About this file

This is a Task Assignment (TA) document executed between the State of Florida Department of Environmental Protection (FDEP) and Professional Service Industries, Inc. (Intertek-PSI) under the umbrella Contract Number HW686, Waste Cleanup Program Services. The task authorizes PSI to create an Interim Source Removal Plan (ISRP) using Soil Vapor Extraction technology for the Fashion Cleaners facility (ERIC 5245) located at 6502 Massachusetts Avenue in New Port Richey, Pasco County, Florida. The Department Project Manager is Mary Uribe, and the Contractor's Project Manager is Joshua Newell. The task assignment was executed in October 2024 with a completion date of March 24, 2025.

The scope of work consists of two subtasks: Project Management and development of an ISRP addressing volatile organic compound (VOH) contamination in soil at the site. The design will include calculations for the number of SVE wells, radius of influence, estimated cleanup timelines, and system specifications. The ISRP will incorporate site plans, trench details, cross-sections, treatment system layout, piping and instrumentation diagrams, electrical details, and equipment lists. The total task value is $19,217.46 on a fixed unit rate basis, with the primary deliverable due on February 27, 2025 (with an earlier internal deadline of January 31, 2025 per the proposal). PSI's cost estimate includes 122 labor hours across multiple staff categories, travel expenses of $240, and equipment and expendable costs of $690, along with overhead and completion fees. Invoice payment is contingent upon submittal and approval of the ISRP.

PSI Proposal: 0552-427417 Rev 1 FDEP Fashion Cleaners ERIC_5245

Jully 17, 2024 www.intertek.com/building

During the Site Assessment and subsequent Supplemental Site Assessment consisting of a Deep MAGS study, the site remains impacted with VOH compounds which could ultimately create groundwater impacts. The Deep MAGS study consisted of eight additional deep wells with varying screen lengths ranging from 8 to 28 feet. The site lithology consists of layers of sand, sandy clay, and limestone. The sandy clay layer was encountered at about 15 to 25 feet bls, was typically 2-4 feet in thickness, and was only penetrated by the deep MAGS wells. Based on the results of the Deep MAGS test, PCE source areas appear to be concentrated near the southern and western portion of the building and are anticipated to be in greater concentrations towards the surface. The average PCE concentrations from shallow, intermediate, and deep MAGS wells were reported as 3.88 mg/m3, 3.48 mg/m3, and 2.59 mg/m3 respectively.

evelop and implement a remedial action plan utilizing Soil Vapor Extraction to remove chlorinated solvent contaminants from the vadose zone. PSI additionally opined that to enhance air flow from depth toward the surface, remedial action should include the ability to allow ambient air into deep and intermediate MAGS wells. Further investigation into the potential preferential pathways as a result of use should be undertaken during SVE remedial activities should an SVE system be installed.

PSI, therefore, has developed this scope of work, cost estimate and level of effort to prepare an ISR Plan consisting of SVE remedial technology.

PROPOSED SCOPE OF WORK

This proposal consists of preparing the Interim Source Removal Plan (Soil Vapor Extraction) at the referenced site.

Specifically, the proposed scope of work consists of the following subtasks:

1. Project Management

This scope is provided for in the template worksheets.

2. Interim Source Removal Plan (ISR Plan)

The design of the ISRP will address VOH concentrations in the soil and will not directly address groundwater quality. The presumptive remedy for remediation of impacted soil at the former Fashion Cleaners is soil vapor extraction, based on previous experience, feasibility, implementability, reliability, operations and maintenance (O&M) requirements, and cost. PSI will also coordinate with the building tenant, property management company, and the property owner for suggested locations of piping, lines, and equipment compound.

Calculations will be generated to determine the design, construction details and specifications of the system (number of SVE wells, radius of influence, estimated time to cleanup, etc). Figures will include site plan and trench details, SVE cross sections, treatment system layout, piping and instrumentation diagrams (P&ID), electrical detail, equipment list, and any other components of the remediation

PSI Proposal: 0552-427417 Rev 1 FDEP Fashion Cleaners ERIC_5245

Jully 17, 2024 www.intertek.com/building system as necessary. The ISRP will evaluate and provide recommendations on the use of the existing equipment available from other sites.

The SVE design will include a summary of the MAGS survey. The report will include pertinent information to include a detailed description of the remedial system, operation details, start-up testing, site schedule for O&M, tables summarizing proposed samples and analysis, and itemized cost estimates for project capital, and the implementation schedule. The design will be signed and sealed by a Florida Registered Engineer in accordance with Chapters 471, Florida Statues. The report will incorporate specific recommendations and provide opinions based on professional experience in achieve aggressive contaminant reduction in an efficient and cost-effective method.

PSI will review the available Bureau of Waste Cleanup remediation equipment from FDEP-owned equipment list and select an SVE system that will meet the design criteria of the Interim Source Removal Plan.

COST ESTIMATE AND SCHEDULE

The estimated cost to prepare the ISRP is shown on the attached Cost Estimate and Level of Effort. All PSI services will be conducted in accordance with the terms and conditions contained within the terms and conditions contained within DEP Contract No. HW686, Waste Cleanup Program Services, dated December 21, 2021. An invoice and deliverable schedule is presented as follows.
