About this file

This is a Task Assignment Change Order Form (CO 7) issued by the Florida Department of Environmental Protection (FDEP) for a contract with Taylor Engineering, Inc., a consulting engineering firm based in Jacksonville, Florida. The change order modifies an existing indefinite delivery contract (CN335) for the Silver Springs State Park Improvements project, specifically Task 22 related to design and permitting of bathroom facilities and utilities. The project is located at Silver Springs State Park in Florida, with FDEP's Project Manager Theresa Carron and Contract Manager Ralph Perkins overseeing the work. The change order was executed on October 17, 2024, with all required signatures from budget, project management, contract management, consultant, and contract authority representatives.

The change order adds $6,068.60 in fixed fee costs to cover professional and general liability insurance renewal expenses for SAVANNAPUB, Inc., FDEP's architectural subconsultant, attributable to unanticipated project delays caused by permitting issues with the St. Johns River Water Management District and U.S. Army Corps of Engineers. The insurance costs consist of $2,768.60 for 2025 coverage ($2,181.60 professional liability and $587.00 general liability) and $3,300.00 for 2026 coverage ($2,600.00 professional liability and $700.00 general liability). The change order extends the task end date by 576 days from the originally scheduled June 30, 2022, completion date to July 28, 2026. The total task amount with all change orders is $444,748.60, with a cumulative fixed fee of $437,130.60 and reimbursable expenses of $7,618.00.

RE: FDEP Agreement Number CN335 Silver Springs State Park Improvements Design, Permitting and Bid Administration Task 22 – Design and Permitting of Bathroom Facilities and Utilities

Dear Ms. Carron:

As you know, the Silver Springs Swimming Area project has encountered significant delays due to permitting issues with the St. Johns River Water Management District and U.S. Army Corps of Engineers. As a result, the project is still ongoing after the originally anticipated completion date. Our architectural subconsultant, SAVANNAPUB, Inc, is a small business and had to purchase specific Professional and General liability insurance policies to meet the insurance requirements for this project. Due to the unanticipated delays, SAVANNAPUB is requesting a contract mod as listed below, to cover the costs of renewing these policies. Please see the attached memo from SAVANNAPUB for additional information.

Task 22 – Project Specific Insurance

Firm Name Item Unit Rate Quantity Total Fixed Price Cost

Task 22a – Project Specific Insurance 2025 SAVANNAPUB, INC. Professional Liability $2,470.00 1 $2,181.60 SAVANNAPUB, INC. General Liability $553.00 1 $587.00

Subtotal: $2,768.60 Task 22b – Project Specific Insurance 2026 SAVANNAPUB, INC. Professional Liability $2600.00 1 $2,600.00 SAVANNAPUB, INC. General Liability $700.00 1 $700.00

Subtotal: $3,300.00 Task 22 Total $6,068.60

Deliverable: Insurance certificates documenting proper coverage.

Please let me know if you have any questions.

Sincerely, Terence Cake, P.E.

Subject: Restroom Project – General Liability Insurance

Dear Terry, Our Professional Liability insurance coverage is what we have had for years. The General Liability policy was taken out only for the Silver Springs Restroom project. Currently, none of our other projects and clients have required General Liability. Therefore, the coverage continues only for our Silver Springs project

SAVANNAPUB, INC, is a small company with a minor fee for the Silver Springs project. The project literally should have taken a few months to design and permit and that is how we structured our fee. In fact, we were asked to join the team because the previous architect’s fees were too high. We carried the cost of the insurance as our personal overhead to work on the project, yet we are continuing to pay for the insurance that is not being utilized because the project has been on hold for quite a while.

We cannot drop our Professional Liability and continue with the General Liability for Silver Springs since we still need if for all our other projects including the Silver Springs project.

Therefore, as we renew the Professional and General Liability policy, we request a contract modification of $587 for the 2025 General Liability, 2025 Professional Liability at $2,181.60, 2026 General Liability at $700, and 2026 Professional Liability at $2600 to hold these insurances for the Silver Springs project. A total insurance for 2025 and 2026 of $6,068.60

Respectfully, Audie Robinson, NCARB, LEED® AP, DBE, CFRM Director
