From: Skylar Murphy, Project Manager Date: September 15, 2022 Subject: Negotiation Summary

HW686 DC007B

This Task Assignment authorizes Professional Service Industries, Inc. (PSI) to conduct a Deep MAGs Survey in accordance with the PSI scope of work, cost estimate, and schedule received on September 14th, 2022.

PSI confirmed on September 14th, 2022, that an Access Agreement with the current real property owner of the Fashion Dry Cleaners (ERIC_5245) site has been executed.

Design approval was verbally received from Robert Huckaby P.E. following the recommended edits to the scope of work. the email is included on the following page.

The original Task Assignment totaled $40,681.52 with 156 labor hours but after negotiations and corrections with contractor costs are reflected as below:

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $41,483.74 with 159.5 labor hours. The increase in price is due modifications in the location and depth of MAGs wells.

PSI may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

From: Huckaby, Robert To: Schwed, Skylar L Subject: RE: Fashion MAGs Proposal Date: Monday, September 12, 2022 9:21:07 AM

Skylar, I do have some edits that need to be made. I am going to list the MAGS Well Installation changes in PSI’s proposal format.

Eight MAGS wells should be installed and tested in the following locations:

1. New MAGs screened from 1 – 8 feet bls, near SB005

2. New MAGS screened from 9- 20 feet bls, near SB005

3. New MAGS screened from 1 - 8 feet bls, near SB006

4. New MAGS screened from 9 – 20 feet bls, near SB006

5. New MAGS screened from 22 – 27 feet bls, near SB006

6. New MAGS screened from 9 – 17 feet bls, near SB004

7. New MAGS screened from 18 - 28 feet bls, near SB004

8. New MAGS screened from 9 – 17 feet bls, near SB001

The key edits to PSI’s proposal are that:

A shallow MAGS well should be installed near SB005 since we may not be able to hook the interior shallow MAGS well in this area to a system in the future, and Because the existing shallow MAGS wells are screened to a depth of 8-feet bls, the intermediate depth MAGS wells screen should start at 9-feet bls. This will give a 1-foot buffer between the bottom of the shallow screen and the top of the intermediate screen which will hopefully prevent short circuiting between these wells.

Please let me know if you have any questions. Thanks

Robert

Re: Proposal for Supplemental Site Assessment – 2 – Revision 1 Deep MAGS Survey Fashion Cleaners (Former) 6502 Massachusetts Avenue New Port Richey, Pasco County, Florida 34653

ERIC_5245

Dear Ms. Schwed:

Professional Service Industries, Inc. (PSI), an Intertek company, is pleased to provide the proposal contained herein to provide a Supplemental Site Assessment – 2 to consist of a Modified Active Gas Survey (MAGS) for the predominately deeper vadose zone for the Fashion Cleaners at 6502 Massachusetts Avenue, New Port Richey, Florida. Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion. A Site Access Agreement was signed by Mr. Trent Goss in June 2019, who is listed as the registered agent for the current property owner, according to the Pasco County Property Appraiser’s website.

PROJECT BACKGROUND

The Fashion Cleaners (Former) was located in the East Richey Square Shopping Center on the southeast corner of Massachusetts Avenue and Congress Street in New Port Richey, Pasco County, Florida. The Fashion Cleaner operated in the westernmost tenant space in the shopping center which currently houses a bar, the Home Saloon. Based on the FDEP records, drycleaning began at the facility in 1978 and utilized perchloroethene (PCE) as the drycleaning solvent. The drycleaner operated from 1978 to approximately 2007 or 2008. No drycleaning equipment remains on-site. A Site Screening Report dated January 30, 1997 was submitted which identified PCE in a soil sample at 57 ug/kg (0.057 mg/kg) at a depth of 2.5 feet below land surface (bls) outside the back door of the facility.

mailto:Skylar.Schwed@FloridaDEP.gov

FDEP – Fashion Cleaners ERIC_5245 PSI Proposal No. 0552-382310- Rev 1

September 7, 2022, revised September 14, 2022 www.intertek.com/building

PSI completed Site Assessment activities in November 2020 through February 2021, with a Site Assessment Report submitted in March 2021. The results identified low-level PCE concentrations (below SCTLs) within the 0.5-2 feet bls to 8-10 feet bls intervals from soil samples collected beneath the building and soil samples collected outside of the former drycleaning tenant space. A MAGS study from January 2021 was conducted in the upper vadose zone which revealed elevated PCE concentrations above the Department’s guidance value of 1.0 mg/m3 for SVE implementation.

