters will be determined in the field by the project manager.

The EM survey will be performed using a Subsite 95R and/or a Radiodetection RD (8000, 8100 or 8200) system. As appropriate, the EM survey will be conducted using both passive and active modes. The passive mode is used to find cables and conduits carrying live power (50-60 Hz) and communication (29-33Hz) signals. For the active mode survey, a current is transmitted directly or broadcast into using electromagnetic induction a metal pipe or conduit and the path of the pipe/conduit is identified by following the signal with a portable receiver.

The active mode is used to trace out cast iron pipes, metal water and sewer lines, utilities that have trace wires, and inactive power and communication lines.

GeoView’s utility locating personnel are all certified GPR and conventional pipe and cable locator operators with extensive in-house training.

Based on information provided by Intertek-PSI, it does not appear that the work at any of the sites will require MOT (traffic calming efforts). This proposal does not include the costs for the design or implementation of an MOT program.

Presentation of Results

The location and estimated depths of underground utilities or any other identified structures shall be indicated on the ground surface using spray paint or other methods as specified by Intertek-PSI. Designated utilities will also be documented on a scaled AutoCAD site map. The site map will provide the location, depth and purpose of any underground utilities or other structures identified by the survey. GeoView will use a differentially corrected GPS system to determine the location of any identified underground features. In most site settings the documented positions will be accurate within +/- 1-2 feet.

If requested GeoView can provide drone aerial imagery services to document site conditions as they exist at the time of the geophysical site investigation. All images will be geo-rectified in order to provide a scaled and orthographically correct base map upon which the results from the geophysical investigation will be presented. High resolution images will be collected with a DGI m300 drone with a Zenmuse P1 camera. Lidar surveying services can also be provided using a Zenmuse L1 or equivalent. GeoView’s drone pilots are FAA Part 107 licensed, and the imagery will be collected per FAA guidelines when working in both urban areas and near commercial airports. Cost for arial imagery services upon request.

Compensation

Cost to complete the survey will be $1,600. This price is inclusive of all charges associated with the project. Unless otherwise agreed upon, GeoView shall be compensated for all services within 30 days of invoice date. GeoView shall be paid in full regardless of whether the results of the geophysical survey are what Intertek-PSI anticipated. If it is determined during the survey that the geophysical survey will not achieve the objectives of the project, Intertek-PSI will immediately be notified. If a decision is made to discontinue the survey, only charges for time and materials costs to that point will be submitted.

These prices are based upon GeoView being able to access the site during normal business hours, Monday through Friday. It is also based upon the assumption that no site-specific training or drug testing will be required. If there are any such requirements, additional costs will be incurred.

Approval of this proposal and scheduling of the project indicates that the site will be accessible and ready for the geophysical investigation. If the site is not accessible at the scheduled day and time of the survey, a mobilization charge of $250 will be applied, along with a standby rate of $150 per hour.

Requirements of Client

Intertek-PSI will provide a scaled map and digital file, if available, of the project site showing the boundaries of the project site, areas of specific concern and pertinent landmarks. Intertek-PSI will also supply all available information regarding the presence and history of underground utilities at the project site (see limitations for further discussion). Unless otherwise requested, Intertek-PSI will be responsible for the coordination of site access, traffic control, clearing of onsite obstructions or any other logistical consideration necessary to conduct the survey.

If any test drilling, monitor well installation or other excavation activities are planned it will be the responsibility of Intertek-PSI to submit a notification (dig ticket) to the appropriate 811 One-Call agency. It is recommended that such notification be filed sufficiently in advance so that GeoView will have the opportunity incorporate the 811 findings into their results.

Limitations

The objective of the geophysical survey is to determine the presence and configuration of underground utilities. However, the geophysical response of these features may be highly complex depending upon on the physical characteristics of the underground utilities, the electrical properties of the surrounding soils, the depth of burial of the utilities, and the proximity of the utilities to each other.

GeoView shall conduct the geophysical survey using the most “up-to-date” geophysical equipment in a manner consistent with the level of care and skill ordinarily exercised by members of the geophysical profession practicing in the same locality under similar conditions. GeoView’s utility locating personnel are all certified GPR and conventional pipe and cable locator operators with extensive in-house training.

It is acknowledged that GeoView is not an 811 Sunshine One-Call or equivalent utility mark-out service. GeoView does not have access to any utility-related records for either public or private properties. It is the responsibility of Intertek-PSI to obtain such records and provide to GeoView prior to the site investigation. GeoView will use these records during the site investigation and report to Intertek-PSI any utilities that are shown on the site records but cannot be confirmed by GeoView. GeoView will not mark on the ground surface any utilities which it cannot confirm.

GeoView shall only be liable for the successful locating of underground utilities or any other underground structures whose presence is both known and indicated on site maps to be provided prior to the performance of the geophysical survey. Every attempt will be made to locate both known and unknown utilities within the survey area. However, GeoView will only be liable for utilities whose presence is known prior to the survey. Intertek-PSI accepts that due to the inherent limitations of non-intrusive geophysical technologies GeoView cannot be held liable for damages to utilities that cannot identified using non-intrusive technologies.

Intertek-PSI accepts that a non-intrusive utility designation study does not replace the need to employ widely accepted industry safety standards for subsurface work. Concerning the installation of well or test borings this standard of care includes post-holing or hand auguring at the planned test locations to the minimum depth at which underground utilities are anticipated at the project site. If absolute confirmation is needed to a specified depth that underground utilities are not present at a planned work location GeoView can provide vacuum excavation services. Pricing for such services is available upon request.

When Intertek-PSI is working around indicated utilities, a minimum of 2 ft clearance from either side of the marked utility will be maintained. Unless otherwise indicated, a minimum vertical clearance of +/- 20 percent of the indicated depth shall also be maintained. Intertek-PSI maintains the responsibility for manually exposing any indicated underground utility for absolute confirmation.

Authorization and scheduling of the project is considered acceptance of these terms and conditions.
