About this file

This is a Task Assignment Change Order Form (Change Order No. 3) issued by the Florida Department of Environmental Protection (FDEP) for a contract with Taylor Engineering, Inc. The document modifies Task Assignment No. TA14 under FDEP Contract No. CN335, Project No. R2236, for the Sea Level Impact Projection (SLIP) Study. The change order was executed on November 29-30, 2021, and is signed by representatives from FDEP (Eddy Bouza, Project Manager; Jeffrey Whitfield, Contract Manager; and Lisa Yarborough, Contract Authority) and Taylor Engineering (Angela Schedel, Consultant and Vice President, Community Resilience).

The change order addresses Task 7, which requires Taylor Engineering to provide analysis, design, and technical documentation for the SLIP Web Application Service, including system documentation, a licensing agreement, and copyright submission. The net decrease in task amount is $4,209.61, resulting in a revised total task cost of $73,700.55. Labor costs increased by $3,209.61 due to additional meetings and document revisions related to copyright deliverables, from 113 to 120 man-hours. Conversely, licensing agreement fees were reduced from $7,500 to $3,000 because Taylor Engineering will perform this work in-house rather than engaging external attorneys. An additional $1,000 reduction corrects a calculation error from Change Order 2. The task execution date remains September 27, 2021, with the original end date of June 30, 2022. Funding is allocated under Florida FLAIR Code 3710100028637500400001005930, with a total project allocation of $73,700.55.

RE: FDEP Agreement Number CN335 TA14 Change Order 3

Sea Level Impact Projection (SLIP) Study

Website Design and Web Application Development

Dear Mr. Whitfield:

As requested by FDEP, Taylor Engineering is submitting a change order for Task 7 of the above referenced project. Justification for the change order is as follows:

1. One of the Task 7 deliverables, copyright documentation, was unable to be completed by the due date of November 15, 2021 due to delays in approving the copyright by the FDEP reviewers.

We requested a 30 day extension in order to finalize the documentation for submittal in a previous change order, and this change order is to reconcile cost differences after meeting following the execution of change order 2.

2. The increase in number of man hours from 94 to 120 and associated labor cost of $3,209.61 is due to additional meetings, document review, and document revisions for the copyright deliverable. The Licensing Agreement Fees are reduced from $7,500 listed in change order 2 to

$3,000, since Taylor Engineering will be performing this work instead of their attorneys.

3. An additional $1,000 reduction is being made from the task assignment total amount due to a calculation error during the processing of change order 2. Change order 2 added an additional

$8,635.84 to the task assignment, though the costs being added only added up to $7,635.84.

This $1,000 discrepancy is being addressed in this third change order.

Please let me know if you have any questions.

Sincerely, Angela Schedel, Ph.D., P.E.

Vice President, Community Resilience