A second round of MAGS testing was conducted in August 2021 which revealed similar VOH air concentrations. During recent technical discussions between the FDEP and PSI, it was agreed to evaluate the deeper vadose zone prior to any active remediation plans, and to augment any previous data gaps in the upper vadose zone.

PSI is, therefore, proposing to conduct a deeper MAGS study to address the data gaps.

PROPOSED SCOPE OF WORK

The proposal contained herein consists of installing MAGS wells at select locations and conducting a MAGS test at the Fashion Cleaners. The results will be used to confirm current soil quality conditions and to evaluate if a contaminant mass is located in the vadose zone at deeper areas. Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets.

2. MAGS Well Installation

PSI will provide oversight of a licensed water well contractor to install eight (8) MAGS wells. The MAGS are proposed to be installed and tested at the following locations.

September 7, 2022, revised September 14, 2022 www.intertek.com/building

The proposed MAGS wells will be constructed of 2- inch diameter Schedule 40 PVC, with similar riser. A filter pack and bentonite seal will be emplaced, and the well will be constructed with a 10 – 12-inch manhole cover with an approximate 4 to 6 - inch clearance space between the grout and the manhole cover to allow a compression fitting to conduct the MAGS test. Once the MAGS wells are installed, a MAGS test will be conducted to evaluate potential contaminant sources.

Three drilling firms (Huss Drilling, Groundwater Protection, and Preferred Drilling) were solicited for price proposals to install the wells, which are attached. Groundwater Protection (a division of DrillPro) was selected to perform the work based on price and our previous experience with the firm.

The wells will be constructed such that the MAGS wells may be converted to Soil Vapor Extraction (SVE) wells in the event that an SVE remediation strategy is necessary. Any investigative derived waste (IDW) will be containerized in 55-gallons drums, labeled for date of generation, and transported off-site for disposal at an approved facility.

3. MAGS Test

PSI will conduct a deep zone MAGS test using the newly installed wells to evaluate the extent of soil contamination and contaminant source identification. A regenerative blower and generator will be utilized to conduct the MAGs test. Flexible hose will be brought on-site to allow for the extraction of each MAGs point by the system. A high-speed peristaltic pump will be connected to the vapor sampling point to allow for continuous vapor screening with a MiniRAE 3000 OVA-PID or equivalent.

During each MAG point test, flow rate, well-head vacuum, and vacuum influence (measured from other MAG points, existing vacuum monitoring points and the horizontal SVE wells) will be measured at select time intervals.

The vacuum will be varied at the well head, starting from a low applied vacuum and incrementally increasing the vacuum. During each applied vacuum, the vapor flow rate will be measured for the purpose of determining the relationship of applied vacuum to flow rate. During this test, at each vacuum level, the vacuum influence will be measured from the nearby MAGs points to assess the Radius of Influence

(ROI).

Vapor samples will be collected using SUMMA® canisters for analysis of volatile organic compounds using EPA Method TO-15. It is anticipated that one vapor sample will be collected for each well head test. The sample will be at the end of the test.

4. Data Evaluation and Report Development

A Supplemental Site Assessment Report – 2 will be generated for the MAGS well installation and testing event. The data generated from the above described services will be tabulated and inserted onto maps at each specific test point. Tables will be generated summarizing the data collected from each MAG point and monitoring well. Conclusions and recommendations will be provided for the next appropriate action.

The reports will be signed and sealed by a Professional Engineer or Professional Geologist certified in the State of Florida.

Good Afternoon – Hope everyone is well these crazy days.

PSI is working for the FDEP under the Drycleaning Solvent Cleanup Program and will be installing 7 vapor monitoring wells near an old drycleaner.

The site is now a bar, but we will be outside. The area of work is asphalt, near the side of the building. See attached map.

From: Nana Westmark Sent: Thursday, January 13, 2022 8:14 AM To: Natacha Jackson Cc: Greg Lassi Subject: Annual IDW prices for FDEP Drycleaning Contract for PSI Team

Good Morning Natacha – Hope you are doing well these days.

The PSI Team (PSI and Chastain Skillman) have been awarded one of the FDEP “Drycleaning Contracts” and we are anxious to get started.

We are inviting your firm to provide costs for pick-up, transport, and disposal to an approved facility of Investigation- Derived Waste (IDW) on a per 55-gallon drum basis from drycleaning sites and state funded sites, and site investigation sites that are in the programs.

If you are interested in providing a quote, we would appreciate it. I am attaching a form in which to submit back to us, along with the General Conditions.

If you could provide these prices to us by January 21, 2022, we would appreciate it.

Thank you, Nana
